Objectives: The aim of this study was to determine the therapeutic effects of a traditional Korean treatment on the symptom improvement and loss of bile juice in cases of functional dyspepsia of gastric dysmotility complicated by bile juice reflux. Methods: Dyspeptic symptoms, gastric motility (electrogastrography and bowel sound analysis), and gastric mucosa (gastroendoscopy) were evaluated. The treatment consisted of Banwhasashim-tang (extract) used as a herbal drug. Both ST36 electrical stimulation and simple immersion stimulation of CV11, 12, and 13 in the abdomen were also applied. Results: Dyspeptic symptoms, including a foreign body sensation in the throat, indigestion, and upper abdominal pain, were all relieved by the treatment and gastric myoelectrical activity and gastric pyloric function were improved. Bile juice disappeared from the gastric mucosa. Conclusion: The traditional Korean treatment was effective at relieving dyspeptic symptoms and bile juice reflux by improving the pyloric sphincter function.

제안 방법

(1) 반하사심탕(半夏瀉心湯) 엑기스제(상품명 청역, 제일약품)를3포/일, 식후30분에복용하였다. (2) 반하사심탕(半夏瀉心湯) 엑기스의 구성은 감초 1g, 건강0.
기능성소화불량증환자에서 한방치료에 의한 위 운동성 및 유문부의 기능 호전으로 소화불량증상완화와담즙역류소실을관찰할수있었던 증례를소개하였다.
2016년 5월 4일에 또 다시 실시한 위내시경 검사에서담즙역류가확인되었고그날본원으로내원하였다.본환자도지속적 인양방치료를받았음에도불구하고내원당시인 후이물감, 소화불량, 복통 등을 호소하였다.신체검사에서 별다른 이상소견은 없었으며 가족력에대해서기술할만한특별한내용은없었다.
구분한 visualanalogscale(VAS)을이용하였다.심함(very severe, 4점), 약간심함(severe, 3점), 증상이 있으나 미약함(moderate, 2점), 간헐 증상(mild, 1점), 증상없음(non, 0점)으로하였다.
위 평활근의 전기적 활성과 위 유문부 기능은체표위전도검사와장음분석에의해이루어졌다. 환자는검사전날22시부터검사전까지금식상태 를유지하였으며, 검사는오전8시부터11시사이에조용하고약간어두운방에서시행되었다.

장음분석에는전자청진기ES-120(JacInstrument, VanNuys, CA, USA)이사용되었는데, 자작한 흡음 기능이 있는 헤드 고정기와adhesive tape를사용하여우측하복부에부착시켜청진기의소리가 디지털녹음기에기록되도록하였다.기록은위전 도측정과동시에진행되었다.

대상 데이터

대상 환자는 72세여 성으로 어릴 때부터 소화상태는 좋지 않았다고 하였으며 평소 인후 이물감, 소화불량및복통등의증상으로인해여러대학 병원과개원가에서진료를받았다고하였다. 2015 년3월6일, K대학소화기내과에서실시한위내시경검사에서담즙역류를확인하였다.
대상환자는48세여성으로2010년4월16일소화불량증으로인해서위내시경을K대학소화기내 과에서실시하였고, 정상소견으로 확인되었음에도 불구하고 증상이 지속되었다. 치료를 받는 도중 2012년3월12일자궁근종으로인하여자궁적출수 술을받았다.

두종류 의검사는환자를35도각도의앙와위로눕힌상태 에서식전15분, 식후 35분 동안동시에 실시되었다.식후검사를위한자극제로뉴케어400 cc(대상웰라이프)가 공급되었다.
증상이 지속되었다. 치료를 받는 도중 2012년3월12일자궁근종으로인하여자궁적출수 술을받았다.이후소화불량증상이더욱악화되었 으며 아울러 인후 이물감과 복통이 병발되어서 2014년3월11일위내시경검사를재실시했으며, 금식에도불구하고위배출지연을보여주는위내 음식물이 관찰되었다.
환자는검사전날22시부터검사전까지금식상태 를유지하였으며, 검사는오전8시부터11시사이에조용하고약간어두운방에서시행되었다.검사 전motionartifact를줄이기위해서불필요한움직 임, 말하는것, 과도한심호흡및수면을삼가는등의주의사항을환자에게미리알려주었다.

