인식이란? | 인식이란 기억과 경험을 통합하여 현상을 이해하는 개인 혹은 집단 특유의 관점[8]으로 질병인식은 질병에 대한 개인의 믿음이나 기대, 해석을 의미하며 인지적 측면과 감정적 측면으로 구분된다 [9]. Leventhal 등[9]에 의하면 인지적 질병인식은 증상이나병명에 대한 명명(identity), 질병이 예상되는 기간(timeline), 질병에 대한 원인 (cause), 질병으로 인하여 삶에 미치는 결과(consequence), 질병에 대한 치료와 개인의 통제력(controllability)으로 통합되고, 감정적 질병인식은 불안, 두려움, 디스트레스와 같은 부정적인 감정과 반응으로 나타난다. | |
혈액투석 환자의 질병인식이 부정적이 아니라 긍정적이여야 하는 이유는? | 혈액투석 환자의 질병인식은 약물복용이나 수분조절과 같은 건강행동뿐만 아니라 우울 및 삶의 질과도 관련된다[11]. 치료나 자신의 행동이 질병을 통제하고 상태를 호전시킬 수 있다고 믿는 질병에 대한 통제력 인식은 혈액투석 환자의 우울과 음의 상관관계가 있으며[12], 분노, 불안, 귀찮음으로 경험되는 부정적인 감정적 질병 인식은 아무런 계획이나 행동을 하지 않는 결과를 초래하며 삶의 질과는 음의 상관관계가 있다[13]. 혈액투석 환자와 같이 완치될 수 없는 질병을 가진 환자의 경우 사건의 심각성을 긍정적 시각에서 가볍게 다루거나 상황의 상대성을 강조하는 인식의 변화와 대처는 우울을 감소시키고 삶의 질을 개선하기 위한 효과적인 전략이 될 수있기때문이다[14]. |
