The aim of this study is to identify the influence of low back pain experienced by emergency room (ER) nurses and the mediating effect of impairments in the activities of daily living (ADL). The subjects of the study were 188 clinical nurses employed in emergency rooms at high-level hospitals in Daegu and Gyeongsangbuk-do. The collected data were analyzed using a program called SPSS 22.0, and the methods used include descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and regression analysis. The results of the study are as follows: First, low back pain in emergency room nurses positively correlated with impairments in the ADL and depression. Impairments in the ADL also positively correlated with depression. Second, low back pain in ER nurses influenced their depression, which was found to be mediated by the ADL.

이상에서와 같이 선행연구를 바탕으로 간호사의 우울에 대한 중요성을 주목하고, 응급실 간호사가 요통과 일상생활 활동장애가 우울에 미치는 영향력을 확인하고자 한다. 이에 대한 연구의 문제는 다음과 같다.
따라서 우울의 위험에 처해 있는 간호사들을 모니터하고 적극적으로 대처할 필요가 있다[6]. 이에 응급실 간호사의 간호 인력 운영을 위해 우울과 관련된 요인을 파악하고자 한다.

대상 데이터

본 연구의 대상은 대구·경북 지역의 상급병원 응급실에 근무하는 임상간호사로서 적어도 지난 1년 동안 1회 이상의 요통을 경험한 간호사로서 본 연구의 목적을 이해하고 자발적 참여에 동의한 간호사를 모집단으로 하였다. 본 연구에 필요한 표본 수는 G-power 3.1.9.2 프로그램으로 산출하였을 때 173명으로 산출되어 부정확한 답변과 탈락 가능성 10%를 고려하여 총 190명에 대하여 설문지를 배부하고 회수되었으며, 그 중 부적절한 2부를 제외한 188명의 자료가 분석에 활용되었다.

넷째, 요통이 우울에 미치는 영향은 회귀분석을, 일상생활 활동장애의 매개효과는 Baron & Kenny[21]가 제시한 분석법을 실시하였다(p<.05).
둘째, 조사대상자의 일반적 특성에 따른 우울의 차이는 t-test와 ANOVA를, 사후검정으로 Scheffe test를 하였다(p<.05).
셋째, 주요변수 간의 상관성은 Pearson correlation coefficient를 실시하였다(p<.05).

이론/모형

요통을 측정하기 위해 Huskisson[16]이 개발한 시각상사척도(Visual Analog Scale)를 사용하였다. 측정은 주관적으로 느끼는 요통의 정도에 따른 것으로 ‘전혀 아프지 않다 0점’에서 ‘참을 수 없이 아프다(매우 심한통증) 10점’으로 척도화 하여 점수가 높을수록 요통의 정도가 심한 것을 의미한다.
우울을 측정하기 위해 Beck[19]이 개발한 Beck Depression Inventory(BDI)도구를 이영호와 송종용[20]에 의해 재구성 되어진 단일변인 20개 문항을 사용하였다. 측정은 Likert 4점 척도로 하여 점수가 높을수록 우울의 정도가 높은 것을 의미한다.
일상생활 활동장애를 측정하기 위해 Fairbank 등[17]이 개발한 기능장애 척도(Disability Index)를 임현술 등[18]이 번역·수정한 단일변인 9개 문항을 사용하였다.

성능/효과

응급실 간호사들이 현장에서 경험하는 우울을 온전히 감소시킬 수 없지만 관련요인들을 이해하고 예방을 위한 실천적 행동은 중요하다. 본 연구결과 간호사가 경험하는 요통의 정도가 높을수록 우울에 영향을 미치는 것으로 나타났고, 요통으로 인한 일상생활의 활동장애는 우울의 정도를 높이는 매개적 역할을 하고 있었다. 따라서 요통과 일상생활에 불편함이 없도록 하기 위한 두 가지 측면에서 예방 및 관리 전략이 필요하다.
특히 응급실 간호사는 긴급을 요하는 환자를 대상으로 의료서비스를 제공하기 때문에 실수나 잘못된 판단은 환자의 생명을 위협할 수 있으므로 우울증에 적극적으로 대처해야 한다. 본 연구의 결과에서도 알 수 있듯이 요통은 일상생활 활동에 장애를 일으킬 분 아니라 우울을 유발하는 것으로 파악되었다. 결국 요통은 삶 전반에 문제를 일으킬 수 있기 때문에 간과하여서는 안되는 문제라는 사실을 강조한다.
본 연구의 분석결과 일반적 특성에 따른 차이는 연령이 35세∼39세가 가장 높고, 29세 이하가 가장 낮았다.

응급실 간호사의 요통은 우울에 정의 영향을 미치는 것으로 나타났다. 서복남 등[30]이 중년여성의 통증 정도가 높을수록 우울정도가 높다.
일상생활 활동장애는 요통이 우울에 미치는 영향관계에서 매개적 역할을 하는 것으로 나타났다. 요통은 우울증과 같은 정신 장애가 발생하여 일상적인 활동에 부정적인 영향을 주고[14], 6개월 이상 지속 되는 만성 요통은 지속적인 약물 및 물리치료에도 불구하고 동통이 쉽게 호전 되지 않고 이차적으로 근력이 약화되어 신체기능과 일상생활 활동 수준을 저하시킨다[36].
일상생활 활동장애는 우울(r=.362, p<.001)과 정(+)적인 상관성을 보였다.

후속연구

따라서 우울증상을 보이는 간호사를 조기 발견하여 중증의 단계로 넘어가지 않도록 관리하는 시스템 마련도 시급하다고 판단된다. 추후 연구들은 체계적인 이론적 모델에 근거하여 그 결과를 확인하는 노력이 필요하다.
