목적: 대퇴 비구 충돌을 일으키는 대퇴 경부의 pistol grip 변형을 확인하는데 이용되는 다양한 방사선 촬영 중 양측 고관절 단순 전후방 방사선 사진을 이용한 일차적인 감별진단 및 유용성에 대해 알아보고자 하였다. 대상 및 방법: 2004년 3월부터 2006년 3월까지 대퇴 비구 충돌증후군으로 관절경적 치료를 시행받은 20세에서 50세까지의 27명의 환자(양측 대퇴 비구 충돌군 5예)를 대상으로 시행한 수술전 단순 방사선 촬영(Both Hip AP, Frog leg lateral, Groin lateral, False profile)중 일차 으로 양측 고관절 단순 전후방 방사선 사진에서 대퇴골 경부의 가장 협소한 부위와 대퇴골 두 지름, 그리고 두 선사이의 이분점 (이 부위는 대퇴경부 전상방 융기 부위로 추정)에서의 대퇴골 두 길이를 측정하였다. 대조군으로는 고관절 동통을 호소하지 않는 동일 연령군의 15명의 정상인(30예)에서 양측 고관절 단순 전후방 방사선 사진에서 실험군과 같은 방법으로 측정한 3 가지의 길이의 비를 각각 서로 비교 하였다. 결과: 실험군에서의 대퇴골 두 지름은 평균 53.45 mm, 최소 경부 길이는 37.58 mm, 두 선의 중간점의 대퇴골 두 길이는 41.52 mm 였으며, 대조군에서의 대퇴골 두 지름은 53.38 mm, 최소 경부 길이는 35.43 mm, 그리고 두선의 중간의 대퇴골 두길이는 39.03 mm 였다. 대퇴골 두 지름과 최소 경부 길이의 비는 실험군 0.67, 대조군 0.65로 비슷하였으나, 대퇴골 두 지름과 중간점의 대퇴골 두 길이비는 실험군 0.84, 대조군 0.75이었다(p=0.01). 결론: 고관절의 동통을 주소로 내원한 환자에서 단순 전후방 방사선 사진 상 pistol grip 변형을 의심 할수 있는 실질적인 수치를 확인코자 하였으며, 대퇴골 두 직경과 골두와 경부 중간부위 직경 비율이 0.8이상인 경우 비구 충돌증후군을 진단하는데 일차적인 지표로 유용할 수 있었다.

Results: In the FAI group, the femoral head diameter was 53.45 mm, the minimal femoral neck length was 37.58 mm and the femoral head length at the midpoint was 41.52 mm. In the control group, the femoral head diameter was 53.38 mm, the minimal femoral neck length was 35.43 mm and the femoral head length at the midpoint was 39.03 mm.
