치주 질환의 발생 및 진행을 미리 예견할 수 있는 위험지표(risk indicator)로써 많은 요인들이 연구되고 있다. 그 중 미생물학적 요인에 대한 연구는 치주 질환과 연관된 세균의 동정, 질환의 심도에 따른 세균의 분포 등 여러 분야에서 이루어지고 있다. 특히 Porphyromonas gingivalis는 adult periodontitis(AP)와 rapidly progressive periodontitis(RPP) 같은 치주질환과 밀접하게 연관되어 있다고 보고되어 왔다.
본 연구에서는 P. gingivalis의 검출
치주 질환의 발생 및 진행을 미리 예견할 수 있는 위험지표(risk indicator)로써 많은 요인들이 연구되고 있다. 그 중 미생물학적 요인에 대한 연구는 치주 질환과 연관된 세균의 동정, 질환의 심도에 따른 세균의 분포 등 여러 분야에서 이루어지고 있다. 특히 Porphyromonas gingivalis는 adult periodontitis(AP)와 rapidly progressive periodontitis(RPP) 같은 치주질환과 밀접하게 연관되어 있다고 보고되어 왔다.
본 연구에서는 P. gingivalis의 검출 및 역학연구를 위하여, P. gingiualis 각 세균 균종에 특이하게 존재하는 IS element인 IS1126을 표적으로 하여, P. gingivalis의 검출 및 균주간 차이 식별을 위한 PCR 분석 방법을 개발하고, 이를 이용하여 치주 질환과 관련된 P. gingivalis의 역학적 현상을 규명해 내고자 하였다. 이를 위하여 IS1126의 유전자 염기서열(본 연구책임자의 이전 연구결과)을 바탕으로 하여 이 IS element의 internal part를 증폭할 수 있는 inward primer pair(P1/P2)와, 또한 이에 상보적인 것으로서 IS element사이의 random chromosomal sequence를 증폭하기 위한 outward primer pair(PIIRC/P12RC)를 고안하였다.
먼저 P. gingivalis의 여섯 개의 reference strain(381, A7Al, W50, 2561, 9-l4K-1, HG564)을 대상으로 각 strain의 염색체내 IS1126의 존재 및 상이한 분포양상의 판단(fingerprinting)이 이러한 IS element-specific primer를 이용하는 PCR 방법으로 가능한지를 확인하였다. Inward primer pair를 이용한 PCR에서는 모든 strain에서 기대하였던 대로 하나의 unique한 PCR band가 관찰되었으며, outward primer pair의 경우에는 각 strain마다 고유의 band pattern이 관찰되었다. 이것은 IS element-specific primer를 이용하는 PCR 방법으로 P. gingivalis의 존재를 확인한 수 있으며, 또한 P. gingivalis의 strain 간의 식별이 가능함을 보여주는 결과이다.
이 결과를 바탕으로 하여 치주질환자 및 정상인에서, 치은열구액 내 P. gingivalis의 검출 및 fingerprinting을 시도하였다. Inward primer pair를 이용한 PCR 결과 대다수의 치주질환자에서 IS1126-specific한 unique band가 관찰되었다. 정상인에서는 이 PCR band가 거의 관찰되지 않았다. Inward primer pair의 PCR에서 하나의 unique한 band가 관찰되었을 때는, reference P. gingivalis strain에서의 경우와 마찬가지로 outward primer pair를 이용한 PCR에서는 환자마다 서로 다른 band pattern을 보였다.
이에 따라 이 방법을 이용하여 P. gingivalis의 역학적 현상을 규명하고자 하였다. 먼저 가족 구성원 간의 P. gingivalis 전파 양상을 파악하기위해 우리는 치주질환자 가족을 대상으로, 부모와 자식간, 또는 배우자간에 P. gingivalis의 균주의 차이를 관찰하고자 하였다. 치주질환자들 중 이 연구에 호응하는 환자 가족들의 치은열구액을 채취하여 P. gingivalis 존재 유무를 확인하고, 개별 P. gingivalis 균주 간의 차이를 분석하였다.
