뇌동맥류의 파열로 인한 지주막하 출혈은 높은 사망률과 중병유병률을 나타내므로 뇌동맥류의 진단, 치료 및 파열의 예측은 현대 질병의 방어 및 건강 증진에 중요한 역할을 한다. 본 연구는 뇌혈관 내부의 혈류의 유동이 뇌동맥류의 형성 및 파열에 미치는 영향을 연구하여 뇌동맥류의 진단 및 치료에 응용하고자 한다. 이를 위하여 환자의 뇌혈과 영상자료를 이용하여 동맥류의 발생위치, 형상, 파열 부위를 통계학적으로 분석하고 영상자료를 이용하여 환자에게 발생한 뇌동맥류 모델을 제작하여 혈관과 동맥류 내부의 혈류 유동 특성을 밝히고 동맥류의 형성 및 파열 부위와 비교 분석하여 동맥류의 형성 및 파열에 미치는 혈류역학적 원인을 규명함으로 동맥류의 치료와 진단에 기여하고자 한다. 국내환자의 뇌동맥류가 자주 발생하는 뇌혈관의 위치를 통계적으로 분석하기 위해 전국의 34개 대학병원을 대상으로 지난 3년간 뇌동맥류로 인하여 수술 또는 혈관 내 치료를 실시한 총 8,990명의 환자를 대상으로 발생 부위별 통계를 얻었다. 동맥류 발생 부위별 통계는 전교통동맥류 ; 2,674(31.7%) 중대뇌동맥 분지동맥류; 1,867(221.1%) 후교통동맥류; 1,568(18.6%) 다발성동맥류; 762(9.0%) 내뎡동맥동맥류; 669(8.0%) 후방순환동맥류; 486(5.8%) 전대뇌동맥동맥류; 313(3.7%) 중대노동맥동맥류; 99(1.2%) 이었으며 이는 외국의 발생 빈도와 거의 유사하였다. 뇌동맥류의 모양, 크기, 위치와 동맥류의 파열 빈도의 상관관곌,f 알기 위해 뇌동맥류로 진단 받은 환자를 대상으로 뇌혈관조영술(Digitial subtraction angiography)을 이용하여 뇌동맥류의 크기(경부 및 두부의 내경, 장축 길이)를 측정 및 분석하고 모양과 위치에 따른 분류를 하였다. 파열된 동맥류는 전교통동맥류, 중대뇌동맥류, 내경동맥류 순이었으며 파열된 동맥류 모양은 타원형, 원형, 분엽형, 벌집형 순이었으나 구형은 파열된 동맥류가 비 파열된 동맥류에 비해 상대적으로 적었다. 2개이상의 분엽을 갖는 동맥류는 파열의 위험이 크게 나타났으며 파열된 동맥류는 파열안된 동맥류에 비해 장축의 길이가 길었고 경부/기저부 비가 작았으며 높이/기저부 비가 컸다. 따라서 본 연구의 결과는 혈과 조영술로 동맥류의 형상을 진단하여 상대적 파열의 위험도를 예측할 수 있는 중요한 자료가 될 수 있다. 동맥류 발생에 미치는 혈류역학적 요인을 연구하기 위해 내경동맥, 기저동맥, 전교통동맥 동맥류 모델을 제작하고 입자영상속도계를 이용하여 유동장을 측정하였다. 본 연구 결과는 혈류의 혈관벽 부딪힘(impingement)이 동맥류 발생의 혈류역학적 원인이라는 기존 연구의 결과를 확인하고 있으며 부차적 유동에 의한 혈관벽의 전단옹력이 동맥류 발생의 원인이 될 수 있다는 새로운 가설을 제안하고 있다. 혈류유동이 동맥류 파열에 미치는 영향을 고찰하기 위하여 내경동맥, 기저동맥 및 전교통동맥 동맥류 내부의 유동장이 측정되었다. 본 연구 결과 동맥류 치료 후의 유동을 측정하기 위해, 동맥류 목이 50% 또는 75% 폐색된 모델을 제작하여 맥동류에서 혈관 및 동맥류 내부의 유동을 측정하였다. 동맥류 파열에 영향을 미치는 혈류역학적 인자를 규명하기 어려웠으므로, 클립 시술 후 동맥류의 파열을 예측할 수 없었다. 그러나 클립의 부분폐색의 정도와 위치, 모혈관의 형상에 따라 동맥류 내부의 혈류 정체 특성이 바뀜을 알아냈으며, 이는 동맥류의 부분 폐색 위치의 선정에 중요한 자료를 제공할 수 있으므로 이에 관한 연구가 향 후 수행되어야 할 것이다.

Since subarachnoid hemorrhage (SAH) caused by aneurysm rupture causes high mortality and morbidity rate the diagnosis treatment and the prediction of rupture of cerebral aneurysm play an important role in modern society health care. We would like to study the effects of hemodynamics on the formation and rupture of cerebral aneurysm. The location where aneurysm formed and the shapes of aneurysm are statistically analyzed for ruptured and unruptured aneurysms. Aneurysm models were manufactured based on the image data of patients and blood flow fields were measured. The flow characteristics of the sites where aneurysm formed and ruptured were analyzed in order to clarify the hemodynamic factors which after the formation and rupture of aneurysm. In order to analyze the location of the cerebral arteries where aneurysms are frequently formed we classified the aneurysm formed arteries according to the locations for the 8,990 patients who have been hospitalized for last 3 years at 34 university hospitals in Korea. The statistics showed that the number of occurrence is AcomA; 2,674(31.7%), MCA bifurcation ; 1,867(18.6%) PcomA; 1,568(18.6%) multiple aneurysm; 762(9.0%), ICA; 669(8%) PcirA; 486(5.8%) ACA; 313(3.7%) MCA; 99(1.2%) which are similar to the statistics of USA. The location shape and geometry of aneurysms were analyzed in order to observe the effects of them on aneurysm rupture using the patients’ digital subtraction angiographys. The frequent locations of ruptured using the patients’ digital subtraction angiography. The frequent locations of ruptured aneurysms were observed in the order of AcomA, MCA and ICA. The frequent shape of ruptured aneurysm was the order of ellipsoidal circular lobular and beehive. The ruptured aneurysm had multiple lobes longer axis length smaller neck/dome ratio and higher height dome ratio comparing to unruptured aneurysm. Therefore this results may provide important information of predicting rupture possibility from the geometry of aneurysm. In order to clarify the hemodynamic factors influencing the formation of aneurysm velocity fields were measured for the ICA, BA and AcomA model. Our results confirmed the previous workers’hypothesis that blood jet impingement is responsible for the aneurysm formation and suggested a new hypothesis that wall shear stress caused by the secondary flow may affect the aneurysm formation. In order to find the hemodynamic factors causing aneurysm rupture. the velocity fields inside the aneurysms were also measured. The results showed that hemodynamics did not cause aneurysm rupture directly, In order to study the hemodynamic changes caused by surgical clipping of aneurysm neck flow fields were measure for the 50% and 75% partially clipped models. The results showed the clip sites and the amount of neck closing significantly changed the inflow characteristics of aneurysm sac. Further study will be performed to find out the effects of geometry of mother vessel and treatment sites on the aneurysmal flow characteristics and aneurysm treatment prognosis.
