Purpose: We developed and tested the effects of a coping skill training program for caregivers in feeding difficulty among older adults with dementia in long-term care facilities. Methods: A non-equivalent control group pretest-posttest design was used. The subjects comprised 34 caregivers (experimental group: 17, control group: 17) and 40 older adults with dementia (experimental group: 20, control group: 20). The developed program was delivered in 4-hour sessions over 6 weeks (including 2 weeks of lectures and lab practice on feeding difficulty coping skills, and 4 weeks of field practice). Data were collected before, immediately after, and 4 weeks after the program (January 3 to April 6, 2016). The data were analyzed using t-test and repeated measures ANOVA using SPSS/WIN 20.0. Results: Compared to their counterparts in the control group, caregivers in the experimental group showed a significantly greater improvement in feeding knowledge and feeding behavior, while older adults with dementia showed greater improvements in feeding difficulty and Body Mass Index. Conclusion: The study findings indicate that this coping skill training program for caregivers in feeding difficulty is an effective intervention for older adults with dementia in long-term care facilities.

제안 방법

돌봄제공자의 급식행동은 Chang [15]이 개발한 Formal Caregivers’ Behaviors toward Feeding Dementia Patients Observation Checklist를 연구자가 연구목적에 맞게 자가보고형 도구로 수정한 후 노인환자 간호 분야에서 연구를 수행해 온 간호학 교수 1인에게 내용타당도 검증을 받은 도구로 측정하였다. 본 연구에서 급식행동 도구를 수정한 이유는 본 연구의 예비실험 시 요양보호사가 자신의 급식행동 수행 업무를 타인이 관찰하는 것을 원하지 않아 자가보고형으로 수정한 것이다.
식사행동장애 대처기술훈련 프로그램은 치매노인에 대한 식사행동장애 대처전략[18], 요양시설 간호제공자를 대상으로 훈련 프로그램을 개발하여 효과를 검증한 선행연구[13-15,22] 등을 기반으로 연구자가 개발하였다. 프로그램은 식사행동장애 대처기술훈련 프로그램에 참여한 요양시설의 치매노인 돌봄제공자인 요양보호사의 급식지식, 급식태도, 급식행동을 증진시키고 그 결과, 치매노인의 식사 행동장애가 감소하고 영양상태가 증진되도록 하는 것을 목적으로 하여, 총 6주간 6회기 4시간의 강의와 실습교육, 현장실습교육으로 구성되었다.
식사행동장애는 Watson [16]이 개발한 Edinburgh Feeding Evaluation in Dementia (EdFED) Scale을 Hong과 Gu [19]가 원저자로부터 도구사용 허락을 받아 번안하고 수정하여 타당도와 신뢰도를 검증한 도구를 저자의 허락을 받아 연구자가 관찰도구로 수정하여 사용하였다. 원 도구는 돌봄제공자가 자신들이 돌보는 치매노인들의 식사행동장애 정도를 평가(자가보고)한 도구였는데 본 연구에서 관찰도구로 수정하여 사용한 이유는 치매노인들의 식사행동장애를 돌봄제공자의 평가에만 의존하는 것보다 다중 관측이나 제 3의 평가자가 평가하는 것이 바람직하다는 선행연구[13]에 따라 본 연구에서는 연구자와 연구보조원이 관찰로 측정하기 위한 것이었다.

대상 데이터

실험군 요양보호사와 치매노인은 C시에 소재한 J요양시설，대조군은 S요양시설에서 각각 임의표집으로 선정하였다. 대조군 요양시설은 요양시설의 노인대비 요양보호사 구성비, 요양보호사 근무형태 등이 실험군 요양시설과 유사하면서, 실험군과 교류가 없는 지역적으로 멀리 떨어진 시설이다.

데이터처리

넷째, 식사행동장애 대처기술훈련 프로그램에 대한 효과 검증은 Repeated measures ANOVA로 분석하였다. Repeated measures ANOVA 분석 시 필요한 연구변수의 정규분포, 분산의 동질성은 모든 연구변수에서 충족되었으나 복합대칭(구형성) 가정이 급식태도와 체질량지수 변수에서 충족되지 못하여 이들 변수는 Greenhous-Geisser로 교정하여 단일 변량값을 제시하였다.
둘째, 실험군과 대조군 간의 대상자의 특성, 연구변수에 대한 동질성 검증은 Chi-square test, Fisher’s exact test, t-test를 이용하여 제시하였다.
셋째, 실험군과 대조군의 연구변수의 정규성 검증은 Shapiro-Wilk test를 이용하였으며, 모든 변수는 정규분포를 따르는 것으로 나타났다.
