Objectives The purpose of this study is to analyze the trends of Chinese clinical research in the last 5 years of Chinese herbal medicine treatment for anorexia in children, and to find ways to utilize the diagnostic criteria and herbal medicine treatment for children who have anorexia in Korea. Methods The authors searched literatures using China National Knowledge Infrastructure published from January 1, 2015 to June 18, 2019. Selected literatures were collected in order of recent publication year. And then, demographic data, duration of illness, intervention, treatment period, outcome, and adverse events were analyzed. In addition, the diagnostic criteria for anorexia in children are categorized and presented in chart. Results A total of 50 papers were included in the analysis of this study. In the selected studies, the most commonly used diagnostic criteria for anorexia in children were ${\ll}$Zhu Futang Practice of Pediatrics${\gg}$ in Western medicine and ${\ll}$Diagnostic and therapeutic criteria for TCM syndromes${\gg}$ in traditional Chinese medicine. As a result, in all the studies reporting total efficacy, the total effective rate of the treatment group treated with herbal medicine was significantly higher than that of the control group. Also, appetite recovery rate and weight gain were significantly higher, gastric emptying time and appetite recovery time were significantly shorter in the treatment group. Conclusions Herbal medicine treatment can help improve anorexia in children. In Korea, diagnostic criteria for anorexia in children should be established. In addition, criteria should be established to evaluate the effectiveness of herbal medicine treatment.

제안 방법

선정된 문헌은 최신 출판연도 순으로 자료를 수집하였다. 각 문헌별 연구 대상의 인구통계학적 정보, 유병기간, 치료방법, 치료기간, 평가 지표 및 평가 결과, 안전성 평가에 관한 정보를 추출하였다 (Table 1, 2). 한약치료에 사용된 한약 처방의 구성은 도표로 제시하였으며 (Table 3), 그 중 빈용된 한약재를 별도로 제시하였다 (Table 4).

1. 문헌 검색

검색원으로는 중국의 전자 데이터베이스 검색 사이트인 중국학술정보원 (China National Knowledge Infrastructure, CNKI)을 이용하였다. 검색범위는 醫藥衛生科技에서 中醫學, 中藥學, 中西醫結合에 한정하였으며, 검색어는 ‘小儿厌食症’ and ‘中药’ or ‘中医药’ or ‘颗粒’ or ‘胶囊’ or ‘自拟’ or ‘汤’ or ‘丸’ or ‘散’ or ‘方’으로 설정한 후, 중영문 교차 검색 및 결과 내 검색을 시행하였다.
으로 다양하였다. 모든 연구는 14세 이하의 소아를 대상으로 시행되었으며, 그 중 6세 이하의 소아만을 대상으로 한 연구가 15편^{14,15,17,18,20,21,25,35,36,41,43,51,53,54,58)}이었다.
. 본 연구에 포함된 한약 처방들을 분석한 결과, 50편의 연구에서 총 37개의 처방들이 활용된 것으로 나타났다. 비록 처방명이나 구성 약재들에는 연구마다 차이가 존재했지만, 10편의 연구를 제외하고 나머지 40편의 연구에서 모두 補氣劑와 消導劑가 들어간 처방을 활용한 것으로 나타났다.

선정된 연구^14-63)는 1편의 연구를 제외하고 모두 치료군과 대조군의 양 군으로 이루어졌으며, 세 군으로 이루어진 연구가 1편¹⁷⁾이었다. 이 중 한약치료군과 양약치료군을 비교한 연구가 20편^{18,22-24,27,31,33,34,38,43,48,51-54,56,57,60,62,63)}, 한약치료 및 양약치료 병용군과 양약치료군을 비교한 연구가 16편^{14,16,19,28,30,32,35,37,41,45-47,49,50,55,61)}, 한약치료 및 생활습관 교정군과 양약치료 및 생활습관 교정군을 비교한 연구가 6편^{21,26,29,40,44,59)}, 한약치료 및 양약치료 및 생활습관 교정군과 양약치료 및 생활습관 교정군을 비교한 연구가 6편^{15,17,20,39,42,58)}이었으며, 그외 통상치료법으로 철분제, 비타민 섭취 등을 포함하여 한약치료 및 양약치료를 병용한 치료군과 통상치료법 및 양약치료를 시행한 대조군 연구가 1편³⁶⁾, 통상치료법과 생활습관 교정을 포함하여 한약치료 및 양약치료를 병용한 치료군과 통상치료법 및 생활습관 교정 및 양약치료군을 비교한 연구는 1편²⁵⁾이었다.
식욕부진이 있는 소아에게 한약치료를 시행하고 치료효과를 평가한 중국의 임상연구 중에서 무작위 배정 대조군 연구만을 선별하였다. 한약은 경구 복용하는 경우로 한정하였으며, 한약 이외에 鍼, 灸, 貼敷, 推拿, 耳壓法 등 다른 한의학적 치료를 병행한 경우 제외하였다.
한방소아과학에 전문 지식이 있는 저자들이 1차로 제목과 초록을 검토하여 869편의 문헌이 제외되었고, 최종적으로 100편의 문헌이 포함되었다. 이 문헌들에 대하여 전문 검토를 시행한 결과, 선정기준에 맞지 않는 50편의 문헌이 제외되어 최종적으로 50편의 문헌이 본 연구의 분석대상으로 선정되었다 (Fig. 1).

