1. Aim and purpose As a single disorder, stroke is the most important cause of death in Korea. With the increasing number of aged people in Korea, stroke will be even more important problem in our society. Thus, clinical studies on the epidemiology, diagnosis, reatment as well as base researches
1. Aim and purpose As a single disorder, stroke is the most important cause of death in Korea. With the increasing number of aged people in Korea, stroke will be even more important problem in our society. Thus, clinical studies on the epidemiology, diagnosis, reatment as well as base researches are definitively needed. For more effective research, investigators of diverse expertise should collaborate closely. We studied a variety of subjects in stroke ranging from acute treatment to basic research. 2. The spectra of our study 1) Protocol setup for the acute thrombolysis with the use of advances MR imaging and analysis of the factors influencing the clinical outcome of the patient. 2) Assessment of strokes in Koreans with the use of MRI and MRA: clinical characteristics, brain and vascular imaging findings, and clinical-imaging correlation 3) Management of post-stroke depression, emotional incontinence and fatigue. 4) Assessment of factors affecting the quality of life of stroke patients and caregivers 5) Assessment of relationship between the nutritional status and recurrence of stroke 6) Assessment of factors affecting the progression of intracranial atherosclerosis 7) Assessment of factors relating to the recurrent intracerebral hemorrhages 8) Assessment of neuron-glia interaction by observation of the patterns of appearance and disappearance of these cells. Expression of antioxicant, ion channel, GABA shunt enzyme in astroctye/microglia was also analyzed. 9) Assessment of neuroprotective effects of NSAID, valproic acid and ibudalist in the MCA occlusion model of the rat 3. Results 1) We set up a protocol for thrombolysis with the use of advances MR imaging and identified factors affecting the clinical outcome of the patients who receive thrombolytic therapy 2) With the use of MRI, we characterized clinical syndromes, related vascular pathogenesis as well as the general vascular characteristics in Korean stroke patients 3) We identified factors affecting the outcome of stent/angioplasty 4) We found that SSRI is effective in the management of post-stroke depression, emotional incontinence and anger-proneness. However, SSRI was not effective in treating fatigue suggesting that post-stroke fatigue is not simply related to the serotonergic system 5) We identified factors affecting the quality of life of stroke patients. We newly found that central post-stroke pain is one of the factors related to the quality of life in stroke patients 6) We found that factors affecting the caregiver burden in Korea included patients` physical status, depression, and caregiver`s emotional status. Being a daughter in law was also a factor related to the high burden, an observation suggesting unique cultural characteristics of Korea 7) We found that the presence of microbleeds in the initial GRE was a significant factor related to the recurrent intracerebral hemorrhages 8) We identified the time sequence of neuron-glia interaction in ischemic brain. Expression of various antioxicants, ion channel, GABA shunt enzyme in astroctye/microglia was also assessed. 9) We identified neuroprotective effects of NSAID, valproic acid and ibudalist in the MCA occlusion model of the rat 4. Application of the results 1) Protocol and guideline setup for thrombolysis, stent/angioplasty and embolectomy in the treatment of acute stroke patients 2) Identification of stroke subtypes and guideline setup for the diagnosis of stroke in Korean patients 3) Guideline for the management of post-stroke depression and emotional incontinence 4) Our results can be used as basic information in the policy to improve the quality of life of stroke patients and caregivers 5) Prospective study using stalin in the management of intracranial atherosclerosis 6) Development of neuroprotective strategy based on the results from neuron-glia interaction 7) Further understanding of the mechanism of neuroprotection of NSAID and ibudilast and future application of these agents in clinical trials
