본 연구는 대학생을 대상으로 정서적 외상경험, 스트레스, 우울, 불안이 자살생각에 미치는 영향요인을 확인하고자 시도되었다. 자료수집은 B시에 소재한 4개 대학의 보건계열 대학생을 대상으로 설문지를 이용하여 최종 289부를 분석에 사용하였다. 수집된 자료는 SPSS/WIN 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test와 ANOVA, multiple regression analysis로 분석하였다. 연구결과 연구대상자의 일반적 특성에 따른 정서적 외상은 성별에서 유의한 차이가 있었으며(p=.005), 스트레스는 학년에서 유의한 차이를 보였다(p=.029). 우울은 성별(p=.020)과 종교(p=.022)에 대해서, 불안은 성별(p=.047)에 대해 유의한 차이를 보였다. 자살생각은 일반적 특성과 유의한 차이가 없었다. 대상자의 자살생각은 정서적 외상(p<.001), 스트레스(p<.001), 우울(p<.001), 불안(p<.001)과 유의한 상관관계를 나타내었다. 대상자의 자살생각에 영향을 미치는 요인은 정서적 외상(t=4.117, p<.001), 스트레스(t=2.909, p=.004), 우울(t=3.421, p=.001), 불안(t=2.875, p=.004)으로 나타났고, 설명력은 34.1%였다. 본 연구는 대학생의 자살생각과 관련성이 높은 정서적 외상경험, 스트레스, 우울, 불안과의 상관관계를 규명하고 자살생각에 미치는 요인을 확인함으로써 자살예방 프로그램 개발을 위한 기초자료로 활용할 수 있을 것이며, 대학생 자살률을 감소시키기 위한 프로그램의 개발이 필요하다.

The purpose of this study was to examine the relationship between emotional traumatic experience, stress, depression, and anxiety and to identify factors influencing suicide ideation. Data collection was carried out using a questionnaire survey administered to students from four health colleges in B city and a total of 289 questionnaires. Collected data were analyzed by frequency and percentage, mean, standard deviation, t-test, ANOVA, and multiple regression analysis using the SPSS/WIN program. The results show that there were significant differences in emotional trauma according to the general characteristics of the study subjects (p=.005) and stress related to grades (p=.029). Depression showed significant differences for gender (p=.020), religion (p=.022), and anxiety related to gender (p=.047). Suicidal thinking was not significantly different from the general characteristics. The suicidal ideation of the subjects was significantly correlated with emotional trauma (p<.001), stress (p<.001), depression (p<.001) and anxiety (p<.001). Factors affecting the suicidal ideation of the subjects were emotional trauma (t=4.117, p<.001), stress (t=2.909, p=.004), depression (t=3.421, p=.001), and anxiety (t=2.875, p=.004), and the explanatory power was 34.1%. This study can be used as basic data for the development of a suicide prevention program by identifying correlations between emotional traumatic experience, stress, depression, and anxiety related to suicide ideation and identifying factors affecting suicide ideation. Based on these results, the development of programs to reduce the college student suicide rate is necessary.

대상 데이터

본 연구의 대상자는 B시에 소재한 4개 대학 보건계열 학과의 학과장에게 연구의 목적을 설명하고 동의를 얻었다. 동의를 얻은 해당 학과의 만 19세 이상의 재학생을 대상으로 본 연구자가 연구의 목적을 설명하였다.
95로 지정하였을 때 필요한 표본 수는 280명이었다. 응답률을 고려하여 300명의 대상자를 임의표집 하였고, 총 300명 중 설문지 작성이 불충분한 14부를 제외한 총 289부를 최종 분석에 사용하였다.

