Backgrounds : The market of Complementary Alternative Medicine(CAM) in the United State(U.S.) accounts for a large proportion of the global CAM market and has a high growth rate. The recent introduction of Obama Care has brought the change in the health insurance system for CAM, and we need to analyze it for its implication to Korean system. Objectives : The purpose of this study is to investigate the current status of acupuncture and chiropractic health insurance in the U.S., and to draw implications for expanding the health insurance coverage for Korean traditional medicine through the comparison between the U.S. and Korean health insurance systems. Methods : We examined the data through the literature search and from the websites of both U.S. government departments and related organizations for the health insurance policy. Based on the collected data, we analyzed its CAM health insurance system in Korea. Results : The acupuncture covered by public health insurance in the U.S. has a limit in the number of treatments and a range of applied diseases compared with Korea. In addition, the practice of acupuncture is not subdivided. However, the chiropractic in the U.S. which also has a limited number of coverage and only three categories of practices are similar to that of Korea. Conclusions : Although the use of CAM by public health insurance is not active in the U.S., but the organizations such as Veterans Health Administration in Vermont is already discussing the use of acupuncture to solve the problem of opioid overuse. Thus Korea also needs to discuss to promote the expansion of the insurance system for CAM.

제안 방법

본 연구는 미국의 건강보험제도 관련 문헌 검색과 건강보험 관련 정부 부처, 유관 기관, 연구기관 홈페이지를 통해 2월 18일부터 3월 27일까지 자료를 조사하였다. 그리고 문헌 및 자료 조사와 연계하여 파악한 내용을 확인하고 추가 필요자료를 획득하기 위해 미국 현지조사원을 통한 자료조사와 관련 전문가들에게 인터뷰를 수행하여 내용을 확인하였다. 마지막으로 수집된 모든 자료를 체계적으로 항목을 구성하여 분석 후 고찰하였다.
그리고 문헌 및 자료 조사와 연계하여 파악한 내용을 확인하고 추가 필요자료를 획득하기 위해 미국 현지조사원을 통한 자료조사와 관련 전문가들에게 인터뷰를 수행하여 내용을 확인하였다. 마지막으로 수집된 모든 자료를 체계적으로 항목을 구성하여 분석 후 고찰하였다. 자료조사를 수행한 미국 내 주요 기관은 다음과 같다.
. 메디케이드 수혜자 중 최근 3개월 이내 30일 중 15일 이상 통증을 경험한 환자 156명을 대상으로 환자결과보고(Patient-Reported Outcomes Measurement Information System, PROMISE)방식의 설문지를 활용하여 침술 효과 등을 측정하였으며, 60일 동안 28명의 공인된 침구사들이 환자당 평균 8.17회 침술을 시행하였다. 침술효과를 측정하기 위해 측정항목을 통증 강도와 장애, 신체기능, 피로, 불안, 우울증, 수면장애, 사회적 격리 등 8개 영역으로 구분하여 침술 전후를 비교하였다.
이를 알아보고자 미국에서 침술사 면허를 발행하는 45개 주 중 침술사가 많고 침술을 보험으로 인정하는 3개주 워싱턴, 메릴랜드, 뉴멕시코주 선택하여 대표적 민간보험회사 상위 3∼5개사에서 제공하는 침술 보장 플랜을 추출하였다.
17회 침술을 시행하였다. 침술효과를 측정하기 위해 측정항목을 통증 강도와 장애, 신체기능, 피로, 불안, 우울증, 수면장애, 사회적 격리 등 8개 영역으로 구분하여 침술 전후를 비교하였다. 침술 전후를 비교한 결과 사회적 격리 항목을 제외하고는 7개 영역에서 통계적으로 유의미한 개선을 보였다.

침술 전후를 비교한 결과 사회적 격리 항목을 제외하고는 7개 영역에서 통계적으로 유의미한 개선을 보였다. 오피오이드를 복용 한 환자의 57%가 사용량이 감소하였으며, 환자의 74%가 일할 수 있는 능력이 향상되었으며, 만성 통증에 대해 다른 사람들에게 침술을 권유한다고 응답한 환자는 96%였다. 마약성 진통제인 오피오이드 대신 침 치료를 받은 환자가 신체적, 정신 감정적 개선에 치료효과 뿐만 아니라 업무 생산성 향상이라는 결과를 얻어 현재 메디케이드에 침 치료 포함 여부에 대해 논의가 진행 중이다.
침술 보장 플랜을 조사한 결과, 공통적으로 침술이 보장되는 주요 질환은 주로 만성통증(chronic pain)과 오심(nausea)이었으며, 보장 횟수는 1년에 약 10∼25회로 제한되고 월평균 1∼2회였다.

후속연구

우리나라는 아직 마약성 진통제에 대한 남용 문제가 붉어지지는 않고 있으나 만성 통증질환 환자는 증가하고 있는 상황이다. 단순히 약물대비 비용효과성 만으로 급여 포함여부를 결정하지 않고 부작용이 없는 비약물요법으로 한의학 치료를 급여에 활용할 것을 제안할 필요가 있을 것으로 보인다.
그러나 이러한 표준화는 전통적인 한의 임상과는 차이가 있어 부작용이 발생할 수 있는데, 독일에서는 침이 표준화된 행위로 급여화 되면서 급여 적응증 이외의 환자 증상에 대해 소홀하게 되거나 표준화된 침술이 효과가 없을 때 다른 노력을 하지 않게 되는 등 부작용을 발생하였다고 알려져 있다⁴¹⁾. 따라서 우리나라에서도 향후 첩약 등이 급여화 될 때 상병과 진료 방식에 대한 여러 표준화와 제한 등을 어떻게 대처할지에 대한 준비와 논의가 필요할 것으로 보인다.
