환자 의료정보 접근을 위한 상황인식 기반의 인증서비스 모델 개발 Development of Authentication Service Model Based Context-Awareness for Accessing Patient's Medical Information원문보기
최근 유비쿼터스 기반의 의료 및 헬스케어 환경이 구축됨에 따라 다양한 센서들로부터 상황정보를 얻을 수 있는 의료정보시스템이 증가되고 있다. 이러한 상황인식 기반의 의료정보시스템 환경에서는 상황정보를 이용하여 환자상황을 정상 또는 응급으로 판별이 가능하며, 또한 의료진은 스마트기기의 애플리케이션을 통해 ID와 Password를 이용한 간단한 사용자인증이후 환자정보에 쉽게 접근이 가능하다. 그러나 이러한 인증서비스 및 환자정보접근서비스는 의료진 중심의 시스템이며, 유비쿼터스 기반의 의료정보시스템 환경을 충분히 고려하지 못한다. 이에 본 논문에서는 의료정보에 접근하는 사용자에 대해 기존 의료진 중심의 인증과는 달리 상황정보 중심의 인증서비스를 제공하기 위한 상황인식 기반의 인증서비스 모델을 제시하고 구현하였다. 본 논문에서 제안한 상황인식 기반의 인증서비스 모델은 센서들을 통해 환자상황을 인식하여 환자상황에 의하여 의료진의 인증방식 및 권한부여를 다르게 진행하는 서비스이다. 다양한 상황정보 측정환경을 구성하기 위해 웨어러블 및 생체데이터측정모듈, 카메라센서 등을 이용하여 구현하였다. 환자상황이 응급상황일 경우, 의료정보서버는 담당의료진의 스마트기기에 응급메시지를 전송하고, 응급메시지를 받은 담당의료진은 환자정보에 접근하기 위해 스마트기기의 애플리케이션을 이용하여 인증을 시도한다. 모든 인증이 완료되면 의료진은 상위등급 의료정보 접근권한을 부여받아 정상상황에서 볼 수 없었던 환자 의료정보까지 확인이 가능하도록 하였다. 제안한 상황인식 기반의 인증서비스 모델을 통하여 유비쿼터스 의료정보시스템 환경을 충분히 고려할 뿐만 아니라, 환자중심의 시스템적인 보안 및 접근투명성을 강화하였다.

In the medical information system environment based on context-awareness, the patient situation can be determined as normal or emergency using situational information. In addition, medical staff can easily access patient information after simple user authentication using ID and Password through applications on smart devices. However, these services of authentication and patient information access are staff-oriented systems and do not fully consider the ubiquitous-based healthcare information system environment. In this paper, we present a authentication service model based context-awareness system for providing situational information-driven authentication services to users who access medical information, and implemented proposed system. The authentication service model based context-awareness system is a service that recognizes patient situations through sensors and the authentication and authorization of medical staff proceed differently according to patient situations. It was implemented using wearables, biometric data measurement modules, camera sensors, etc. to configure various situational information measurement environments. If the patient situation was emergency situation, the medical information server sent an emergency message to the smart device of the medical staff, and the medical staff that received the emergency message tried to authenticate using the application of the smart device to access the patient information. Once all authentication was completed, medical staff will be given access to high-level medical information and can even checked patient medical information that could not be seen under normal situation. 그러나 위와 같이 상황인식 기반의 의료정보시스템 연구들은 환자 정보에 접근할 때에 사용자인증과정 및 제한적 환자 정보 접근에 대해 환자 상황이 적용되지 않는다. 이에 본 논문에서는 환자 상황을 적용한 인증 서비스 및 환자 정보 접근 서비스를 제공하는 모델을 제안 및 구현하고자 한다.
제안 방법
본 논문에서는 유비쿼터스 의료정보시스템 환경에서 상황 정보를 고려한 환자 중심의 인증 서비스 및 의료정보 접근 서비스와 서비스 모델을 개발하였다. 다양한 의료정보지원환경을 위해 웨어러블 기기 및 병동에 설치된 센서들을 이용하여 상황 정보를 수신하고, 상황 정보를 통해 환자 상황을 실시간으로 판별하였다. 이에 모든 시스템을 환자 상황 판별을 기준으로 상황인식 기반 인증 서비스 및 의료정보접근 서비스 모델을 설계하였다.

