본 연구의 목적은 온라인 뉴스 기사를 분석하여 환자안전에 대한 사회적 이슈를 확인하고, 환자안전법 시행 이전과 시행 이후 사회적 이슈의 변화를 확인하기 위함이다. R 프로그램을 이용하여 2010년 1월 1일부터 2020년 3월 5일까지 총 7600건의 온라인 뉴스 기사를 수집하였으며, 키워드 분석, 토픽 모델링, 동시출현 네트워크 분석을 시행하였다. 2609개의 키워드는 다음의 8가지 주제로 범주화되었다 : "의료행위", "의료인력", "감염 및 시설", "간호·간병통합서비스", "의약품", "개선을 위한 시스템 개발 및 구축", "환자안전법", "의료기관 인증". 그리고 환자안전법 시행 이전에는 환자안전 인식, 감염관리, 의료기관 인증 등의 키워드가 등장하였으나 시행 이후에는 환자안전 문화, 투약 등의 키워드가 등장하였으며 간호의 중요도 순위가 상승하였다. 의료계뿐 아니라 대중에게도 환자안전에 관한 관심은 높아지고 있으며, 환자안전 향상에 간호의 역할은 중요하다. 따라서 환자안전을 간호의 핵심 역량으로 삼고 지속적인 교육을 해나가야 할 것이다.

This study aims to analyze online news articles to identify social issues related to patient safety and compare the changes in these issues before and after the implementation of the Patient Safety Act. This study performed text mining through the R program, wherein 7,600 online news articles were collected from January 1, 2010, to March 5, 2020, and examined using keyword analysis, topic modeling, and word co-occurrence network analysis. A total of 2,609 keywords were categorized into 8 topics: "medical practice", "medical personnel", "infection and facilities", "comprehensive nursing service", "medicine and medical supplies", "system development and establishment for improvement", "Patient Safety Act" and "healthcare accreditation". The study revealed that keywords such as "patient safety awareness", "infection control" and "healthcare accreditation" appeared before the implementation of the Patient Safety Act. Meanwhile, keywords such as "patient safety culture". and "administration and injection" appeared after the act's implementation with improved ranking of importance pertaining to nursing-related terminology. Interest in patient safety has increased in the medical community as well as among the public. In particular, nursing plays an important role in improving patient safety. Therefore, the recognition of patient safety as a core competency of nursing and the persistent education of the public are vital and inevitable.

제안 방법

이에 본 연구에서는 토픽 모델링을 적용하여 환자안전과 관련한 사회적 이슈를 확인하고자 한다. 그다음 이슈의 변화를 살펴보기 위하여 환자안전법 시행 이전과 시행 이후로 온라인 뉴스 기사를 나누어 각각 동시출현 단어 분석을 시행하고자 한다.
본 연구는 비정형 텍스트 형태의 온라인 뉴스 기사를 환자안전법 시행 이전과 시행 이후로 나누어 각각 환자안전 키워드와 동시출현한 키워드를 찾아내고 토픽 모델링 방법으로 범주화하였다. 그 결과 “의료행위”, “의료인력”, “감염 및 시설”, “간호·간병통합서비스”, “의약품”, “개선을 위한 시스템 개발 및 구축”, “환자안전법”, “의료기관 인증”의 8가지 주제를 도출할 수 있었다.
본 연구에서는 동시출현 단어 분석을 시행하기 위하여 전처리 단계에서 생성된 키워드들을 대상으로 전체 키워드들의 상관관계 값을 구하고, 키워드 분석에서 생성된 DTM을 이용하여 동시출현 단어 데이터셋을 만들었다. 이후 “환자안전”이라는 특정 키워드를 넣어 이와 문서 내에서 함께 출현한, 즉 상관관계가 높은 키워드를 20개씩을 각각 추출하였다.
본 연구에서는 환자안전으로 검색된 온라인 뉴스 기사를 이용하여 키워드를 추출하고 이를 토픽 모델링과 동시출현 단어 분석 방법으로 분석하였다. 토픽 모델링결과, 환자안전과 관련된 사회적 이슈로 “의료행위”, “의료인력”, “감염 및 시설”, “간호·간병통합서비스”, “의약품”, “개선을 위한 시스템 개발 및 구축”, “환자안전법”, “의료기관 인증”이라는 8가지 주제를 도출할 수 있었다.

데이터처리

본 연구에서는 TF-IDF 가중치를 적용한 문서-단어행렬(Document-Term Matrix, DTM)을 만들어 토픽모델링과 동시출현 단어 분석을 시행하였다. 더불어 동시출현 단어 분석 시에는 환자안전법 시행 이전과 시행 이후의 기사에서 각각 TF-IDF 기준 상위 30개의 키워드를 추출한 후 비교하였다.
본 연구에서는 TF-IDF 가중치를 적용한 문서-단어행렬(Document-Term Matrix, DTM)을 만들어 토픽모델링과 동시출현 단어 분석을 시행하였다. 더불어 동시출현 단어 분석 시에는 환자안전법 시행 이전과 시행 이후의 기사에서 각각 TF-IDF 기준 상위 30개의 키워드를 추출한 후 비교하였다.
하지만 형태소 분석을 시행하면 “환자안전”과 같은 복합명사가 “환자”, “안전”과 같은 명사로 추출이 되어 의미가 불분명해지게 된다. 이에 기사의 본문에서 복합명사로 사용되었던 키워드를 확인하기 위해 N-gram 분석을 시행하여 동시 출현하는 단어들을 확인하였고 이를 바탕으로 사용자 사전을 구축하여 등록하였다. 또한, 불용어 사전 (stopword)을 구축하여 검색어 자체인 “환자”와 “안전”을 키워드에서 제외하였고, “지금”, “등등”, “하다” 등 분석에 있어 특별한 의미가 없는 단어들을 삭제하였다.

성능/효과

그 결과 “의료행위”, “의료인력”, “감염 및 시설”, “간호·간병통합서비스”, “의약품”, “개선을 위한 시스템 개발 및 구축”, “환자안전법”, “의료기관 인증”의 8가지 주제를 도출할 수 있었다

또한, 환자안전법 시행 이전에는 환자안전 인식, 감염관리, 의료기관 인증 등에 중점을 두었다면 환자안전법 시행 이후에는 환자안전 문화와 투약이 강조되었으며, 간호의 중요도가 높아진 것을 확인할 수 있었다. 이러한 결과는 환자안전 향상을 위한 다양한 중재 프로그램 및 정책을 개발하기 위한 기초 자료로 활용될 수 있을 것이다.
