본 연구는 SNAGs(Sustained natural apophyseal glides)기법과 시각을 융합한 바이오피드백 훈련이 목 기능 장애 환자에게 고유수용성 감각과 목의 기능장애에 미치는 영향을 알아보고자 하였다. 목 기능장애 환자 31명을 대상으로 무작위로 SNAGs기법과 바이오피드백 훈련으로 할당하였으며, 관절위치 감각과 목 기능장애 평가를 중재 전-후 실시하였다. 16명의 SNAGs 중재그룹은 멀리건 벨트를 이용하여 굽힘, 폄, 회전을 실시하였고, 15명의 바이오피드백군은 시각적 피드백을 통한 깊은목 굽힘근 훈련을 실시하였다. 두 집단은 동일한 시간, 동일한 조건에서 20분 동안 주당 2회를 4주간 실시하였다. 바이오피드백 훈련 집단이 SNAGs 집단보다 고유수용성 감각에서 좌, 우 회전 모두 유의한 향상을 보였으며(p< 0.05), 목 기능장애는 SNAGs기법을 이용한 집단이 바이오피드백 훈련 집단보다 유의한 효과를 보였다(p<0.05). 따라서 SNAGs기법은 목의 기능제한 해결에 효과적이며, 바이오피드백 훈련은 고유수용성 감각 향상에 긍정적인 중재 방법이 될 것이다. 향후 목 통증 환자에 대한 연구를 진행시, 질환의 특성에 따라 중재방법을 선택 할 수 있을 것이다.

또한 목의 고유수용성 감각과 기능향상에 있어서 CCF운동 또한 다양하게 접근되어 지고 있다. 그래서 본 연구에서는 목에 기능장애를 겪는 환자를 대상으로 바이오피드백을 이용한 CCF와 SNAGs기법을 적용하여 목에 고유수용성 감각과 기능에 더욱 효과적인 중재방법을 알아보고자 한다.
목의 고유수용성 감각 향상에 바이오피드백을 활용한 깊은 목 굽힘근 운동이 널리 알려져 있다. 또한 자가 척추 가동술이 목 고유수용성 감각활성화에 효과적이라는 연구를 바탕으로 본 연구는 목 기능장애 환자를 대상으로 효과적인 고유수용성 감각 활성화 중재방법을 알아보고 나아가 목의 기능장애 해소에 대한 효과적인 중재방법을 알아보고자 연구를 진행하였다.
제안 방법
자가 SNAGs기법은 목의 굽힘(Fig 1 A), 폄(Fig 1 B), 왼쪽(Fig 1 C)과 오른 쪽 돌림(Fig 1 D)에 대해서 적용하였다(Fig 1). 모든 적용은 목의 불편감이나 제한이 없는 방향을 찾은 후 적용하였 으며, 굽힘은 앉은 자세로 멀리건 벨트를 사용하여 목의 돌기사이 관절의 45°각도를 맞추어 앞,위쪽으로 견인 후 목 뼈 굽힘을 반복적으로 시도하였고, 목 폄에 대한 SNAGs기법도 마찬가지로 같은 방법으로 폄을 시도 하였다. 돌림에 대한 SNAGs기법도 같은 자세로 적용 하였으며, 활주 (gliding)는 대상자의 목이 중립자세로 될 때 까지 지속 하였다.
본 연구는 목 기능장애를 경험하고 있는 환자들을 대 상으로 SNAGs기법과 목 바이오피드백 운동중 목의 고유수용성 감각 및 기능에 대한 영향을 알아보기 위한 유사 실험 연구(Quasi-experimental studies)로, 두 집단 사전 사후 설계 방법을 사용하였다. 선정된 31명의 대상자를 모집된 31명의 대상자를 2집단과 남녀를 구분하여 Random Allocation Software (version 2.
0 for window)를 사용하여 무작위로 배정하였다. 자가 SANGs 기법을 적용하는 16명, Stabilizer를 이용한 바이오피드백 운동을 적용하는 15명으로 중재 프로그램 시작 전에 사전 검사를 위해 NDI, 고유수용성감각 관련 종속변수를 측정하였고, 치료 프로그램으로 각 군은 총 4주 동안 주 2회 20분간 그룹에 해당한 중재만을 수행하였다.
대상 데이터
연구대상자의 선정 조건은 선행연구를 참고하여 다음 과 같이 설정하였다[18]. 1. 목 통증을 가지고 있는자, 2. 목 장애 지수(Neck disability index, NDI)가 15점 이상으로 중등도 장애 수준 이상의 대상자, 3. 근력강화 및 관절가동시 증상이 심해지지 않는 자, 4. 최근 3월 이상 지속적인 목의 통증 및 불편함을 경험한자, 5. 해당 부위 에 외과적 수술을 받지 않은 자, 6. 최근 3개월 이내 목에 관련된 치료를 받지 않은 자로 선정하였다. 대상자의 일반적 특성은 다음과 같다(Table 1).