P1/P2 primer를 이용한 PCR 분석으로 치주질환자 및 그 가족의 치은열구액 샘플 내 P. gingivalis의 존재를 확인한 후, PIIRC/P12RC primer를 이용하는 PCR 분석을 시행하면, 부모와 자식간의 경우에는 PCR product가 모두 같은 band pattern을 보이거나 같은 size의 band를 다수 이용한 PCR 분석에서 16명 중 10명에서 완전히 서로 다른 band pattern이 관찰되었다.
본 연구에서는 p. gingivalis의 염색체내에 IS element인 IS1126이 다수로 존재하는 사실을 이용하여 PCR method를 기반으로 치주 질환 원인균인 P. gingivalis의 동정, 정량 및 균주간의 식별 방법을 확립하였다. 또한 본 연구의 결과로 P. gingivalis에 의한 구강감염의 연구에 있어 신속하고도 정확한 역학적 도구를 얻었으며, 향후 치주 질환과 관련된 연구에 많은 도움이 될 것이라고 믿는 바이다.

The incidence of Porphyromonas gingivalis is closely related with the occurrence of periodontal diseases, especially adult periodontitis and rapidly progressive periodontits. Until now, two different insertion sequence (IS) elements of P. gingivalis, IS1126 and PGIS2 have been positively identified, while another insertion sequence resembling element, IS195, has been reported. IS elements are 0.7-7.1 kbp sequences that only have the genetic information required for transposition in the prokaryotic chromosomal DNA, bacteriophage and plasmid. They are known to be species-specific and to cause specific genetic effects such as mutation and IS-mediated rearrangement of chromosomal DNA. In this study, we assessed the usefulness of a polymerase chain reaction (PCR)-based method for the identification and fingerprinting of P. gingivalis, using IS1126-specific primer pairs. We made two PCR primer pairs; an 'inward' pair for the amplification of the internal part of the IS element, and an 'outward' pair whose nucleotide sequences were complementary to the first one in order to amplify the random chromosomal sequences that are limited by any two of the IS elements in the bacterial chromosome. All six strains of P. gingivalis possessed by our laboratory produced a unique PCR band of the expected size with the 'inward' primer pair, but no strains of Prevotella intermedia (3 strains) or Actinobacillus actinomycetemcomitans (3 strains) did. With the 'outward' pair, each P. gingivalis strain showed its own unique pattern of PCR bands. Using these primer pairs, we could not only determine the presence of P. gingivalis in the gingival crevicular fluids (GCFs) of the periodontal disease patients but could also ascertain the strain specificity of the putativ P. gingivalis strain. We could observe strain-specific PCR band patterns with the 'outward' primer pair only when a unique band showed up in PCR with the 'inward' primer pair. This confirms the specificity of the PCR using this double primer pairs method.
We applied this method to the epidemiological study regarding the transmission of P. gingivalis among the family members, i.e., between parents and their offsprings and between the spouses. We sampled gingival crevicular fluid, extract bacterial DNA from it, and analyzed by PCR with the 'inward' and 'outward' primers.
PCR using P1/P2 ('inward') primers could detect the presence of P. gingivalis in almost all of GCF samples from periodontal disease patients, but in few samples from healthy subjects. When PCR using P1/P2 primers gave positive result in parents and their offsprings, PCR products using PI1RC/PI2RC ('outward') primers always gave identical band patterns or show many idenical bands. In contrast, between periodontal patients and their spouses, PCR products using PI1RC/PI2RC primers gave different patterns in 10 out of 16 couples.
The result of this work showed the usefulness of the method we established to detect and identify P. gingivalis by targeting P. gingivalis-specific IS element, IS1126, in PCR using IS1126-specific 'inward' and 'outward' primers. It also confirmed that the method could be very useful in epidemiological study of P. gingivalis-mediated periodontal disease.
