RésuméCas cliniqueNous relatons un cas de méningite à Fusobacterium Nucleatum chez un enfant de 19 mois traité depuis 24 heures par macrolide pour une otite purulente.CommentairesLes germes anaérobies sont responsables d'environ 10 % des infections bactériennes et de moins de 1 % des méningites bactériennes. Cette proportion est probablement sousestimée, du fait de conditions particulières de prélèvement et de culture pour les germes anaérobies. Le pronostic vital et fonctionnel des méningites à anaérobies est lié au terrain sur lequel elles surviennent, et à la mise en œuvre rapide d'une antibiothérapie adaptée. La recherche et le traitement d'une pathologie locorégionale sont indispensables. Le métronidazole est l'antibiotique de choix. Il doit être associé à une céphalosporine de troisième génération en première intention, du fait de la présence possible d'autres germes et de l'apparition de pénicillinases.
