목적: A군 사슬알균은 소아 환자들에 있어서 흔한 감염원으로 다양한 임상양상을 보인다. 급성편도염이나 피부 및 연조직 감염이 흔하며 상당한 이환율과 사망률을 동반하는 혈류감염 또한 발생할 수 있다. 본 연구는 국내 소아 A군 사슬알균 혈류감염 환자들의 임상양상과 치료 결과를 확인하고자 하였다. 방법: 이 연구는 단일기관의 후향적 연구로, 2000년 1월부터 2016년 12월까지 A군 사슬알균 혈류감염으로 입원치료 받은 18세 이하 소아 환자들을 대상으로 하였다. 균혈증 발생시 임상양상, 기저질환, 중환자실 치료 여부, 그리고 균주의 항생제 감수성을 조사하였다. 결과: 19명에서 A군 사슬알균 혈류감염이 확인되었다. 10명(53%)이 남자였으며 연령의 중앙값은 7.4세였다(범위, 0.3-17.4세). 14명(74%)의 환자들이 만성 기저질환을 가지고 있었다. 5명(26%)은 면역저하(백혈병 및 만성신질환) 환자였다. 8명(42%)이 림프낭종, 혈관종, 정맥 기형 등 림프관 및 혈관 기형이 있었으며 그 중 7명은 발열과 병변부 국소 염증소견이 있었다. 3명(16%)의 환자들에게 폐렴이 발생했고 이 중 2명은 인공호흡기 치료를 받았다. 환자들의 30일째 사망률은 6% (1/19)였으며 해당 환자는 균혈증을 동반한 폐렴으로 사망하였다. 모든 검출된 A군 사슬알균은 페니실린에 감수성을 보였다. 15균주(79%)는 에리스로마이신과 클린다마이신에 감수성을 보였다. 결론: 본 연구에서 A군 사슬알균 혈류감염이 발생한 소아 환자들의 임상양상을 분석하였다. A군 사슬알균은 소아에 있어 균혈증을 유발할 수 있는 심각한 감염의 원인균으로 고려가 필요하다.

In addition, bacteremia with significant morbidity and mortality can also occur. This study was conducted to describe the clinical manifestations and treatment outcomes of pediatric GAS bacteremia patients in Korea. Methods: This was a single-center, retrospective study. From January 2000 to December 2016, pediatric patients aged ≤18 years with GAS bacteremia were studied. Clinical manifestations, underlying diseases, intensive care unit stay, and antibiotic susceptibility were evaluated. Results: During the study period, 19 patients had GAS bacteremia. Ten (53%) were male, and the median age was 7.4 years (range, 0.3-17.4 years). Fourteen (74%) had chronic underlying diseases. Five (26%) were immunocompromised (leukemia and chronic kidney disease). Eight (42%) had lymphatic or vascular malformations, of which seven had lesions with signs of inflammation. Three (16%) developed pneumonia, and two of them received ventilator care. The 30-day mortality rate was 6% (1/19), and the cause of death was bacteremic pneumonia. All GAS isolates were sensitive to penicillin. Fifteen (79%) were sensitive to both erythromycin and clindamycin. Conclusions: This study identified various clinical manifestations of GAS bacteremia. GAS should be considered as a potential pathogen that can cause bacteremia and result in a serious clinical course.

본 연구는 소아환자에서 발생한 A군 사슬알균 감염 중 대표적인 침습 감염질환인 균혈증에 대한 임상양상, 치료경과를 분석하였다.
제안 방법
세균 동정은 VITEK 2 설비(bioMérieux, Marcy-l'Étoile, France)로 시행하였으며, 항생제 감수성 검사는 검사시점의 Clinical and Laboratory Standards Institute 지침에 따라 페니실린, 클린다마이신, 에리스로마이신에 대한 감수성을 조사하였다.

2000년 1월부터 2016년 12월까지 삼성서울병원에서 치료를 받은 0세부터 18세 이하 소아환자를 대상으로 의무 기록을 후향적으로 검토하였으며 A군 사슬알균이 배양검사에서 확인된 환자들 중 혈액에서 A군 사슬알균이 검출된 환자를 선정하였다. 연구 대상의 균혈증 발생시 동반 증상, 성별, 나이, 기저 질환, 중환자실 치료 여부, 항균제 감수성을 조사하였다.
성능/효과
2000년 1월부터 2016년 12월까지 A군 사슬알균이 배양된 환자는 249명으로, 이 중 19명(8%)에서 균혈증이 발생하였다. 이들 19명 중 남자가 10명(53%)이었으며, 연령의 중앙값은 7.
신이식 후 면역억제 치료 중이던 환자 1명에서는 심낭염이 발생하였으며 혈액 및 심낭액에서 A군 사슬알균이 검출되었다. 기저질환으로 림프관 및 혈관 기형을 지닌 환자 8명 중 7명(88%)에서는 발열 및 기형부위의 통증 또는 부종 등 염증 소견을 보였다(Table 1). 백혈병 환자를 제외한 국소 증상이 없는 균혈증 환자는 3명(16%)이었다.
환자에서 검출된 A군 사슬알균은 모두 페니실린에 감수성이 있었으며, 클린다마이신에 84% (16/19), 에리스로마이신에 79% (15/19) 감수성이 있었다. 클린다마이신과 에리스로마이신 모두에 감수성인 경우는 79% (15/19)였다.
후속연구
본 연구에서는 국내 소아에 있어 A군 사슬알균 균혈증 발생시 다양한 임상양상을 확인할수 있었다. 면역저하자, 림프 및 혈관 기형 환자 및 기저질환이 없는 건강한 환자의 경우에도 A군 사슬알균은 심각한 침습 감염인 균혈증을 일으키는 것으로 확인되어 국내 발병에 대한 지속적인 감시와 고려가 필요하다.
