목적: 한국인 염증성 장질환 환자에서 AZA/6-MP 치료에 따른 부작용 빈도와 그 경과에 대해서 알아보고자 하였다. 대상 및 방법: 1996년 6월부터 2006년 9월까지 크론병, 궤양성 대장염, 베체트장염을 진단받고 AZA/6-MP 치료를 받고 12개월 이상 추적 관찰한 환자를 대상으로 후향 코호트연구를 시행하였다. 결과: 133명의 AZA/6-MP 치료 환자 중에서 남자는 75명(56.4%)였다. 평균나이는 31.7±10.9세였다. AZA/6-MP를 사용하기까지 유병기간은 평균 46.2±35.8개월이었다. AZA/6-MP를 투여하기 시작한 후 최대 관용 용량을 투여하기까지 평균 15.7±32.8주가 소요되었고, AZA/6-MP의 평균 투여 기간은 32.5±27.0개월이었다. 부작용은 백혈구 감소증 75예(56.4%), 오심/구토 32예(24.1%), 관절통 6예(4.5%), 간염 6예(4.5%), 피부발진 4예(3.0%), 대상포진 3예(2.3%), 그리고, 두통 1예(0.8%)가 있었다. 백혈구 감소증은 최대 관용 용량투여 후 3개월 이내에 가장 많이 발생하였으며(58.7%), 오심은 대부분 투여 초기 3개월 이내에 발생하였다(81.3%). 부작용으로 인해 AZA/6-MP 치료를 중단한 경우는 38예(28.6%)가 있었다. Background/Aims: This study was to evaluate the frequency and the course of the adverse effects of AZA/6-MP in Korean patients with inflammatory bowel disease (IBD). Methods: Medical records of the patients with IBD treated with AZA/6-MP at Severance hospital from June 1996 to September 2006 were retrospectively analyzed. Results: A total of 133 patients were studied. Male to female ratio was 1.3:1. The mean age was 31.7±10.9 year. Adverse effects included leukopenia occurred in 75 cases (56.4%), nausea/vomiting in 32 cases (24.1%), arthralgia in 6 cases (4.5%), hepatitis in 6 cases (4.5%), skin rash in 4 cases (3.0%), herpes zoster in 3 cases (2.3%), and headache in 1 case (0.8%). Most of leucopenia (58.7%) developed within 3 months after maximal tolerated dose of AZA/6-MP and nausea/vomiting frequently occurred within 3 months after start of AZA/6-MP treatment. Thirty-eight patients (28.6%) required the discontinuation of medication due to adverse effects. Conclusions: Leukopenia was the most common adverse effect of AZA/6-MP treatment. Leukopenia and nausea/vomiting developed frequently in the early period of treatment of AZA/6-MP in patients with IBD. AZA/6-MP should be used cautiously to scrutinize bone marrow suppression.
