Purpose: This descriptive research was on hand dermatitis in clinical nurses. Nurses often have a high rate of hand dermatitis with several factors associated with the dermatitis. The factors analyzed in this study included; general characteristics, work-related factors, hand washing, and allergies relate to hand dermatitis. Methods: Data were collected from April to May 2016. The subjects were nurses who had worked 12 months or more in a University Hospital. They voluntarily gave their consent for the research. The Symptombased questionnaires were sent to 220 nurses and 199 responses were returned. Collected data were analyzed using frequency, percentage, mean, standard deviation, ${\chi}^2-test$, and logistic regression analysis using SPSS 23.0. Results: Prevalence of dermatitis in nurses was 48.2%. Nurses who had worked for over three years were twice more likely to suffer hand dermatitis than nurses who had worked for 3 years or less (adjusted odds ratio [adj. OR]= 1.97, p= .036). Nurses who used alcohol sponge swabs more than 20 times per shift were 2.5 times more likely to suffer hand dermatitis as compared to those who used them less times (adj. OR = 2.51, p= .023). Conclusion: Results revealed that about a half of the clinical nurses suffered from hand dermatitis, hence it is crucial to find appropriate interventions for dermatitis prevention. This concern needs to be addressed especially in nurses who have more work experience and use more alcohol sponge swabs.

대상 데이터

본 연구의 대상자는 원주시 소재 W종합병원에 근무하는 간호사로 선정기준은 임상경력이 12개월 이상이고 본 연구의 목적을 이해 하고 연구 참여에 자발적으로 동의한 자이며 제외기준은 연구 참여에 동의하지 않은 자, 입원환자가 없는 특수부서에 근무하는 간호사로 하였다. 연구 대상자의 수는 G*power program 3.
설문조사는 2016년 4월부터 5월까지 연구자가 각 병동을 직접 방문하여 각 병동 파트장에게 연구 목적 및 설문내용을 구두로 설명하고 각 대상자 의 참여는 대상자가 자발적으로 결정하였으며 연구 참여중 언제라도 원하지 않으면 철회할 수 있고 익명보장과 연구의 목적 외에는 사용하지 않는다는 것을 알린 후 서면 동의서에 스스로 서명한 대상자에게 설문지를 배부하였다. 설문지는 총 220명에게 배부하였고 199부를 분석에 사용하였다.

데이터처리

대상자의 일반적 특성, 업무관련특성, 손위생 특성, 알러지 특성을 빈도분석을 통해 실수와 백분율로 산출하였고, 각각의 특성에 따 른 손피부염 유무는 χ² -test를 이용하여 분석하였다. χ² -test에서 유의 한 값을 나타내는 특성을 독립변수로 하고 손피부염을 종속변수로 하여 단변량 로지스틱 회귀분석을 시행하였으며, 최종적으로 단변량 분석에서 유의하게나온 요인들을 독립변수로하여 다변량 로지스틱회귀분석을시행하였다.

이론/모형

손 피부염 도구로는 Smit 등[1]이 개발하고 Jeon [4]이 번역하여 사용한 증상기반 손피부염 진단도구를 사용하였다. 도구 개발자인 Smit에게 E-mail을 통해, 번역자에게 전화통화로 도구사용에 대한 허락을 받았다.

성능/효과

본 연구는 자가보고형인 증상기반 손피부염 도구를 이용하여 일 병원 간호사의 손피부염 유병률과 증상을 알아보고 이에 영향을 미치는 요인을 규명하고자 시도한 서술적 조사연구이다. 간호사 손 피부염 유병률은 전체 대상자 199명 중에 96명으로 48.2%를 차지하 였으며 중환자실과 일반병실의 손 피부염 유병률은 통계적으로 유 의한 차이가 없었다. 간호사 임상경력이 3년 초과인 경우 3년 이하인 경우보다 손피부염 발생 위험이 2.
0배 높았다. 또한 하루 근무 중 알코올 솜을 20회 이상 사용하는 경우 20회 미만 사용하는 경우보다 손 피부염 발생위험이 2.5배높았다.
본 연구결과 근무 중 알코올솜 사용 횟수가 손피부염과 관련이 있는 것으로 나타났다. 알코올솜을 20회 이상 사용하는 경우 그렇지 않은 경우에 비해 2.5배 손 피부염의 발생이 높게 나타나 알코올 솜 또한 손피부염의 위험요인이 됨을 알 수 있었다. 이러한 결과는 일본 간호사에서 알코올 사용으로 손피부염이 4.

후속연구

이러한 결과는 경력이 높을수록 손피부염 위험요인에 오랜 시간 노출되어 손 피부염의 발생이 높아 진 것으로 해석된다. 그러나 나이가 증가할 수록 손피부염 위험이 0.95배[5]이고, 연령과 손피부염이 역상관을 나타내며[21], 젊은 연령 에서 손피부염 발생이 높았다는[9,21] 결과와는 상반된 결과이므로 간호사 경력이 손피부염에 미치는 영향은 반복연구를 통하여 규명 될 필요가있겠다.
이러한 결과는 아직도 손피부염 영향요인으로 일관된 결과가 보고되지 못한 가사시간[5], 보호장갑 이용[15,18], 물이용[15] 손씻기 횟수[4,15,18], 보습제사용과 알러지[5,15] 등의 연구와는 유사한 결과이다. 본 연구는 일 병원간호사를 대상으로 한 연구이므로 확대해석할 수 없고, 아직 국내 간호사 전체를 대상으로 한 연구가 수행되지 않았으므로 국내에서 전체 간호사의 대표성을 확보한 연구가 수행되면 간호사의 손피부염에 대한 정확한 실태와 영향요인이 규명될 수 있을것이다.