본 연구는 2018년 01월 08일부터 2019년 02월25일 까지 G시에 위치한 G종합병원 스포츠 재활센터에 방문한 입원 또는 외래환자를 대상으로 연구를 진행하였으며, 연구 설명을 듣고 자발적 참여 의사를 밝힌 대상자 총 52명 중 중도 탈락(4명) 및 선정조건(NDI 15점 미만: 14명, 근력강화 및 관절 가동시 증상악화: 3명)을 충족시키지 못한 21명을 제외한 총 31명의 대상자로 진행하였다. 연구대상자의 선정 조건은 선행연구를 참고하여 다음 과 같이 설정하였다[18].

이론/모형
목 자가 SNAGs기법은 Shereen 등[16], Kim과 Han[3]의 연구를 참고하여 적용 하였다. 자가 SNAGs기법은 목의 굽힘(Fig 1 A), 폄(Fig 1 B), 왼쪽(Fig 1 C)과 오른 쪽 돌림(Fig 1 D)에 대해서 적용하였다(Fig 1).
목의 기능장애는 Veron와 Mior[21]가 개발한 기능 장애지표(Neck Disability index, NDI)를 번안하여 사용하였다. 한국어로 번안한 도구를 사용하였다.
본 연구에서는 Kim[19]의 연구를 참고하여 바이오피드 백 훈련을 압력 센서(Pressure bio-feedback Stabilizer device, Chattanooga group, USA)를 이용하여 머리-목 굽힘 운동(CCF)을 실시하였으며, 운동단위로 mmHg를 사용하였다. 대상자는 바로 누운 자세에서 입술을 부드럽게 다문 상태로 눕는다.
성능/효과
목의 기능장애 환자들에게 고유수용성 감각에 더욱 효 과적인 중재 방법을 알아보기 위하여 연구를 진행하였으며, SNAGs 기법 보다 시각적 피드백을 활용한 깊은 목 굽힘근 훈련이 더욱 효과적 이였으며, 목의 기능적인 측 면에서는 SNAGs 기법이 더욱 효과적 이였다. 이 결과를 통해 목의 기능장애 환자들에게는 SNAGs기법을, 고유 수용성 감각 저하 환자들에게는 시각적 피드백 훈련을 적용하여 필요한 치료적 요소에 따라 개별적인 중재 방 법을 선택적으로 사용할 수 있을 것이다.
본 연구에서 바이오피드백 훈련을 활용한 깊은 목 굽 힘근 훈련이 SNAGs기법을 적용한 그룹보다 관절위치감 각에서 유의한 효과를 보인 이유는 바이오피드백 훈련방 법은 시각적 피드백을 활용하여 스스로 관절위치감각을 찾는 능동적인 방법인 반면에 SNAGs기법은 자가 유동 기법이라고는 하나 멀리건 벨트를 활용한 목의 수동적 자가유동 기법이라고 할 수 있다.
연구 결과 왼쪽과 오른쪽 돌림에 대한 고유수용성 감 각은 SNAGs그룹보다 바이오피드백훈련 그룹이 더욱 유의한 효과를 보였다(p<0.05). 이는 SNAGs기법을 활용한 자가 관절 유동기법보다 바이오피드백을 활용한 깊은 목 굽힘근 훈련이 고유수용성 감각 활성화에 효과적이라는 결과를 얻을 수 있다.
후속연구
목의 기능장애 환자들에게 고유수용성 감각에 더욱 효 과적인 중재 방법을 알아보기 위하여 연구를 진행하였으며, SNAGs 기법 보다 시각적 피드백을 활용한 깊은 목 굽힘근 훈련이 더욱 효과적 이였으며, 목의 기능적인 측 면에서는 SNAGs 기법이 더욱 효과적 이였다. 이 결과를 통해 목의 기능장애 환자들에게는 SNAGs기법을, 고유 수용성 감각 저하 환자들에게는 시각적 피드백 훈련을 적용하여 필요한 치료적 요소에 따라 개별적인 중재 방 법을 선택적으로 사용할 수 있을 것이다.
본 연구의 한계점은 고유수용성 감각을 측정하는데 있 어 굽힘, 폄과 회전을 별도로 분석하지 않고 좌우측에 대한 통합적 고유수용감각 평가를 진행하였다. 향후 연구에 서는 굽힘과, 폄, 회전에 대한 개별적 분석은 향후 더욱 체계적인 연구 발전에 기여할 것으로 생각된다. 또한 본 연구를 진행하는데 있어 대조군의 부재가 두 그룹에 대 한 명확한 결과 해석을 하는데 한계를 가지고 온 것으로 생각된다.
