이 연구의 목적은 남한 거주 후 1년이 지난 정착 초기의 북한이탈주민의 우울 및 불안, 외상 후 스트레스의 정도와 이러한 정신건강 문제에 영향을 미치는 변인들을 알아보는 것이다. 인구사회학적 특성들과 외상경험 등의 예측변인들과 정신건강 간의 관계를 알아보았다.
외상 후 스트레스 장애는 조사응답자의 약 5% 정도, 우울 및 불안은 약 48%의 유병율을 보였다. 정신건강 문제는 여성들이 남성보다 취약한 것으로 나타났고, full-PTSD는 모두 여성인 것으로 나타났다. 외상경험은 외상 후 스트레스 장애와 우울 및 불안장애 모두와 긴밀한 관계를 보여 외상 경험이 북한이탈주민의 정신건강에 주요 영향요인임을 알 수 있다. 특히 외상경험이 총격이나 추격, 구금, 고문, 인신매매, 북송, 가족의 상실 등과 같은 심각한 폭력, 생명의 위협, 성적 폭력, 친밀한 관계의 상실 등일 경우 정신건강 문제에 영향을 많이 주는 것으로 나타났다. 정신건강 문제는 응답자의 초기 근로능력인식과 유의한 관계를 보여주어 정신건강문제가 경제적 적응에 영향을 미칠 것을 예측할 수 있다.
본 연구가 갖는 의의는 정착기간이란 중요한 시간적 변인을 통제한 상태에서 최근에 남한에 입국한 북한이탈주민의 정신건강 문제의 정도와 이들의 정신건강의 위협요인을 발견함으로써 정신건강문제를 조기에 발견할 수 있는 방안과, 고 위험군에 대한 다양한 개입 전략과 북한이탈주민을 위한 서비스 전달체계에 대한 정책을 제안할 수 있었다는 점이다.

The purposes of this study were to investigate the prevalence rates of PTSD, and depression and anxiety among the North Koreans arrived in the South in 2007, and to identify factors influencing their mental health condition. The relationships between socio-demographic factors and trauma exposure level, and mental health condition were examined. The prevalence rates of PTSD, and depression and anxiety were 5% and 48%, respectively. The mental health prevalence rates in the current study subjects were lower than those of the earlier arrivals reported in the previous studies, which suggests a conclusion that recent arrivals are in better mental health conditions than those who arrived 5-7 years earlier. The findings of this study show that women are more susceptible to mental health problems. Trauma exposure was related with poor mental health conditions and atrocious experiences such as incarceration, torture, being a victim of human trafficking, arrest and extradition to N. Korea, loss of family members were attributable to adverse mental health conditions. Mental health problems were associated with negative self-evaluation of ability to work and intent to work, which may result in unemployment and dependence on the long run. The implications of the study findings for mental health service strategy and the resettlement policies for the mental health high risk groups were discussed at the conclusion.
