목표 : Naringin, Naringenin, Hesperidin, Hesperetin 등 천연물 혹은 천연물 유래 bioflavonoid를 이용하여 동맥경화증 예방, 치료 활성 신소재를 탐색, 선별하고 유도체합성, 약리활성 및 전임상 동물실험을 통하여 신의약품 개발에 적합한 후보물질을 도출하고, 토끼동맥경화모델에 naringin, hesperidin 등의 bioflavonoid를 투여하여 혈중 지질농도에 미치는 영향과 지방선(fatty streak)을 포함한 죽상경화반의 변화를 관찰하고, 이러한 변화를 일으키는 기전을 연구하며,
목표 : Naringin, Naringenin, Hesperidin, Hesperetin 등 천연물 혹은 천연물 유래 bioflavonoid를 이용하여 동맥경화증 예방, 치료 활성 신소재를 탐색, 선별하고 유도체합성, 약리활성 및 전임상 동물실험을 통하여 신의약품 개발에 적합한 후보물질을 도출하고, 토끼동맥경화모델에 naringin, hesperidin 등의 bioflavonoid를 투여하여 혈중 지질농도에 미치는 영향과 지방선(fatty streak)을 포함한 죽상경화반의 변화를 관찰하고, 이러한 변화를 일으키는 기전을 연구하며, 동맥성형술 후 재협착에 대한 naringin의 효과와 기전을 연구하는데 그 목표가 있다. 연구방법 : 1) 생물전환공정에 의한 유도체 창출 : Biotransformation은 미생물이나 효소 혹은 식물체 내의 반응 기구를 생촉매로 이용하여 약리활성 물질의 구조를 다양화하는 기술이며, 입체이성질체와 같이 접근이 어려운 부위에서 특이적 반응 산물을 도출할 수 있다(Holland, 1992). 본 연구과제에서는 미생물 혹은 효소를 생촉매로하여 귤피 추출물인 Hesperidin과 Naringin의 aglycon 부분이나 당골격을 생물전환/치환시켜 다양한 flavone 유도체 및 구조유사체를 생합성한다 2) 유기합성에 의한 유도체 창출방법 : 본 실험에서는 hesperetin과 naringenin의 OH 그룹에 적당한 치환기를 선택적 또는 무작위적으로 도입하여 아래와 같은 ester, ether, cabonate, carbamate 계열의 유도체를 합성한다. 혼합물의 경우에는 재결정, chromatography 등의 효과적인 방법으로 분리하여 각 유도체들의 순수 활성을 검정한다. 치환 그룹과 위치의 변화에 따른 활성의 변화 경향을 파악하여 구조와 활성의 상관관계를 확립하고 유도체 합성의 방향을 설정한다. 3) In vivo 활성 검정 : 0.25% 또는 1% 고콜레스테롤 사료를 6-8주간 투여한 토끼 동맥경화모델에서 naringin, hesperidin, 합성유도체 등을 투여 후 혈액화학 분석, 대동맥 조직염색을 통한 지방선등 죽상경화반의 퇴축 정도 분석, 각 장기의 조직학적 관찰, 간조직 염색을 통한 간독성 유무 및 형태계측, hepatic microsomal enzyme의 활성 분석 등을 HMG-CoA reductase 억제제인 Lovastatin(1 mg/kg/d) 투여군, 비치료군과 비교하여 고지혈증 및 동맥경화증의 예방, 치료 활성을 검정하였으며, 이 과정에서의 세포고사는 TUNEL(Terminal deoxyuridyl transferase nick end labelling)방법으로, 세포증식은 PCNA(proliferating cellular nuclear antigen)에 대한 면역조직화학 염색법으로 연구하였고, 대식세포, von Willebrand factor, 부착분자에 대한 면역조직학적 염색을 하였다. 4) In vitro 활성 검정 : Naringenin의 합성 유도체의 항산화능을 측정하기 위한 LDL-Antioxidant의 활성 측정은 Cu2+-mediated LDL-oxidation을 유발시키고, 이때 생성된 불포화지방산의 산화산물인 dialdehyde를 TBA(thiobarbituric acid)법으로 측정하였다. 흰쥐 대동맥 평활근세포를 배양하여 naringin을 각각 0, 10, 100 μM의 농도로 24 시간 전처리 후, 산화 저비중지단백의 주 작용성분인 lysophosphatidylcholine(LysoPC)을 0, 10, 20, 100 μM의 농도별로 투여, 그 효과를 관찰하였다. 