제1세부과제 : 결핵균 항원의 특성 규명 및 진단기술개발 결핵의 혈청진단에 응용하기 위하여 예비평가를 마친 항원은 culture filtrate(CF), lipoarabinomannan(LAM), PPD, diacyltrehalose(DAT)등 결핵균체 또는 배양액을 농축한 항원으로 혈청진단 민감도는 60%-75% 였으며, 특이도는 73%-93%로 항원에 따라 차이가 많았다. 제 3세부과제에서 유전자재조합기법으로 생산한 r16kDa, r19kDa, rAg85C, r38kDa 항원을 이용한 혈청진단 응용성 평가에서는 민감도는 16%-85%, 그리고 특이도는 97%-100%였다. 이상의 결과로 보면 결핵균에서 분리정제한 항원가운데는 DAT항원, 그리고 유전자재조합기법으로 생산한 항원 가운데는 r38kDa 항원이 가장 유용할 것으로 예상되며, 두 개 이상의 항원을 혼합할 경우 민감도를 더욱 높일수 있을 것이다. 한편 객담에서 항원검출검사는 민감도가 53%로 도말검경검사의 20-30% 보다는 월등히 우수하였으며, 배양음성객담에서의 항원양성을 20%의 의미를 규명한다면 항원검사의 실험실 진단 응용성은 매우 높을 것이다. 제2세부과제 : 결핵균체 Triton 용해 단백질 항원의 면역학적 유용성 TSP 항원성분에는 단백질 뿐만아니라 지질성분, 당성분등의 다양한 항원성분을 포함하 고 있었다. 일부의 단백질은 세포질분화에 존재하였으나 대부분은 결핵균 외막에 존재하는 항원이었다. 또한 결핵균체로부터 TSP항원의 추출방법을 개선하였고 결핵균배양액과의 성분 비교시 뚜렷한 차이를 나타내었다. 특히 본 연구에서 적용한 추출방법은 항원성분의 변성이 없이 분리할 수 있다는 것을 간접적으로 증명할수 있었다. 특히 Triton X-114 phase separation을 통해 LAM과 LM같은 glycoconjugate 성분들을 효과적으로 제거할수 있었다. 또한 이들 성분이 제거된 TSP 수용성향원은 결핵환자에서 미약한 항체반응을 나타낸 반면 PPD양성 건강인의 말초혈액림프구에 대해 PPD항원 보다 유의하게 높은 증식능을 나타내었고 IFN-γ의 mRNA 발현을 강력하 게 유도하였다. 따라서 수용성 항원성분은 결핵예방백신 개발을 위한 후보항원으로 인정되며 이들 항원성분으로부터 유효한 항원의 분리를 시도하고 있다. 또한 수용성 항원 분획에 존재하는 36kDA glycoprotein을 정제할수 있었고 기존의 38 kDA항원과 부분적인 동질성이 증명되어 본 항원에 대한 특성분석을 시도하고 있다. 제3세부과제 : 결핵균의 항원유전자 발현 기술을 위한 기초연구 기존의 항결핵예방백신인 BCG를 개발하여 multivalent vaccine으로 이용하기 위한 기초 준비로서, Mycobacterium-E. coli shuttle vector인 pYMC를 제작하였으며, 이를 이용하여 결핵균의 주요 항원유전자인 19, 32, 38 kDa의 각 유전자를 발현하는 recombinant BCG를 제조하였다. 이들 recombinant BCG는 생백신으로서 가능성을 조사하기 위하여 제5세부과제에서의 동물 실험에 사용되었으며, 한편, 발현된 재조합 단백질을 다량제조, 정제함으로써 재조합 단백질의 subunit 백신으로서의 가능성 및 면역진단법에 사용되었을때의 효용성 등이 제1세부과제에서조사 되었다. 또한, 이들 19, 32, 38 kDa의 각 유전자를 대장균과 BCG에서 다량발현하여 정제하고, 이를 이용한 면역진단법에 이용하고, 예방백신으로서의 응용성 평가에 이용케 하였다. 제4세부과제 : 결핵균 세포벽성분 생합성 관련 유전자 클론과 기능분석 Operon 구조상 가장 상류에 위치한 rfbB probe와 결핵균 genomic DNA를 사용하여 Southern blotting을 시행하여 3.5 kb 크기의 PvuII DNA 단편을 발견하였고 이를 클로닝하였다. ApaI, BglII, EcoRV, XhoI, KpnI, HincII, 및 NaeI 등의 제한효소를 사용하여 rfbB 및 rfbC 유전자를 함유하는 3.5 kb의 plasmid를 제한효소지도를?? 후 그 염기서열을 결정하였다. rfbB 및 rfbC 유전자의 염기서열을 결정하여 GeneBank에 보고하였다 (U43540). rfbC 유전자는 결핵균 genome내에 하나의 copy만이 존재하여 추후 신약개발의 적합한 표적임을 알 수 있었다. rfbC 유전자의 하류에는 새로운 반복서열이 존재함을 발견하였다. 이 453 bp의 새로운 반복서열은 M. tuberculosis H37Rv, II37Ra, Erdman, Cannetti에 모두 존재하였고 결핵균 이외에도 M. bovis, M. bovis BCG 등의 M. tuberculosis comples 균주에서 모두 존재하였다. 그러나, M. fortuitum, M. simiae, M. smegmatis, M. kansasii, M. avium, 및 M. phlei등의 항산균 및 그람음성 및 그람양성 일반세균들에는 존재하지 않아 이 반복서열은 결핵균의 PCR검출에 유용하게 사용될 수 있음을 알 수 있었다. 대장균에서 rfbC 변이주를 제조하기 위하여 rfbC 유전자의 상류 및 하류에 위치하는 rfbA 및 rfbX 유전자 그리고 rfbC 유전자를 PCR 클로닝하였다. rfbA 및 rfbX 유전자사이에 FLP효소가 인식하는 염기서열인 FRT를 양쪽에 넣어준 kanamycin 내성유전자를 삽입한 deletion vector를 제작하였다. 이 vector를 사용하여 homologous recombination이 일어난 균주를 kanamycin으로 selection하였다. Kanamycin유전자를 제거하기 위하여 이 균주에 ampicillin과 chlorampenicol 내성유전자를 함유하는 FLP 효소발현 vector를 transformation시켜 ampicillin과 chlorampenicol로 selection하였다. Selection한 콜로니들을 배양하여 FLP가 kanamycin양쪽의 FRT를 인식하여 kanamycin내성 유전자를 excise하도록 한 후 kanamycin 감수성 콜로니들만을 selection하였다. 실제 이와 같이 제조한 균주가 rfbC를 결손하고 있는지를 PCR로 확인하였다. 제5세부과제 : 실험동물을 이용한 결핵의 진단 및 예방백신 평가기술 개발 본 세부과제에서는 결핵의 실험동물모델을 확립하여 결핵의 진단기술 및 예방백신 후보 물질의 응용성평가를 위한 기초연구를 수행하였다. 감염동물실의 완공으로 결핵군 감염실험을 수행할 수 있는 시설의 완공과 BCG균을 이용한 마우스 감염실험 (호흡기감염 포함)을 시도하였으며, 감염후 장기별 생존균수의 측정, 림프구증식반응, 싸이토카인 생산측정 등에 관한 기초연구를 수행하였다. 또한 기니픽을 이용한 여러항원의 피내반응 검사기법을 확립하고 유전자재조합 항원들의 DTH 반응정도를 측정하였다. 토끼나 염소를 이용하여 여러 항원에 대한 항체를 대량생산하여 타세부과제에서 항원의 분석과 항원검출기술을 개발하는데 이용하게 하였다.

