Purpose : This systematic review examined evidence published 2000-2021 for occupational therapy interventions for adult Attention Deficit Hyperactivity Disorder (adult ADHD). Methods : In order to find out the studies on the effect occupational therapy intervention on individuals with adult ADHD, studies published from 2000 to February 2022 were searched on Google Academic Search. The keywords used were "adult ADHD" AND "Occupational therapy". A total of 785 studies were searched, and from these, 3 studies were selected based on the application of the selection and exclusion criteria. The included studies comprised of 1 Quasi-experimental design study(one group pre-post test) and 2 randomized controlled trials design studies. The included studies were analyzed in PICO(patients, intervention, comparison, outcomes). Results : As a result of analysing the subjects of the included studies, subjects diagnosed with adult ADHD or classified as high-risk group in Adult ADHD Self-Report Scale test, the number of subjects was 97, and the average age was about 33 years old. Occupational therapy intervention methods for adult ADHD include Tailored Intervention, Computerized Cognitive Training, and Cognitive-Functional Intervention. The Adult ADHD Self-Report Scale was the most used to check the symptoms of ADHD, and the COPM was used the most to measure the effects of occupational therapy interventions. The dependent variables of occupational therapy intervention were occupational performance, quality of life, executive function (behavioral regulation, metacognition), self-regulation function, stress management, and sustained attention. The effect of the intervention was statistically significant in 78.5% of the total. Conclusion : It is expected that the results of this study can be used as basic data for evidence-based practice for occupational therapists to apply adult ADHD interventions. In the field of occupational therapy in Korea, research on various interventions that can improve the occupational performance of adults ADHD should be conducted.

제안 방법

대상 연구에서 실시한 성인 ADHD를 위한 작업치료중재방법으로는 맞춤형 중재(Tailored Intervention), 컴퓨터 인지훈련(Computerized Cognitive Training), 인지기능 중재(Cognitive–Functional Intervention)가 있었다
둘째, 성인 ADHD를 대상으로 한 작업치료 중재 연구를 PICO(Patient, Intervention, Comparison, Outcome) 형식에 따라 체계적으로 분석하고, 중재효과의 근거를 마련하고자 한다.
선정된 문헌의 내용은 PICO(Patient, Intervention, Comparison, Outcome)로 분석하여 제시하였다. 연구 대상자는 수, 연령 등을 분석하였고, 작업치료 중재방법은 중재 제공자 및 중재방법에 대하여 분석하였다. 성과측정은 표준화된 평가도구와 비표준화된 성과측정방법을 나누어 분석하였고, 결과에 대한 분석은 대상자 모두에게 효과가 있었던 경우에 ‘효과있음’, 일부 대상자에게만 효과가 있었던 경우에 ‘일부 효과있음’, 효과가 없었거나 확인이 불가한 경우 ‘효과없음’으로 표기하였다.

검색어 검색으로 785개의 연구가 검색되었으며, 제목과 초록 확인을 통하여 14개 연구가 선별되었다. 14개 연구는 원문을 확인하여, 최종 4개 연구를 선정하였다. 제외된 연구는 작업치료 중재인지 모호한 연구 4편, 원문을 구하지 못한 연구 1편, 대상자에 성인 ADHD가 포함되어 있으나 주 대상자가 아닌 연구가 1편, 질적연구 1편, 문헌고찰 1편, 예비연구 2편이었다(Figure 1).

이론/모형

대상 연구에서 ADHD 증상을 확인하기 위해 사용된평가도구는 2개(Adult ADHD Self-Report Scale: ASRS; Wender Utah Rating Scale: WURS)이었다. 표준화된 평가도구는 5개(Perceived Stress Scale; PSS, Canadian Occupational Performance Measure; COPM, Behavior Rating Inventory of Executive Function–Adult Version; BRIEF-A, Adult ADHD Quality-of-Life Scale; AAQoL, Self-Regulation Skills Interview; SRSI)가 사용되었다.
대상 연구의 내용은 PICO(patients, intervention, comparison, outcomes)로 분석하였다. PICO의 틀은 대상 자(patient), 중재(intervention), 대조군(comparison), 결과(outcome)로 분석하였다(Appendix 1).
선정된 연구의 질 평가는 Physiotherapy Evidence Database scale (PEDro scale)을 사용하였다(PEDro, 1999). PEDro scale은 내적 타당도에 관한 11개의 문항으로 질적 평가를 할 수 있는 도구이며, 1번 문항을 만족해야 나머지 10문항을 점검할 수 있다.
성인 ADHD를 대상으로 한 작업치료 효과를 분석한 논문을 수집하기 위하여 구글학술검색(google scholar)의 검색엔진을 사용하였다. 2022년 2월에 검색하였고, 검색을 위한 용어는 “adult ADHD” AND “Occupational therapy”를 사용하였다.

컴퓨터 인지훈련은 국내에서는 정신분열증(Cho 등, 2013), 뇌졸중(Kim 등, 2014), 치매환자(Kang 등, 2009)에게 적용하는 것이 일반적이며, 성인 ADHD에게 적용한 근거는 없다. 본 연구에서 성인 ADHD에게 컴퓨터 인지훈련을 적용하여 실행기능과 작업수행 만족도 증진, ADHD증상 완화, 유지집중력 향상에 유의미한 차이를 보였다(Stern 등, 2016). 맞춤형 중재에 대한 국내 연구에서 Lee 등(2020)이 주간보호센터의 치매노인에게 적용하여 인지기능과 신체기능의 향상, 우울의 감소에 효과가 있다는 근거를 마련하였다.

이는 연구대상자 모집과 선별의 어려움으로 인한 것이 아닌가 사료된다. 국내 연구에서도 성인 ADHD의 연령층을 확대하여 연구할 필요성이 있다.
선정된 대상 연구의 절대적인 수가 적어 성인 ADHD 작업치료 중재의 근거 수준을 논하기에는 무리가있는 것으로 사료된다. 국외 작업치료 분야에서 성인 ADHD와 관련된 연구가 활발히 이루어지는 시작 시점으로 판단되며, 국내에서도 이러한 추세에 발맞추어 성인ADHD를 대상으로 한 다양한 근거를 제시할 필요성이 있다. 문헌의 선정과 배제 기준에 따라 제외되었던 성인 ADHD 대상 작업치료 중재에 관한 연구를 살펴보면 예비 연구가 2편(Gutman 등, 2019; Maeir 등, 2019), 성인ADHD가 포함되어 있으나 주 대상자가 아닌 연구(Lee 등, 2017)가 1편, 질적연구(Ek & Isaksson, 2013) 1편, 문 헌고찰(Adamou 등, 2021) 1편이 있었다.
본 연구는 성인 ADHD를 위한 작업치료의 중재방법에 대한 근거와 정보를 제시하였다는 데 그 의의가 있다. 작업치료사들이 성인 ADHD 중재를 적용하기 위한 근거기반 임상의 기초자료로 활용될 수 있기를 기대하며, 국내 작업치료 분야에서도 성인 ADHD의 작업수행을 향상시킬 수 있는 다양한 중재법에 관한 연구가 활발히 이루어지기를 희망한다.
이는 성인 ADHD를 대상으로 한 작업치료 중재의 효과를 알 수 있는 결과이다. 하지만 포함된 연구의 수가 3편으로 제한적이기 때문에 이 결과를 일반화할 수 없는 제한점이 있다.