데이터처리

수집된 자료의 분석은 연구대상자의 일반적인 사항에 대해서는 SPSS Win을 이용하여 대상자의 일반적 특성은 빈도와 백분율, 정서적 외상경험, 스트레스, 불안, 우울, 자살생각의 정도는 평균가 표준편차로, 일반적 특성에 따른 정서적 외상경험, 스트레스, 불안, 우울, 자살생각의 차이는 t-검증, 일원분산분석으로 분석하였고, 사후 검정은 scheffe’s test를 사용하였다. 정서적 외상경험, 스트레스, 불안, 우울, 자살생각의 상관관계를 알아보기 위하여 피어슨 상관계수를, 자살생각의 영향요인을 파악 하기 위해 다중회귀분석을 실시하였다.

이론/모형

정서적 외상경험은 정서적 학대 또는 방임의 경험으로 공포, 두려움, 무력감 등의 감정을 경험한 것을 의미한다[29]. 본 연구에서는 Bernstein과 Fink[30]가 개발하고 Lee[29]가 번안한 CTY도구 중 정서적 학대와 정서적 방임의 각 5문항씩 10문항을 사용하였다. 4점 Likert 척도로 측정하였으며, 최저 10점에서 최고 40점까지로 점수가 높을수록 학대나 방임을 당한 경험이 많음을 나타낸다.
자살생각은 자살 행동이 동반되지 않는 자살에 대한 생각이나 사고를 의미하며[37], 본 연구에서는 Harlow, Newcomb과 Bentler[38]가 개발하고 Cho[34]가 번안한 5문항의 5점 Likert 척도로 측정하였다. 최저 5점에서 최고 25점까지로 점수가 높을수록 자살생각의 정도가 높음을 의미하며, Cho[34]의 연구에서 신뢰도 계수는 Cronbach’s α = .
지각된 스트레스는 개인의 주관적 경험에 초점을 둔 스트레스 경험으로[31], 본 연구에서는 Cohen, Kamarck, Mermelstein[32]이 개발한 지각된 스트레스 척도를 Park, Seo[31]가 번안한 한국판 지각된 스트레스 척도를 사용하였다. 총 10문항으로 긍정적 지각(4,5,6,7,8문항)과 부정적 지각(1,2,3,9,10문항)으로 구성되어 있으며, 긍정적 지각 5문항은 역환산하였다.

성능/효과

둘째, 본 연구에서 변수 간의 상관관계를 조사한 결과, 자살생각은 정서적 외상, 스트레스, 우울, 불안과 양의 상관관계를, 정서적 외상은 스트레스, 우울, 불안과 양의 상관관계를, 스트레스는 우울, 불안과 양의 상관관계를 나타내었으며, 우울은 불안과 양의 상관관계를 보였다. 본 연구에서 대학생의 정서적 외상, 스트레스, 우울, 불안 정도가 심할수록 자살생각과 관련성이 높았다.
셋째로 본 연구에서 대학생의 자살생각에 영향을 미치는 요인으로 정서적 외상, 스트레스, 우울, 불안이 확인되었다. 이는 청소년을 대상으로 한 Kim, Sohn과 Kim[49]의 연구에서 남, 여 학생 모두에서 폭력피해 경험이 없는 학생에 비해 폭력피해 경험이 많을수록 자살생각이 높은 결과와 여학생에게 양친 부모보다 편부, 편모, 재혼부모에서 자살생각이 더 높은 결과와 유사하다.
첫째, 본 연구에서 대학생의 정서적 외상점수를 조사한 결과, 40점 만점(1-4점 척도)에서 평균값이 38%를 차지하였으며 Ko[39]의 연구에서 정서적 외상점수의 평균값은 61.5%를 나타내었으며, Jung, Park[40]은 간호 대학생을 대상으로 정서적 외상에서 학대와 방임으로 구분하여 보았으며, 학대의 평균값은 77.3%, 방임의 평균값은 73.7%를 보여 차이가 있었다. 반면 Lim[41]의 청소년을 대상으로 한 정서적 외상점수 평균값은 48.

후속연구

따라서 본 연구를 바탕으로 자살생각에 영향을 미치는 다양한 영향요인과 직간접 효과에 대한 반복 연구가 필요하다. 또한 대학생의 자살생각을 예방하는 효과적인 교육 및 프로그램의 개발이 필요하다.