Thberculosis is still a major public health problem, killing more than three millions worldwide each year. With a rapid increase in HIV infection and emergence of multidrug resistant Mycobacterium tuberculosis becomes on of the most serious infectious diseases even in the developed countries. Major strategies toward control of tuberculosis include early detection of tuberculosis patients followed by effective treatment and increase in BCG vaccination coverage. However, current diagnostic tests are far from satifaction, and the effectiveness of BCG vaccine varied markedly from 0 to 80% and is still controversial. Thus further imporved diagnostic tests and more effective vaccine development has been among the highest priorities in tuberculosis research. This study was, therefore, initiated to develop and evaluate new laboratory diagnostic tests and vaccine(s) for tuberculosis, and specific aims were: (a) to isolate and characterize M. tuberculosis antigens, (b) to isolate and characterize Triton soluble protein (TSP), (c) to develop Mycobacterium-E. coli shuttle expression vectors, (d) to produce and characterize recombinant proteins of M. tuberculosis in E. coli and BCG, (e) to clone and express genes involved in the biosynthesis of rhamnose, a major component of cell wall, and (f) to establish an experimental animal model of tuberculosis for evaluation of diagnostic tests and vaccine(s). During the first stage of the study, several native M. tuberculosis antigens including culture filtrate (CF), lipoarabinomannan (LAM), diacyltrehalose (DAT), TSP, and PPD were examined for use in the serodiagnosis of truberculosis. In overall, the sensitivity of serological test ranged from 60% to 85%, and its specificity from 88% to 93%. In addition, recombinant protein antigens such as r16 kDa, r19 kDa, rAg85C, and r38 kDa were also evaluated for their seroreactivity. The results were similar to those obtained by using the native antigens above and ranged from 16% to 87% in sensistivity and 73% to 100% in specificity. Among the native and recombinant antigens, DAT and r38 kDa antigens look promising for use in the serodiagnosis of tuberculosis. Antigen detection in sputum samples was also evaluated, and the results were compared with those by sputum smear examination. For the diagnosis of active tuberculosis, antigen detection by a sandwich ELISA was far better than the microscopic smear examination, and further evaluation of such antigen detection would be, however, required before full implementation of the assay in the clincal laboratories. Triton X-100 solubilized protein (TSP) antigen showed a characteristic antigen profile and was extracted from M. tuberculosis without massive degradation of proteins. To characterize the nature of their composition, the TSP antigen was fractionated by Triton X-114 phase partitiongin. The TSP antigen contained a variety of lipids and glycoconjugates such as glycolipid and lipoarabinomannan, as well as diverse proteins including 30 kDa, glycoproteins, lipoproteins, and a few cytoplasmic proteins. Most proteins were partitioned into the aqueous phase during phase fractionation, whereas such irrelevant nonprotein molecules and lipoproteins were recovered in the detergent phase. The anfigens present in the TSP and TSP-aqueous fraction were considerable different from antigenic contents of the culture filtrate. The lymphoproliferative responses to the TSP-aqueous fraction in healthy tuberculin reactors were significantly higher than those to the purified protein derivative (PPD) and unfractionated TSP. In contrast, the antibody responses to TSP-aqueous fraction in tuberculosis parients showed weak reactivity. Thus, our study suggests that the TSP- aqueous fraction can be used as a T cell immunogen associated with protective immunity against tuberculosis in human. For expression of foreign antigens including M. tuberculosis antigens in BCG and in other mycobateria, a couple of Mycobacterium-E. coli shuttle vectors were prepared in this study: pYMC and pYMC-his. Using the vectors, 19 kDa and 38 kDa antigens of M. tuberculosis were expressed in M. smegmatis and BCG, respectively. The r38 kDa expressed in BCG was