성능/효과

1. 총유효율을 보고한 47편의 연구에서 모두 한약치료를 시행한 치료군의 총유효율이 대조군에 비해 유의하게 높았으며, 식욕 회복률, 체중 증가가 유의하게 높았고, 위 배출시간 및 식욕 회복시간은 유의하게 단축되었다.
2. 소아의 식욕부진을 진단하는 데 가장 많이 활용된 진단기준은 서양의학 서적인 ≪諸福棠實用兒科學≫의 진단기준과 중의학적 서적인 ≪中醫病證診斷療效標準≫의 진단기준이었다.
3. 평가 지표의 경우, 환아의 정상 식사량 회복 정도를 평가하여 총유효율을 계산한 경우가 가장 많았으며, 중의증상점수를 활용한 경우가 그 다음으로 많았다. 그 외에 아연 (Zn), 철 (Fe), 헤모글로빈 (Hb) 등의 혈청 수치와 leptin, ghrelin, gastrin, motilin, neuropeptide Y 등을 위 운동성 지표로 평가하여 활용한 연구들도 있었다.
4. 소아의 식욕부진 치료에 활용된 한약재들은 山楂, 神麯, 麥芽, 鷄內金 등의 消導劑와 白朮, 山藥, 甘草, 黨蔘, 人蔘, 太子蔘 등의 補氣劑가 다빈도로 활용되었으며, 醒脾養兒顆粒과 蔘苓白朮散을 사용한 연구가 각각 7편과 5편으로 가장 많이 나타났다.
5. 안전성 평가를 시행한 결과, 부작용을 보고한 20편의 연구 중 15편의 연구에서 양 군 모두 명백한 부작용이 발견되지 않았으며, 대조군에 비해 치료군에서 유의하게 적은 수의 부작용이 보고되었다. 이는 소아의 식욕부진을 개선하는 데 한약치료가 보다 안전하고 효과적이라는 근거자료가 될 수 있을 것이다.
각 연구에서 활용된 처방의 구성 약재들을 빈도별로 분석한 결과, 白朮이 33회로 가장 많이 사용되었으며, 그 다음으로 山楂가 32회, 茯苓이 31회 사용되었다. 그 외 陳皮, 甘草가 25회, 神麯이 20회, 麥芽가 18회, 山藥이 16회, 蒼朮, 鷄內金이 14회 사용되었다 (Table 4).
한 실험연구에서는 蔘苓白朮散 투여가 궤양발생 억제에 유의한 효과를 나타내었으며, 위액분비 억제 및 pepsin 배출량 억제 효과가 있다고 보고하였다⁹⁰⁾. 본 논문에 포함된 5편 ^{25,36,47,60,61)}의 연구에서도 蔘苓白朮散을 복용한 한약치료군에서 총유효율이 유의하게 높았으며, 중의증상점수가 유의하게 낮았고, 증상 소실시간이 유의하게 단축되었으며, 체중 증가가 유의하게 나타났기에, 임상에서 활용도가 높을 것으로 기대된다.
분석에 포함된 연구들에서는 소아의 식욕부진을 진단하기 위하여 서양의학적 진단기준 및 중의학적 진단기준을 활용하였으며, 그 중에서도 서양의학 서적인≪諸福棠實用兒科學≫⁶⁴⁾의 진단기준과 중의학적 서적인 ≪中醫病證診斷療效標準≫⁷⁰⁾의 진단기준을 가장 많이 활용하였음을 알 수 있었다.
선정된 연구에서 소아의 식욕부진을 진단하는 데 활용한 진단기준은 서양의학적 진단기준과 중의학적 진단기준이 있었다. 서양의학적 진단기준에 대해 명시한 연구는 총 26편이었으며, 그 중 16편의 연구에서는≪諸福棠實用兒科學≫⁶⁴⁾의 진단기준을 활용하였고, 4편의 연구에서는 ≪小兒厭食症的診療標準≫⁶⁵⁾의 진단기준을, 2편의 연구에서는 ≪小兒厭食研究進展≫⁶⁶⁾의 진단기준을, 2편의 연구에서는 ≪藥理學≫⁶⁷⁾의 진단기준을, 1편의 연구에서는 ≪兒科學≫⁶⁸⁾의 진단기준을, 나머지 1편의 연구에서는 <Wolfson 진단기준>⁶⁹⁾을 활용하였다.
비록 처방명이나 구성 약재들에는 연구마다 차이가 존재했지만, 10편의 연구를 제외하고 나머지 40편의 연구에서 모두 補氣劑와 消導劑가 들어간 처방을 활용한 것으로 나타났다. 처방으로는 異功散加減, 曲麥枳朮湯加減, 蔘苓白朮散, 健脾丸, 四逆散加味, 平胃散加減, 啓脾丸, 醒脾養兒顆粒 등이 활용되었으며, 특히 그 중에서도 醒脾養兒顆粒과 蔘苓白朮散을 사용한 연구가 각각 7편과 5편으로 가장 많이 나타났다. 醒脾養兒顆粒은 중성약 (中成药) 제제로, 一点红, 毛大丁草, 蜘蛛香, 山栀茶를 주요 성분으로 하는데, 4가지 약재를 병용할 경우 開胃醒脾, 固腸止瀉 및 養血安神의 효과가 있어 중국에서 소아의 식욕부진에 많이 활용하고 있으나^19,42), 국내 한약재로 허가되지 않아 한국에서는 사용하기 어려울 것으로 보인다.

이러한 한계점을 보완하기 위해 향후 연구 윤리적 측면을 고려하여 잘 설계된 임상연구가 추가적으로 시행될 필요가 있으며, 이와 더불어 치료종료 후 추적관찰이 포함된 임상연구 설계가 필요할 것으로 생각된다. 또한 성장기 이후에 전향성 추적조사 연구를 바탕으로 한약치료에 의한 소아의 식욕부진 개선이 성장에 미치는 영향을 조사하여 한약치료의 효과에 대한 근거를 마련하는 데 주안점을 두어야 할 것이다.
