Objectives : This study aimed to determine the current trends in manual therapy for low back and pelvic girdle pain as regards pregnancy. Methods : Using 7 online databases, we searched for appropriate clinical studies. Results : Eighteen studies were selected from online databases. The included studies consisted of 8 randomized controlled trials, 6 uncontrolled trials, 4 case reports, and 1 non-randomized controlled trial. Four studies were performed in pregnant patients, while others were in postpartum patients. The majority of studies used relaxing or rubbing of soft tissue and some of the studies were manipulated. The visual analog scale (VAS) was the most frequently used outcome, with the effective rate second. Conclusions : This study demonstrates that manual therapy may be a promising therapeutic option low back and pelvic girdle pain for patients as regards pregnancy or postpartum.

이에 저자는 임신과 관련되어 생길 수 있는 요통과 골반통에 대한 수기치료의 효과를 알아보기 위하여 국내외 총 7개의 데이터베이스를 이용하여 논문검색을 실시하였다. 총 18편의 논문이 검색되었으며 출판 연도에 따라 분석한 결과 2000년대부터 꾸준히 수기치료에 대한 임상 연구가 꾸준히 진행되고 있었음을 알 수 있었다.

제안 방법

연구된 논문들을 출판 년도와 연구 종류, 연구대상, 시험군과 대조군 처치, 평가지표, 수기 치료 방법 및 연구 결과를 분석하여 내용을 추출하였으며 각 항목에 따라 차지하고 있는 비율에 대해 분석하였다.

대상 데이터

이 후 두 명의 연구자(PNR, LYJ)가 선별작업을 시행하였다. 그 결과 국내 데이터베이스 4편, 해외 데이터베이스 14편으로 총 18편의 논문이 본 연구에 선정되었다. 문헌선정과 그 배제는 세 연구자(PNR, LYJ, CHY)의 합의를 통하여서 진행하였으며 선택과정은 다음과 같다(Fig.
다음 7개의 국내외 데이터베이스를 이용하여 2019년 9월까지 출판된 문헌을 모두 검색하였다. 영문 데이터베이스 검색 사이트인 PubMed, EMBASE, 중국 데이터베이스 검색 사이트인 China National Knowlegde Infrastructure(CNKI), Wangfang Med, 국내 데이터베이스 검색사이트인 전통의학정보포털(Oriental Medicine Advanced Searching Intergrated System, OASIS), 국가과학기술전자도서관(NDSL), 한국학술정보(KISS)를 활용하였다.
임신과 산후에 발생한 모든 요통과 골반통을 호소하는 환자군들을 대상으로 하였다. 연령과 인종에 대해서는 제한을 두지 않고 검색하였다.
총 18편의 논문에서 시행된 수기 치료를 분석하였다. osteopathic manipulative therapy(OMT)를 사용한 치료는 독일 및 미국의 연구 4편^4,10-12) 이었으며 카이로프랙틱 기법을 이용한 치료는 1편⁹⁾이었다.

이론/모형

그리고 대표적인 요통기능장애지수 평가척도로 쓰이는 Oswestry Disability Index(ODI)가 총 4편^6,8,9,12)의 논문에서 쓰였다. 그 외에도 Full time and/or graded sick leave due pain intensity, EuroQol-5D (EQ-5D), Japanese Orthopaedic Association criteria score (JOA), Roland Morris Disability Questionnaire (RMDQ), Recovery rate of hirabayashi, special test, Xray, Short-form McGill Pain Questionnaire (SF-MPQ)가 평가 지표로 사용되었다.

성능/효과

이상으로 임신과 관련하여 생긴 요통과 골반통에 쓰이는 수기요법에 대해 고찰한 결과 실제 임상에서도 수기요법의 활용도가 높음을 알 수 있었다. 그러나 국내에서 진행된 연구의 경우 3건의 case report와 1건의 nRCT에 그쳐 근거 수준이 높은 연구가 부족하였다.

후속연구

현재 시행되고 있는 추나요법을 통해 요통 및 골반통에서 통증에 감소를 보인 연구가 다수 진행된 바 있으나 임신과 관련된 요통 및 골반통에 대한 연구동향 분석은 본 연구가 처음이다. 따라서 임신과 관련한 요통 및 골반통에 대하여 수기요법을 시행하거나, 향후 연구 방향을 설정하는데 있어 본 연구의 결과가 도움이 될 것으로 사료되며 이는 적극적인 치료에 제한을 가지는 환자들에 있어 폭 넓은 진료 선택권으로 이어지므로 한의계의 진료 영역의 확장으로 이어질 수 있을 것으로 기대된다.
또한 수기요법 단독으로 구성된 치료보다는 침이나 약물과 병행한 치료의 연구가 더 많았기에 수기요법 단독으로서 효과를 도출하기에 충분하지 못한 것으로 생각되었다. 향후 통일된 지표를 사용하는 보다 많은 수의 연구와 높은 근거수준을 가지는 연구들이 필요하며 연구결과를 종합한 체계적 분석도 필요할 것으로 사료된다.
