Accidents or diseases can lead to paralysis of part of all of thebody, which results in loss of mobility. However, when electrical stimulation is appliedto nerves or muscles of some upper motoneuron injured patients, they can recovermobility such as grasping, standing and walking that might be of great help to theirdaily lives. This is called functional electrical (or neuromuscular) stimulation (FES).Our ultimate goal is to develop a portable stimulator system with which paraplegicpatients can stand up from a wheelchair, maintain the upright postion and walk forthemselves. In order to fulfil such a goal, we proposed a method to derive thefatigue-minimizing stimulation pattern, clinically applied the stimulation pattern to aparaplegic pateint, and then utilized the results to investigate the feasibility of oursystem.First, we developed and used a planar human body model for FES-inducedparaplegic standing and walking to provide a criterion to analyze the mechanical functionof a muscle. Also, we developed an interface which tranfers the stulation patternderived by the computer to the stimulation system, and developed a first-stage8-channel constant-voltage stimulator for clinical application. Our FES exercise program led to more than 10-fold increase of the paralyzedquadriceps and increased some physiological functions of the patient such as sensationand leg-lifting ability. The patient can now stand up from a wheelchair, maintain theupright position for at least 1 min 45 sec, and walk for the same period of time withour 4-channel surface-electrode FES system. Our theoretical and clinical results can, we believe, provide a background sufficientto develop a portable FES system. Furthermore, our current system can generate theoptimal stimulation pattern for individual paraplegic patient.