성능/효과
실제 병원에서는 많은 환자들 관리하고, 환자마다 상태가 각기 다르기 때문에 각 환자에 맞게 응급상황 수치를 일일이 수정하기에는 한계가 있다. 두 번째로, 병동에 설치된 수많은 센서들에 대한 관리가 필요하다. 유비쿼터스 의료정보시스템에서 설치된 많은 센서들을 관리할 시스템이 필요하며, 센서들이 생성하는 데이터들의 트래픽 또한 관리가 필요하다.
환자 상황이 응급상황일 경우 응급 메시지 알림을 통해 보호자 및 의료진에게 응급 메시지를 전달하게 하였다. 또한 응급상황에서 의료진이 인증요청 시 추가 인증요소 입력 및 스마트 기기인증에 대한 사용자개입을 최소화하여 응급상황에서도 빠른 인증 서비스를 제공하였다. 또한 인증 이후 환자 상황 및 의료진 역할에 따라 의료정보 접근 권한을 부여하여 제한적 접근뿐만 아니라 상황 정보를 충분히 고려한 의료정보접근 서비스를 개발하였다.
또한 응급상황에서 의료진이 인증요청 시 추가 인증요소 입력 및 스마트 기기인증에 대한 사용자개입을 최소화하여 응급상황에서도 빠른 인증 서비스를 제공하였다. 또한 인증 이후 환자 상황 및 의료진 역할에 따라 의료정보 접근 권한을 부여하여 제한적 접근뿐만 아니라 상황 정보를 충분히 고려한 의료정보접근 서비스를 개발하였다.

이에 대한 구축한 시스템적 한계를 보안하고자 향후 연구 방향으로 환자 맞춤형 응급상황 수치 적용을 위한 빅데이터 기법을 연구할 계획이다. 또한 수많은 센서들의 관리 및 빠른 응급상황 판단과 의료정보 서버의 업무를 분산하고자 사용자와 의료정보 서버 사이의 엣징컴퓨팅을 적용한 상황인식 기반의 의료정보시스템을 연구할 계획이다.
유비쿼터스 의료정보시스템에서 설치된 많은 센서들을 관리할 시스템이 필요하며, 센서들이 생성하는 데이터들의 트래픽 또한 관리가 필요하다. 마지막으로 더욱 빠르고 안전한 응급상황 판단이 필요하다. 센서와 의료정보 서버 사이의 물리적 거리로 인해 응급상황 판단이 지연될 수 있고, 또한 의료정보 서버의 과부하로 인한 시스템의 오작동을 고려 해야 한다.
첫째로, 모든 환자에게 일관된 응급상황 수치를 적용할 수 없다. 실제 병원에서는 많은 환자들 관리하고, 환자마다 상태가 각기 다르기 때문에 각 환자에 맞게 응급상황 수치를 일일이 수정하기에는 한계가 있다. 두 번째로, 병동에 설치된 수많은 센서들에 대한 관리가 필요하다.
이에 대한 구축한 시스템적 한계를 보안하고자 향후 연구 방향으로 환자 맞춤형 응급상황 수치 적용을 위한 빅데이터 기법을 연구할 계획이다. 또한 수많은 센서들의 관리 및 빠른 응급상황 판단과 의료정보 서버의 업무를 분산하고자 사용자와 의료정보 서버 사이의 엣징컴퓨팅을 적용한 상황인식 기반의 의료정보시스템을 연구할 계획이다.
