Among the three top KCD-7 major group diseases, publications on Group C (D) were increasing significantly, publications on Group G were slightly increasing, but, publications on Group I decreased in the 2010s compared to the 2000s. Conclusions: In this study, we analyzed the frequent disease groups treated with domestic Korean-Western medicine in collaborative studies using the major and medium classifications of KCD-7. Well-designed clinical trials and economic evaluation research are needed, and this study can provide directions for future collaborative studies using Korean-Western medicine.

대상 데이터
두 명의 연구자는 제목 및 초록과 원문을 바탕으로 선정 기준에 맞지 않은 논문들을 개별적으로 배제한 후 확인하는 과정을 거쳤다. 5개의 국내 데이터베이스를 통해 논문명 검색을 한 결과, 최종 287,207편의 문헌이 검색되었다. 검색어 ‘의한의’, ‘의⋅한의’ 결과에서는 논문 제목에 ‘~에 의한’, ‘~의’ 가 포함된 의⋅한 협진과 무관한 문헌이 검색되었고, 검색어 ‘협진’ 결과에서는 의과 혹은 치의과 내에서 타 진료과와의 협진 연구가 많이 포함되었다.

이후 논문 전문을 읽고 2차 screening을 시행한 결과, 재활의학 분야에서의 의⋅한 협진 제도연구11 1편에서 특정 협진 질환군이 실제 명시되어 있지 않아, 해당 문헌을 배제하여 최종 112편을 연구 대상으로 선정하였다(Fig. 1)
성능/효과
대상 문헌들을 연구 방법론별로 분석한 결과, 의⋅한 협진 질환군을 다룬 문헌들에서 증례보고 및 증례군 연구와 임상시험 연구가 75% 이상을 차지하였다.
분류 결과, 99편의 의⋅한 협진 연구는 총 41개의 중분류 질환군을 다루고 있었으며, 그중 뇌혈관질환(I60-I69)이 17편(17.2%)으로 가장 많았고, 다음으로 신경, 신경뿌리 및 신경총장애(G50-G59)가 13편(13.1%), 증상성을 포함하는 기질성 정신장애(F00-F09)는 6편(6.1%), 추체외로 및 운동 장애(G20-G26)는 5편(5.1%)이고, 호흡기 및 흉곽내기관의 악성 신생물(C30-C39), 유방의 악성 신생물(C50), 등병증(M50-M54), 기타 연조직 장애(M70-M79)은 각각 4편(4.3%)으로 8개 중분류 질환군의 합이 50% 이상을 차지하였다(Table 4).

한 예로 질환명이 뇌졸중인 경우, 질환명에 해당하는 코드 I64를 찾은 다음에 상위 대분류 코드 I를 부여하였다. 협진 질환군을 KCD 대분류로 분석한 결과, 신경계 질환(G 코드)이 22편(19.6%)으로 가장 많이 차지하였고, 신생물(C, D코드)과 순환계 질환(I코드)가 각각 19편(17.0%)이었으며, 근골격계 질환(M코드)은 11편(10.4%) 이었다(Table 3).
후속연구
본 연구의 한계점은 연구 대상을 국내 학술지만으로 한정하여 국외 학술지에 게재된 의⋅한 협진 연구들이 연구 대상에서 누락되었다는 점으로, 추후 국외 학술지 논문들을 포함한 후속 연구 시행을 통해 보완해야 할 필요성이 있다.
의⋅한 협진에 대한 건강보험 적용의 타당성을 제시할 만한 신뢰성 있는 근거를 마련하기 위해서 추후 의⋅한 협진 질환군을 대상으로 질 높은 임상연구와 경제성 평가 연구가 필요하다.
이로 인해 해당 질환군에서 활발하게 의⋅한 협진 연구가 수행되고 있다고 여겨지며, 향후에도 지속적으로 이루어져야 할 것이다.
KCD 대분류 및 중분류를 이용하였기에 실제 협의진찰료가 발생하는 의⋅한 협진 다빈도 질환군과의 비교가 가능하다는 것과 기존 문헌고찰 연구에서 부족했던 대상 질환군에 대한 분석이 이루어졌다는 장점이 있다. 이에 본 연구에서의 의⋅한 협진 연구 동향 및 대상 질환군 분석이 의⋅한 협진 연구의 방향성 제시에 단초가 되기를 바란다.