염증관련 유전자의 상위조절인자인 NF-kB의 발현은 EMSA로 연구하였다. LysoPC를 투여한 지 6, 24, 48 시간에 3H-thymidine incorporation assay를 하였고, 24, 48, 72 시간후에는 MTT assay를 시행하여 세포증식정도를 알아보았다. naringin 치료군과 비치료군에서 NF-kB 발현정도, 혈관평활근세포6714, Trametes versicolor KCTC6365, Phanerochaete chrysporium KCTC6147, Phanerochaete chrysporium KCTC6728의 균주 배양액에서 단백질 농도가 높은 분획을 모아서 동결 건조하여 조효소를 확보하였다. 2) Naringenin 유도체 합성 및 약효 검정 : Naringenin의 in vivo 효능 증대와 물성 개선을 위해서 hydroxyl 그룹에 여러 가지 치환기를 도입하여 naringenin 유도체를 만들었으며, naringenin의 hydroxy 그룹에 ester, ether 형 화합물을 중심으로 합성하였다. LDL 항산화력이 증가된 화합물 중 NG5006을 선별하고, 대량으로 합성하여 in vivo 동물실험을 통해 활성을 검증하였다. naringenin 및 NG5006을 투여한 실험군에서 동맥벽에 생성되는 지방선조의 분포비율이 양쪽 모두 대조군에 비하여 유의성 있게 감소되는 결과를 얻을 수 있었으며, 특히 고지혈증 치료제로 널리 이용되고 있는 lovastatin보다 더욱 효과적으로 동맥벽의 지방선을 억제하는 효과를 보였다. 아울러 간에 대한 조직학적 관찰에서도 간소엽내 지방변성세포들이 낮은 비율로 나타나 간에 대한 보호효과가 있는 것으로 나타났다. 4) Biofalvonoid 투여시 고콜레스테롤 식이 동물모델에서의 동맥경화 예방, 치료효과 검증 : 토끼에 lovastatin, naringin 및 hesperidin이 함유된 고콜레스테롤 식이를 시행한 후 고지혈증 및 동맥경화증의 예방 및 치료 효과를 검증한 결과, 감귤류 유래의 naringin 및 hesperidin이 lovastatin보다는 potency가 약하였지만 확실히 동맥벽의 지방부착을 억제하여 동맥경화를 예방하는 효과가 있음을 확인할 수 있었다. 혈액을 분석한 결과, 고콜레스테롤 식이로 인해 정상시보다 10-15 배로 과도하게 혈중지질의 농도가 높아진 상태에서 커다란 지질 강하 효과를 나타내지 못한 것으로 판단된다. 또한 간세포의 기능손상과 관련된 GOT와 GPT 등은 lovastatin 식이군에서 대조군보다 현저히 높고, 간세포변성도도 높아 lovastatin이 간에 독성이 심함을 나타낸 반면, naringin 및 hesperidin 식이군에서는 대조군과 비슷하거나 낮은 값을 보여 간보호 작용이 있다는 것을 알 수 있었다. 토끼의 폐, 심근, 신장 및 근육 등의 조직에 대한 검사에서도 특이한 병변을 관찰할 수 없어 근육이나 타장기에 대한 독성이 없음을 알 수 있었다. 5) Naringin의 항동맥경화 작용기작 연구 : i) In vivo : 토끼 동맥경화 모델에서 naringin 투여에 의한 혈중 지질강하는, 비치료군에 비해 유의한 정도는 아니나(p>0.05), Lovastatin 투여군과 비슷한 정도의 효과를 보이면서도 간독성은 없었다. 또한 naringin이 대동맥 지방선의 크기를 유의하게 감소시키는 항동맥경화 효과를 보였고(p<0.05). 이는 지질을 함유한 대식세포의 내막하 침투가 감소한 것에 의한 효과임을 확인하였다(p<0.05). 지방선내의 세포증식이나 세포고사의 정도는 각 군간에 차이가 없었으나(p>0.05), 고지혈증에 의한 ICAM-1의 발현은 지방선이 많은 대조군에 비해, naringin 투여군에서 유의하게 감소하였다. ii) In vitro : 흰쥐 혈관평활근 세포배양에서 NF-kB의 발현은 LysoPC에 의하여 촉진되었고 naringin에 의해 LysoPC에 의한 혈관 평활근세포의 증식을 억제하였으며(p=0.001), naringin 100 μM 투여시 비치료군에 비해 34%의 가장 강한 억제효과를 보였다. 결론 및 기대효과 : Citrus bioflvanoids는 고지혈증 치료외의 독특한 항동맥경화 작용과 간보호작용이 있으며, 그 기전은 부착분자의 발현에 따른 대식세포 침윤을 억제하는 것으로 죽상경화의 발생과 진행을 억제한다. 항산화 작용을 통해 염증유발에 따른 혈관평활근 세포의 과증식을 억제함으로써 장차 관동맥 성형술 후의 혈관재협착 방지에 유익한 효과를 보일 것으로 예측되어 그 임상적인 이용이 기대된다.

