목적: 비구이형성증의 치료에 시행한 회전 비구절골술의 중장기 추시결과에 대하여 알아보고자 하였다.
대상 및 방법: 1987년 1월부터 1998년 8월까지 회전 비구 절골술을 시행하고 5년 이상 추시관찰이 가능하였던 36예에 대하여 임상적 및 방사선학적 결과를 분석하였다. 추시기간은 평균 8.6년이었다. 임상적 결과는 d’Aubign涌?Postel의 고관절 점수를 이용하여 동통, 보행능력, 관절가동성을 측정하였으며, 방사선학적 결과는 Wiberg의 C-E angle, 비구개경사도, head lateralization index, 그리고 T?nis의 관절염 등급을 측정하였다.
결과: Postel score는 수술 전 평균 12.5점에서 마지막 추시시 평균 15.7점으로 향상되었다. 모든 방사선학적 지표들도 수술 후 향상되었다. 5예(13.1%)에서 수술 전과 비교하여 관절염 등급이 악화되었으며, 이들은 모두 수술 전 관절염 등급이 2인 경우였다.
결론: 회전 비구절골술은 경도의 퇴행성 관절염을 갖고 있는 비구이형성증 환자에서 관절염의 진행을 예방하는데 도움이 되나 진행된 관절염이 동반되어 있을 경우에는 만족스럽지 못한 결과를 보인다.

Purpose: To evaluate the midterm result of patients who underwent rotational acetabular osteotomy for the treatment of acetabular dysplasia. Materials and Methods: The clinical and radiographic outcomes of 36 patients, who underwent rotational acetabular osteotomy between January 1987 and September 1998, were evaluated after a minimum follow-up of five years. The mean follow-up period was 8.6 years. The clinical evaluation included recording the level of pain, the ambulation status and range of motion according to the d"Aubign?and Postel system. Radiographic analysis included the C-E angle of Wiberg, the acetabular roof obliquity, and arthritis grade of Tennis. Results: The average Postel score increased from 12.5 points preoperatively to 15.7 points at the final follow up. All the radiologic indices improved after surgery. In five hips (13.1%), there was a progression of arthritis prior to surgery. Conclusion: Rotational acetabular osteotomy is a valuable procedure for preventing the progression of early osteoarthritis in patients with a dysplastic hip. However the results were unsatisfactory for hips with more advanced osteoarthritis.
