Objectives: This study aimed to investigate the factors associated with unmet dental needs among adults with activity limitations. Methods: Data were obtained from the seventh Korean National Health and Nutrition Examination Survey (KNHANES). The final sample consisted of 945 adults aged 20 years or older with limited activity. Logistic regression analysis was used to examine the influence of predisposing, enabling, and need factors on unmet dental needs. Results: The proportion of adults with unmet dental needs during the last year was 52.2% in the middle-aged group and 45.5% in the older adult group. Higher household income was associated with fewer unmet dental needs in both groups. In the older adult group, married people were less likely to have unmet dental needs. In the middle-aged group, adults who perceived their oral health as poor were more likely to have unmet dental needs. Conclusions: All three factors (predisposing, enabling, and need) were found to be associated with unmet dental needs among adults with activity limitations. Special efforts should be made to improve access to dental care services for middle-aged adults with activity limitations.

대상 데이터

본 연구는 제7기(2016-2018년) 국민건강영양조사 자료 중 지역사회에 거주하는 20세 이상 성인을 대상으로 하였다. 국민건강영양조사는 질병관리본부 연구윤리심의위원회의 승인을 받아 수행되고 있다.
제7기 조사의 경우 1차 연도와 2차 연도는 연구윤리심의위원회의 의견에 따라 심의를 받지 않고 수행되었으며, 3차 연도는 승인(2018-01-03-P-A)을 받아 수행되었다. 제7기 국민건강영양조사 참여자는 총 7,992명이었는데, 본 연구에서는 그 중 조사 당시 ‘건강상의 문제나 신체 혹은 정신적 장애로 일상생활 및 사회활동에 제한을 받고 있다’고 응답한 성인 945명의 자료를 분석하였다.

데이터처리

앤더슨 모형에 기반한 설명변수에 따라 미충족 치과의료 경험 여부가 다른지를 검증하기 위하여 카이제곱 검정을 실시하였다[29]. 다음으로 미충족 치과의료 경험여부에 영향을 주는 요인을 분석하기 위하여 로지스틱 회귀분석을 실시하였다. 모든 자료는 IBM SPSS statistics 25(version 25.
미충족 치과 의료와 관련하여 성인층과 노인층이 서로 다른 특성을 나타낸다는 선행연구에 따라 성인층과(18-64세) 노인층(65세 이상) 두 집단으로 나누어 분석하였다. 앤더슨 모형에 기반한 설명변수에 따라 미충족 치과의료 경험 여부가 다른지를 검증하기 위하여 카이제곱 검정을 실시하였다[29]. 다음으로 미충족 치과의료 경험여부에 영향을 주는 요인을 분석하기 위하여 로지스틱 회귀분석을 실시하였다.

이론/모형

미충족 치과의료에 영향을 미치는 요인을 파악하기 위하여 앤더슨의 의료이용 모형을 적용하였다. 앤더슨 모형은 의료서비스 이용 또는 미충족 의료를 분석하는데 유용한 연구모형으로서 선행요인(predisposing), 가능요인(enabling), 필요요인(need)을 통해 서비스 이용을 분석한다[29].

성능/효과

1. 건강상의 문제나 신체 혹은 정신적 장애로 활동에 제한을 받는 성인 중 미충족 치과의료를 경험한 사람의 비율은 성인층(20-64세)이 52.2%, 노인층(65세 이상)이 45.5%로 매우 높은 것으로 나타났다.
2. 결혼상태는 노인층의 미충족 치과의료 경험률에 영향을 미쳤다. 배우자가 없을 경우 미충족 치과의료를 경험할 가능성이 높아지는 것으로 나타났다.
3. 가구소득이 높을수록 미충족 치과의료를 경험할 가능성이 낮아지는 것으로 나타났으나, 노인층의 경우 이러한 영향이 중상소득 계층에만 한정되었으며 고소득 계층에는 유의미한 영향을 미치지 않았다.
4. 성인층의 경우 주관적 구강건강상태가 나쁠수록, 신체적 제약이 없는 경우 미충족 치과의료를 경험할 가능성이 높아지는 것으로 나타났다.

후속연구

이상의 연구결과를 종합해 볼 때 건강상의 문제나 신체 혹은 정신적 장애를 지닌 성인들의 치과서비스 접근성 향상을 위한 다각적인 노력이 필요한 것으로 판단된다. 특히 활동제한 중장년층 성인들의 미충족 치과 의료 경험률이 매우 심각한 것으로 나타났다.
이럴 경우 고소득 집단은 치과의료에 대한 기대치가 높아서 저소득층 집단은 반드시 필요하다고 생각하지 않는 치과의료가 고소득층 집단에서는 미충족 치과의료에 포함될 가능성이 있다. 큰 규모의 표본과 구강검진을 통한 치과의료 필요성을 측정한 후속연구가 진행된다면 이와 관련한 분명한 연구 결과를 얻을 수 있을 것으로 생각한다.
한편 본 연구에서는 성인층의 미충족률이 노인층보다 오히려 높은 것으로 나타났는데, 이는 전체 성인들을 대상으로 한 차와 박[30]의 연구와는 상반되는 결과로서, 장애 또는 만성질환을 지닌 중장년층 성인들의 치과의료와 관련한 취약성을 보여주는 결과이다. 활동제한 중장년층의 미충족 치과의료 발생이유를 가용성, 접근성, 수용성으로 나누어 분석함으로써 취약계층을 위한 정책 개발에 적용할 수 있는데[13], 이는 본 연구의 범위를 벗어나는 것으로서 후속연구를 통한 더욱 정밀한 분석이 요구된다.