highly correlated with that in E. coli in detecting anti-38 kDa antibodies in sera from tuberculosis (r=0.92); howerver, the sensitivity of BCG-r38 kDa protein gave a slightly greater than that by E. coli-r38 kDa. Interestingly, mice infected with BCG over-expressing 38 kDa protein produced significant level of antibodies to the protein, but not in mice infected with BCG only. This result may imply the usefulness of over- expression of protective antigens of M. tuberculosis for better vaccine against tuberculosis. In addition, r16 kDa, r19 kDa, rAg85C, and r38 kDa proteins were prepared in E. coli in large quantity and evaluated for use in the serodiangosis of tuberculosis as described above. One of the future tasks will be to choose the best combination of these recombinant protein antigens in order to maximize the sensitivity and specificity of assay for the diagnosis of tuberculosis. We found 3.5 kb PvuII fragment that hybridized with the rfbB probe. We cloned the fragment and made restriction map with ApaI, BglII, EcoRV, XhoI, KpnI, HincII, and NaeI. We cloned the rfbA gene by PCR-cloning method with primers based on the rfbA sequence of Enteric bacteria and determined the nucleotide sequence. We determined the nucleotide sequence of the rfbB and rfbC genes (GeneBank U43540). From the sequence data we found a novel repeated sequence form the rfbC gene. M. tuberculosis H37Rv, H37Ra, Erdman and Canetti, M. bovis, and M. bovis BCG contained this repeated sequence. However, M. fortuitum, M. simiae, M. smegmatis, M. kansasii, M. avium, and M. phlei did not contain the repeated sequence. Several general bacteria also did not contain the repeated sequence, suggesting the specificity of this repeated sequence to M. tuberculosis complex strains. Therefore this repeated sequence could be a target for the PCR detection of M. tuberculosis. To make a rfbC deletion E. coli mutant, we cloned the rfbA, rfbC, and rfbX genes of E. coli K-12. We made a vector contruct containing kanamycin resistance gene flanked with FRT sequence that are recognized by FLP DNA excision enzyme. We selected a strain in which homologous recombination occurred by kanamycin. To rescue the kanamycin resistance gene, we transformed the selected strain with FLP expressing vector that contained resistance genes to both ampicillin and chlorampenicol. FLP expressing strains were selected by these 2 antibiotics and rfbC knock-out mutant was selected by kanamycin sensitivity and the rfbC deletion was confirmed by PCR. From this study, we found the RfbC protein is a suitable drug target for tuberculosis. Finally, a P-3 facility equipped for experimental infection of animals with virulent M. tuberculosis was recently built at Yonsei University College of Medicine. In the room, mice were initially infected with BCG for basis technical training and for establishment of infection schemes including aerosol infection, counting viable bacterial count in various internal organs such as lungs, liver, spleen and lymph nodes and measuring cell-mediated immune parameters including lymphocyte prolifieration and cytokine assays. Evaluation skills for tuberculin reactivity of M. tuberculosis antigens were also established using guinea pigs, and such system will be used for future evaluation of skin test reagents for the detection of M.tuberculosis infected persons. In addition, polyclonal antibodies to various M. tuberculosis antigens above were produced in goats and rabbits and used for comparative analysis of antigenicity for the antigens under study and for developing antigen detection assays. The results obtained in this study will be, therefore, starting points for the second stage of the study beginning 1998 and for eventual goal for developing new diagnostic tests and vaccines for tuberculosis.
