Objectives: This study is the human factors and disease factors of the copayment system for the elderly (>65 years old) and to identify does the current status and characteristics of the applied elderly and conducted to provide basic data. Methods: Sample cohort data from the National Health Insurance Corporation database, from the years 2012-2015, were analyzed of 21,772 elderly people over the copayment ceiling. Results: The ratio of those who exceeded the copayment ceiling system rose sharply from progressive rates of 3.39% in 2012, 3.69% in 2013 and 5.03% in 2014, to rates of 37.13% from 2013. Factors identified that affect the instances of being over the copayment ceiling were: age, income group, region, severity, disability, sickness distribution, inpatient days, and outpatient days. Conclusions: The reorganization of the copayment ceiling system in 2014 favored low-income families of the elderly, but in 2015, the proportion of elderly was low (only 5.78%). The government's policies needs to change to allow for the amount of the deductible upper limit for low- and middle- income groups to be further subdivided in order for the elderly to receive more deductibles.

대상 데이터

본 연구는 국민건강보험공단에서 제공하는 가장 최근의 빅데이터 자료인 표본코호트 2.0 DB의 2012년∼2015년 4개년도 자료를 분석자료로 사용 하였다. 연구대상은 4개년도 DB중 65세 이상 노인을 추출하여 총 481,252명의 모집단을 기준으로 본인부담상한액 초과자가 21,772명, 비초과자가 459,480명으로 최종 선정하였다.
0 DB의 2012년∼2015년 4개년도 자료를 분석자료로 사용 하였다. 연구대상은 4개년도 DB중 65세 이상 노인을 추출하여 총 481,252명의 모집단을 기준으로 본인부담상한액 초과자가 21,772명, 비초과자가 459,480명으로 최종 선정하였다. 본인부담상한액 초과자의 추출은 2012년∼2013년도는 연간 본인부 담액이 1구간∼3구간은 200만원 이상, 4구간∼5구간은 300만원 이상, 6구간∼7구간은 400만원 이상인 노인을 추출하였다.

데이터처리

초과자의 인적 특성과 질환 특성에 따른 본인부담상한액 비초과자와 본인부담상한액 초과자의 분포는 카이스퀘어(χ2) test를 시행하였다. 본인부담상한액 초과에 영향을 미치는 요인을 분석하기 위해서는 인적변수, 질환 특성, 의료이용 특성 등을 독립변수로 로지스틱 회귀분석을 실시하였다.
본 연구 자료의 통계분석을 위하여 SAS 프로그램을 사용하여 연구대상자의 특성은 빈도분석을 실시하였다. 초과자의 인적 특성과 질환 특성에 따른 본인부담상한액 비초과자와 본인부담상한액 초과자의 분포는 카이스퀘어(χ2) test를 시행하였다. 본인부담상한액 초과에 영향을 미치는 요인을 분석하기 위해서는 인적변수, 질환 특성, 의료이용 특성 등을 독립변수로 로지스틱 회귀분석을 실시하였다.

성능/효과

둘째, 본인부담상한액 초과에 영향을 미치는 요인은 연령, 소득구간, 지역권역, 중증질환 여부, 장애 여부, 상병, 입원일수 및 외래일수로 나타났다. ;연령별로 보면 65세∼74세에 비해 75∼84세가 가장 높았고, 다음으로 85세 이상 순으로 나타났으며, 중증질환자와 중증장애자에서 높게 나타났다. 소득별로는 2012년과 2013년은 소득 1구간에 비해 2구간, 3구간이 높게 나타났고, 그 중 3구간에서 영향력이 제일 높았다.
본인부담상한액 초과자의 비율은 2012년 3.39%, 2013년 3.69%에서 상한제의 정책이 3단계에서 7단계로 개편된 2014년에는 5.06%로 37.13% 상승하였고, 2015년에는 5.78%로 14.2% 증가하였다. 성별은 남자는 2.
첫째, 본인부담상한액 초과자의 비율은 해가 갈수록 상승하였고, 제도 변화로 좀 더 많은 노인들 에게 정책의 혜택이 적용되었다. 여자, 고연령층, 중증질환자, 장애자에서 초과자 비율이 상대적으로 높아져 정책의 수혜가 높았다.

후속연구

본 연구에서는 본인부담상한액 전체 초과자의 분석이 아닌, 의료이용 빈도가 높은 65세 이상의 노인 인구만을 분석하였기 때문에, 기존의 논문과 차별성이 있고, 주상병을 이용하여 개인별 데이터를 분석한 연구라는데 의의가 있다. 추후에는 각 의료기관의 주상병, 부상병을 연계하여, 본인부담 상한액 초과자의 질병에 따른 의료 이용 행태에 대한 추가 연구가 필요할 것으로 사료된다.
