Objectives: The purpose of this study is to identify the status and opinions of professional oral health care performance in oral cancer patients. Methods: Seven National University Dental Hospitals and the National Cancer Center in Korea surveyed the medical personnel in charge of oral health care for oral cancer patients. The questionnaire consisted of 16 questions, including the status of expert oral health care education and performance for oral cancer patients, etc. A total of 47 questionnaires were retrieved, and the collected data were used in PASW Statistics 23.0 to perform frequency analysis, cross-analysis, and kruskal Wallis tests. Results: A survey of oral health care education found that 29.8% of the medical personnel had received education. The most performed of professional oral health care was found that applying fluoride by dental hygienist, treating stomatitis by dentist, and wiping mouths with sponge by nurse. The assessment of self-performance has shown that applying fluoride by dental hygienists has a statistically significant difference. Only 37.8% of oral health care education for patients was conducted. Most recognized that oral health care in oral cancer patients was important, but it is difficult to provide care due to lack of performance personnel and time, opportunities for performance personnel to be educated. The improvement was found to require an increase in the number of performance personnel, placement of professionals, and practical training to enhance performance capabilities. Conclusions: For oral health care of oral cancer patients, continuous education for medical personnel, establishment of oral health care manual and medical system, research on oral health care of oral cancer patients and public relations campaign will have to be activated. Development of expert care manual analysis and evaluation tools for oral health care in oral cancer patients in the future and development of standardized curriculum will be necessary.

이에 본 연구에서는 국립암센터 구강종양클리닉센터와 전국 11개 치과대학병원 구강악안면외과에서 구강암 환자를 위해 전문가구강건강관리를 수행하고 있는 의료 인력을 대상으로 하여 구강암 환자에 대한 전문가 구강건강관리 수행 현황 및 인식을 파악하고자 한다.
제안 방법
전국 11개 치과대학병원 구강악안면외과와 국립암센터에서 수행하고 있는 구강암 환자의 전문가 구강건강관리 수행 현황 및 인식을 전수조사 하기 위해, 사전 전화 설문을 실시하여 병원 내 구강암 환자를 위한 전문가 구강건강관리 등을 수행하고 있는 담당자 유무와 해당 직업군을 확인하였다. 일부 치과대학병원의 경우 구강암 환자의 관리를 일반 의과 병동으로 의뢰하거나, 병원의 내부규정에 의해 설문 회신이 불가함에 따라, 총 7개 치과대학병원과 국립암센터에서 구강암 환자의 구강건강관리를 담당하는 치과위생사를 포함한 관련 의료 인력을 연구대상자로 선정하였다.
대상 데이터
본 연구에서는 전국 치과대학병원 구강악안면외과와 국립암센터의 홈페이지에서 연락처와 우편주소를 확인하여 담당자 앞으로 우편을 발송하였고, 회수율을 높이기 위해 전화통화로 설문 참여를 독려하여 2019 년 1월 3일부터 2월 20일까지 배부한 설문지 총 58부 중 47부를 회수하였다.

Chicago, IL, USA) 프로그램을 사용하여 다음과 같이 분석하였다. 연구대상자의 일반적 특성에 따라 구강암에 대한 교육수혜여부, 환자 및 보호자 대상 교육여부, 전문가 구강건강관리 중요도의 연관성을 확인하기 위하여 교차분석을 실시하였다. 구강암 환자의 구강건강관리를 위한 전문가 구강건강관리 교육 현황 및 참여 의사, 전문가 구강건강관리 수행에 대한 인식 및 견해조사, 그리고 구강암 환자의 전문가 구강건강관리를 위한 구강관리용품 사용 현황과 구강암 환자 및 보호자 대상의 구강건강관리교육 현황, 전문가 구강건강관리 교육 내용과 경로 및 수행주체에 대한 견해조사는 빈도분석을 실시하였다.
구강암 환자의 구강건강관리를 위한 전문가 구강건강관리 교육 현황 및 참여 의사, 전문가 구강건강관리 수행에 대한 인식 및 견해조사, 그리고 구강암 환자의 전문가 구강건강관리를 위한 구강관리용품 사용 현황과 구강암 환자 및 보호자 대상의 구강건강관리교육 현황, 전문가 구강건강관리 교육 내용과 경로 및 수행주체에 대한 견해조사는 빈도분석을 실시하였다. 전문가 구강건강관리에 대한 연구대상자의 자가 수행능력정도는 빈도분석을 실시한 후 Kolmogorov-Smirnov과 Shapiro-Wilk 검정을 실시하였고, 정규성을 띄지 않는 것으로 나타나 Kruskal Wallis 검정을 실시하였다.
성능/효과
1. 의료인력의 전문가 구강건강관리 교육 현황을 조사한 결과, 29.8%가 교육을 받아본 적이 있는 것으로 나타났으며, 교육받은 내용으로는 구강암 환자의 구강건강관리방법, 구강암 발생원인과 치료법, 치료 중·후 구 강 내 합병증 등의 순서로 나타났다.
2. 전문가 구강건강관리 수행 현황을 직업별로 분석한 결과, 치과위생사는 ‘불소도포’, 치과의사는 ‘구강 내 상처 및 구내염 처치’, 간호사는 ‘스펀지, 거즈봉으로 입 안 닦아내기’가 가장 높게 나타났다. 자가 수행능력에 대한 평가로는 ‘불소도포’가 직업군에 따라 통계적으로 유의한 차이가 있었으며 치과위생사가 가장 높게 평 가하는 것으로 나타났다.
3. 구강암 환자의 구강건강관리를 위해 의료인력이 사용하고 있는 구강관리용품의 현황을 확인한 결과, 생 리식염수, 클로르헥시딘, 거즈 등의 순으로 나타났다. 환자 및 보호자 대상 구강건강관리교육 여부는 37.
후속연구
구강암 환자의 구강건강관리를 위해서는 의료인력의 지속적인 교육 강화, 구강건강관리 매뉴얼 구축, 의료체계 구축방안 등이 모색되어야 할 것이며, 구강암 환자의 구강건강관리에 대한 연구 및 대국민 홍보 캠페인 등이 활성화 되어야 할 것이다. 추후 구강암 환자의 구강건강관리를 위한 전문가 관리 매뉴얼 분석 및 평가 도구의 개발 및 표준화된 교육과정 개발이 필요할 것이다.
하지만 구강암 환자를 관리하고 있는 전문 의료인력을 대상으로 구강암 환자의 전문가 구강건강관리 수행실태와 인식에 대한 연구라는 점에서 의의가 있으며, 구강암 환자 전문가 구강건강관리의 구체적인 실현방안을 모색하는데 필요한 기초자료로서 중요성이 있다. 추후, 조사대상을 확대하고 구강암 환자의 전문가 구강건강관리를 좀 더 객관적으로 평가할 수 있는 전문화된 연구 도구가 마련되어야 할 것이며, 임상에서 사용되고 있는 구강암 환자 구강건강관리 매뉴얼 분석 및 활용방안에 대한 연구, 구강암에 대한 이해와 구강암 환자 관리 등과 관련된 이론 및 실무에 대한 교과목 운영 및 표준화 방안에 대한 연구가 이루어져야 할 것이다.