Purpose: Some bioflavonoid compounds are associated with the prevention of chronic diseases such as cancer and hyperlipidemia. Amoung the naturally accurring flavonoids, some are pharmacologically evaluated as a potential anticancer agent and antioxidants. The present study was conducted to examine the effects of various bioflavonoids on cholesterol metabolism in rabbits fed a high-cholesterol diet. Lysophosphatidylcholine(LysoPC), an atherogenic lysophospholipid contained in oxidized low density lipoprotein, impairs endothelial nitric oxide release, upregulate adhesion molecules and act as a monocyte chemoattractant, but its effect on vascular smooth muscle cells(VSMCs) is not known. Therefore, we examined the effects of naringin, hesperidin, naringenin and naringenin derivative(NG5006) on 1) blood lipid profile, 2) regression of fatty streak of aorta, 3) liver toxicity, 4) cellular mechanism of antiatherogenic effect of naringin in diet-induced hypercholesterolemic rabbit and 5) effect on vascular smooth muscle cell (VSMC). Methods : New Zealand White Rabbits (2.0-2.5 kg) were fed on 0.25% or 1.0% cholesterol-containing diet for 8-weeks and then sacrificed. Blood samples were collected for measurement of total cholesterol, HDL-cholesterol, triglyceride, serum GOT and GPT. Aortas and livers were harvested for evaluation of fatty streak and pathologic examination. VSCMs (passage 4-7) were prepared from aorta of male Sprague-Dawley rat. Near-confluent cells were preincubated with 0, 10, 100M naringin for 24 hours and then, incubated with 0, 10, 20, 100M LysoPC. The cellular growth was measured by H3-thymidine incorporation assay at 6, 24, 48 hours and by MTT assay at 24, 48, 72 hours after the incubation with LysoPC. Results : Naringin, hesperidin, naringenin and NG5006 exhibit the cholesterol-lowering action in high cholesterol diet fed rabbits. The mode of action is a enzyme inhibitory activity against hepatic ACAT. Total cholesterol and triglyceride levels tended to decrease by treatment with lovastatin and naringin, but these decreases were not statistically significant. The regression effect of fatty streak of aorta is simillar to lovastation that is a well-known HMG-CoA reductase inhibitor. The liver toxicities of naringin, hesperidin, naringenin and NG5006 treated animal groups were lower than those of control group and lovastatin treated group. Naringin was more effective in inhibition of fat infiltration into liver and reduced serum GPT level than lovastatin which showed hepatotoxicity as increase of serum GPT level (p<0.05). Microscopic examination revealed that foam cell infiltration into intima was significantly reduced by naringin and lovastatin. And through the pathologic examinations, foam cells were originated from the migration of activated macrophages from blood. The results of PCNA index and TUNEL staining have no significant differences between control, naringin, and lovastatin groups. LysoPC promoted the growth of VSMCs in a dose-dependent manner, and naringin inhibited the proliferative effect of LysoPC on VSMCs. Conclusion : Citrus bioflavonoids have strong antiatherogenic action which may not be associated with its very mild lipid-lowering action. In contrast to lovastatin, citrus bioflvaonoids have hepatoprotective effect. LysoPC promotes the growth of VSMCs and the progression of atherosclerosis, but naringin inhibits the atherogenic process through the supression of the proliferative effect of LysoPC on VSMCs. Citrus bioflvaonoids can be utilized for development of the functional foods or health foods. By applying these bioflavonoids to human clinical test, they can be reevaluated for human use.
