본연구는 국내의 위질환환자와 정상생활자로부터 분리한 H. pylori을 대상으로 병원성의 정도와 관련이 있다고 보고된 cagA와 vacA genotype을 결정하고자하였다. RAPD fingerprinting, PCR-RFLP를 실시하여 개개별 H. pylori 균주의 genotype을 분석하여 단일환자에 감염된 세균이 동일한 strain인가, 또한 생검체별로 그리고 감염기간 경과에 따라 분리한 세균이 동일한 균주인가를 조사하여 H. pylori 감염의 분자역학적인상황을 파악하고 genotyping으로 가족내의 감염원을 추적

본연구는 국내의 위질환환자와 정상생활자로부터 분리한 H. pylori을 대상으로 병원성의 정도와 관련이 있다고 보고된 cagA와 vacA genotype을 결정하고자하였다. RAPD fingerprinting, PCR-RFLP를 실시하여 개개별 H. pylori 균주의 genotype을 분석하여 단일환자에 감염된 세균이 동일한 strain인가, 또한 생검체별로 그리고 감염기간 경과에 따라 분리한 세균이 동일한 균주인가를 조사하여 H. pylori 감염의 분자역학적인상황을 파악하고 genotyping으로 가족내의 감염원을 추적하여 H. pylori의 감염경로를 확인하고자하였다.각종 위질환환자를 대상으로 36주와 위질환의 증상을 호소하지 않은 정상생활자로부터107주의 H. pylori를 분리하여 본실험에 사용하였다.cagA genotype을 PCR을 이용하여 결정하였으며 그 결과 본실험에서 사용한 143주 모두가 cagA를 보유하고 있었다. vacA genotype을 위하여 vacA의 signal sequence의 종류를 구분하는 s1a, s1b, s2를 확인할 수 있는 PCR을 실시한 결과 모든 분리주가 s1a type으로 판정이 되었다. 따라서 본연구에서 분리한 H. pylori 균주는 모두 병원성이 높다고 알려진 cagA 및 vagA s1a 유전자형임이 확인되었다. 또한 외국의 H. pylori 분리주에서 확인되고 있는 IS element인 RS2의 존재를 PCR을 통하여 확인한 결과 143 분리주중 12주(8.4%)가 양성으로 나타나 국내의 균주에서도 IS element를 보유한 H. pylori 균주가 있다는 것을 확인하였다.Primer #1247, #1254, #1281, #1283을 이용한 RAPD fingerprinting으로 genotype을 분석한 결과 143주 모두가 특정한 유전자형을 나타내고 있었다. 또한 flaA segment를 표적으로PCR을 실시하고 그 산물을 HhaI과 MboI으로 처리하여 절단된 DNA pattern을 분석하여genotype을 결정하는 PCR-RFLP를 실시한 결과 HhaI 제한형이 32종류 그리고 MboI 제한형이 22종류로 확인되었다. 그러나 RAPD fingerprinting과 PCR-RFLP으로 분석 및 확인되는 genotype이 특정 임상증상과 관련이 있는 경우는 없었다.단일환자에 한개의 H. pylori 균주 또는 여러 균주가 감염되는 지를 확인하기 위하여30명의 환자에서 2-3개의 H. pylori 분리주를 얻어 RAPD fingerprinting을 실시하였다. 그결과 26명 (87%)은 단일 개체에서 얻은 분리주들은 모두 같은 genotype을 나타내었고 4명(13%)은 단일개체에서 얻은 분리주중 일부는 다른 genotype을 나타내었다. 또한 단일개체의 생검체 부위별 그리고 1년간의 기간을 두고 분리한 세균의 동일성을 확인하기 위하여 5명의 개체에서 1년의 시차를 두고 개인당 body에서 2개 antrum에서 2개의 생검체를 절취하였다. 절취한 생검체당 2-4개의 H. pylori를 분리하여 genotype을 분석하였다. 그 결과 4명은 생검체의 위치 그리고 시간의 경과에 관계없이 개개별로 분리한 세균은 모두 동일한균주임이 확인되었으나 나머지 한사람은 1년뒤에 절취한 생검체 중 한 개에서 분리한 세균이 그외의 시료에서 분리한 세균과 다른 genotype을 나타내었다.부모와 자식이 포함된 3 가족으로부터 얻은 H. pylori 분리주의 genotype을 RAPDfingerprinting 그리고 PCR-RFLP를 이용하여 분석한 결과 6명의 자식중 3명은 아버지 또는어머니가 감염된 균주와 동일한 genotype을 나타내었다.결론적으로 환자와 정상생활자로부터 얻은 H. pylori 분리주는 모두 병원성이 높다고알려진 cagA 및 vagA s1a 유전자형으로 판명되어 한국인은 모두 병원성이 높은 H. pylori균주로 감염되어 있다는 것을 확인하였다. H. pylori genomic DNA는 diversity가 높아 환자별로 분리한 H. pylori의 genotype이 각각 달랐으나 각개체는 대부분 단일 균주로 감염되어 있고 감염기간의 경과에도 그대로 유지되었다. 그리고 일부에서 혼합감염 또는 중복감염될 가능성도 확인되었다. 부모의 균주가 어린이에게로 전염되는 것이 확인되어 H. pylori감염은 주로 가족내 에서 전염된다는 사실을 확인할 수 있었다.