데이터처리

기록은위전 도측정과동시에진행되었다.기록된신호는IBM computer로다시보내졌고, 내장된EnteroTachbowel soundanalysis software(ver4.0, WesternResearch, Tuscon, AZ, USA)에의해장음의주주파수(Hz)가분석되었다.식후/식전의주주파수비를위유문부 기능(Pyloric sphincter function; PSF라 함)으로표현하였으며 1.

이론/모형

소화불량(indigestion; ID라 함) 지수는 이전의문헌11에서 소개한 대로 상태를 5단계로 구분한 visualanalogscale(VAS)을이용하였다.심함(very severe, 4점), 약간심함(severe, 3점), 증상이 있으나 미약함(moderate, 2점), 간헐 증상(mild, 1점), 증상없음(non, 0점)으로하였다.
인후이물감(throatdiscomfort;약어로TD라함) 지수는상태를임의의5단계로구분한visualanalog scale(VAS)을이용하였다.이물감을지속적으로느 끼고 고통스럽다(very severe, 4점), 지속적이지는 않지만 발작하면 고통스럽다(severe, 3점), 지속적이지만 견딜만하다(moderate, 2점), 견딜 만한 증상이 가끔 있다(mild, 1점), 증상없다(non, 0점) 로하였다.

성능/효과

식후30분에복용하였다. (2) 반하사심탕(半夏瀉心湯) 엑기스의 구성은 감초 1g, 건강0.83g, 대추1g, 반하1.67g, 인삼 1g, 황금1g, 황련 0.33g로되어있고생산된엑기스의한포분량은1.875g이다.
본논문의연구목적은인후이물감, 소화불량 및 복통 등을 호소했고 진단 결과, 위 운동성 장애의 기능성 소화불량증과 위내시경검사에서 담즙 역류가 확인된 환자를 대상으로, 1년간의한방치료로위운동성및증상의부분적호전과더불어서 담즙역류를소실시킬수있었던증례를소개하는 데있다.
인후이물감은증례1의경우치료전5점에서치 료후2점으로, 증례2는치료전5점에서치료후 2점으로나타났다.소화불량지수는증례1의경우 치료전4점에서치료후1점으로, 증례2는치료전5점에서치료후2점으로나타났다.복통은증 례1의경우치료전4점에서치료후1점으로, 증례2는치료전5점에서치료후2점으로나타났다 (Table1).

48%BS로정상범위에도달되었다. 유문부 기능 역시 개선되었는데, 증례1의경우치료전0.99 에서 치료 후 1.44으로 상승하였고 증례 2 역시 0.98에서1.03로상승하였다(Table1).
장음에서분석된식후미주신경활성은증례1은치료전0.34%BS에서치료후5.71%BS로증가하 였다.증례2는치료전지나친18.
71%BS로증가하 였다.증례2는치료전지나친18.0%BS에서치료 후8.48%BS로정상범위에도달되었다. 유문부 기능 역시 개선되었는데, 증례1의경우치료전0.
치료후, 증상호전은증례모두에서나타났다. 인후이물감은증례1의경우치료전5점에서치 료후2점으로, 증례2는치료전5점에서치료후 2점으로나타났다.

후속연구

우가간혹있다.본연구의경우도이에해당된다 고여겨지는데, 환자의 증상이 첫째 위 운동성 장애에 의한 것인지, 둘째 담즙 역류로 인한 위염이나 점막 손상에 의한것인지, 셋째아니면이들의복합적인것에의한것인지를판별하는것이연구 목적을달성하는데필요하다고생각하였고이것은 추후 한방치료의 기전을 설명하는데 유익하다 보았다.
따라서이둘을종합하면역류와위 배출지연은모두유문판개폐의불안정과연계되 어질 가능성이 있다. 하지만 이것에 대한 문헌을접할수없었기때문에고찰을더진행할수없었 으며, 향후이부분을연구하는것은흥미로운주제가 될 수 있을 것 같았다.
