1. Study objective This study is aimed to analyze drug distribution system, and perform a comparison analysis of competitiveness between wholesales and pharmaceutical companies. Also it identified the current trend of pharmaceutical e-commerce by categorizing them by sales revenue and major customers. 2. Methods As to wholesalers, stores subject to FSS disclosure requirements and members of the Medicinal Filing Compliance Association were selected first from the KGSP designated wholesalers` list, and then remaining part was selected through 10% randomized sampling. Also surveyed were pharmaceutical companies whose sum of production and import amounts exceeds KRW 10 billion. Total 229 wholesalers and 110 pharmaceutical companies were surveyed with questionnaire developed on our own, and the response rate was 50.7 percent for wholesalers and 57.3% for pharmaceutical companies. 3. Results Analysis on competitiveness per distributive function showed that wholesaling is competitive in terms of logistics efficiency, speed and nationwide access, while direct sales are more competitive in sales power and tracking for return and recall. With respect to flexibility of commercial transaction and contribution to distribution transparency, wholesalers answered that wholesaling was better than direct sales, however pharmaceutical companies answered the contrary. Direct sales turned out to be better for new drug competitiveness. In terms of customer characteristics, wholesalers responded that wholesaling was better than direct sales for pharmacies (80.4%), hospitals with less than 100 beds(64.8%) and clinics(60.2%) in descending order. In contrast, pharmaceutical companies said that direct sales were more competitive than wholesaling for clinics(70.5%), pharmacies(63.3%) and hospitals with less than 100 beds(49.2%) As to the status of wholesalers engaging in e-commerce, 6.0% were brokering businesses between pharmacy and wholesalers; 4.3% were those between hospitals and wholesalers; and 12.1% were wholesalers using their own websites. In regard to pharmaceutical companies, 1.6% was brokering companies between hospitals and wholesalers and 11.7% were companies using their own websites to sell drugs. The study finds out that wholesalers participating in the pharmaceutical e-commerce have large sales revenue, storage capacity and warehouses, short delivery time and small pharmaceutical product on stock, and they are national wholesalers and think badly of secondary wholesalers. In terms of boosting pharmaceutical distribution transparency, E-commerce turns out to contribute somewhat to over-the-counter drugs, but not much to prescription drugs. 4. Conclusion According to the survey, wholesaling is better in terms of logistics efficiency, speed and nationwide access while direct sales are better for new drugs, sales power and tracking, and respondents disagreed on remaining areas. Nationwide competitive wholesalers with considerable sales revenue have high level of participation rate in e-commerce, and pharmaceutical e-commerce contributed a great deal to enhancing distribution transparency of OTC drugs.