This study aims to get knowleges of molecular epidemiology ofH. pylori strains isolated from patients with gastroduodenal disorders and normal heathypersons of Korea, in terms of cagA and vacA genotypes. And genotypic persistence ofH. pylori isolates obtained in follow-up specimens of one year interval was shown byRAPD fingerprinting. In addition, the evidence for intrafamilial transmisson of H. pyloriinfection will presented by genotypic identity of H. pylori isolates recovered withinfamilies.Thirty-six clinical isolates of H. pylori were recovered from gastric biopsies ofpatients with gastric disorders. Also 107 isolates of H. pylori were isolated from normalhealthy persons who did not complain of gastric symptoms.PCR genotyping for cagA genotyping showed that all H. pylori isolates tested inthis study are CagA positive strains. For vacA genotyping, the genomic DNAs of H.pylori isolates were subjected to PCR to differentiate type s1a, s1b, and s2 vacA signalsequences. All the isolates tested in this study were confirmed as type s1 genotype,indicating that they are toxigenic type. Among of 143 strains test, 12 isolates (8.4%)were confirmed as containing the RS2 element.Genomic DNAs of 143 H. pylori isolates were applied to genotyping using RAPDfingerprinting and PCR-RFLP. Each 143 isolates showed a unique RAPD profile.PCR-based RFLP was done to assess the sequence diversity of known H. pylori genesof flagella From 143 H. pylori isolates 32 distinct patterns were found with HhaIdigestion of 1.5 kb flaA segment and 22 distinct patterns were with MboI digestion.None of amplified bands appeared in RAPD fingerprinting were analyzed to bedisease-specific. And there were no RFLP profile which is associated with specificsymptoms of gastric disorders.Different isolates obtained from each of 26 (87%) among 30 persons tested in thisstudy were confirmed to be an identical genotype and those of 4 (13%) were showngenomic heterogenity between isolates originated from a person. These results indicatedthat most of isolates which could be recovered from an individual subject are identicalgenotypic strains and parts of population can be co-infected with different H. pyloristrains. Among 5 persons attended in follow-up study, isolates obtained from each of 4persons appeared to be an identical genotype irrespective of the anatomic site of biopsyspeicmens and the infection duration. However, in one person three different Isolatesobtained from one of the follow-up specimens a period of 1 year showed differentgenotype from 23 isolates of other specimens of the inital and the follow-up specimens.These results manifested that H. pylori isolates are generally maintained in vivo duringinfection for a long time. And superinfection with other H. pylori strains can beassumed to be possible during infection. Genotypes of all or part of isolates recoveredfrom 3 of 6 children were shown to be identical to those of their parents, suggestingthat some children in families acquires H. pylori infection from their parents.In conclusions, all H. pylori isolates from Korean are a strain of cagA and s1avacA genotype which is alleged to be more virulent than other strain irrespect ofclinical symptoms and disease severity. Genotype of H. pylori shows extremelydiversity among isolates from different patients. Each person was infected with a singlegenotypic strain. And in minority mixed infection and superinfection by different straincan be possible. Children infection is mainly transmitted by parent's H. pylori strainswithin family.
