연구목표 본 연구에서 자궁경부종양의 발생과정에 따른 신생혈관형성 과정을 알아보고자 하였다. 이러한 목적을 위해 첫째, 자궁경부종양의 발생과정에서 신생혈관형성을 영상분석을 통해 정량화하고 이를 세포증식 및 고사와 비교하였으며 둘째, 신생혈관형성 과정에서 VEGF와 p53 유전자의 역할을 구명하였다. 연구내용 정상 자궁경부조직 12예, 자궁경부 상피내종양(cervical intraepithelial neoplasia, CIN) I 등급 33예, II 등급 21예, III 등급 30예, 미세침윤성 편평세포암종 34예, 침윤성 편평세포암종 52예(FIGO 임상병기 Ib-IIb)을 대상으로 하였다. 신생혈관형성의 정도를 CD34를 이용한 면역조직화학적 방법을 영상분석으로 정량화하였다. 고사를 TUNEL(terminal deoxynucleotidyl transferase (T dT) m ediated dUTP nick-end labeling)법으로 확인한 후 측정하였고, 세포증식은 MIB1을 이용한 면역조직화학적 방법으로 측정하였다. VEGF와 p53에 대한 면역조직화학적 염색을 실시하였으며 p53 유전자 돌연변이는 최근에 개발된 laser capture microdissection 법을 이용하여 선택적으로 추출한 종양세포의 DNA를 이용하여 및 직접염기서열분석으로 확인하였다. 연구성과 신생혈관밀도, Ki-67 지수, 고사지수 모두는 자궁경부종양이 진행(정상 자궁경부조직 → CIN I/II → CIN III → 편평세포암종)함에 따라 유의하게 증가하였고. 세 가지 지수 상호 간에 유의한 상관관계를 보였다. 신생혈관밀도는 재발하거나 사망한 편평 세포암종에서 질병 없이 생존한 경우보다 통계적으로 유의하게 높았다. VEGF 발현은 자궁경부종양이 진행함에 따라 의의 있게 증가하였다. VEGF 발현이 증가할수록 신생혈관밀도가 의의 있게 증가하였다. VEGF 발현은 p53 단백발현과 유의한 상관관계를 보였으며, VEGF와 p53에 대한 이중염색의 경우 동일한 종양세포에서 VEGF와 p53에 대한 양성반응이 관찰되었다. 본 연구결과로 자궁경부종양의 진행과정 중신생혈관형성이 중요하고 이들이 세포증식 및 고사와 밀접한 관계가 있음을 알 수 있었다. 자궁경부종양의 신생혈관형성에 있어서 VEGF가 중요하며, VEGF 발현 조절에 p53 유전자가 관여함이 확인되었다. 이러한 성적을 기초로 자궁경부종양에 항혈관형성 치료방법을 도입한다면 자궁경부종양의 진행을 억제할 수 있을 것으로 생각되었다. 낮은 병기의 자궁경부 편평세포암종 환자에서 영상분석을 통해 정량화 된신생혈관정도가 재발이나 사망한 환자에서 높음을 알 수 있어 외과적 치료 후 방사선 치료나 화학요법이 필요한 환자를 선택하는데 사용될 수 있을 것이다.

Purpose of Research The aim of this study was to establish the method of quantification of tumor angiogenesis by image analysis system and to investigate the relationship between angiogenesis, cell proliferation, and apoptosis in uterine cervical neoplasia. We also investigated the correlation between angiogenesis, VEGF expression, and p53 mutation. Contents of Research 182 biopsy specimens (12 as benign, 33 as cervical intraepitehlial neoplasia (CIN) I, 21 as CIN II, 30 as CIN III, 34 as microinvasive carcinoma, and 52 as invasive carcinoma) were included. The microvessels were immunohistochemically labeled with antibody to CD34. Computerized image analysis was used to quantify of angiogenesis. The apoptotic cells were visualized by a terminal deoxynucleotidyl transferase mediated dUTP nick-end labeling technique and proliferative cells by staining with Ki-67 antibodies. Immunohistochemical staining with a VEGF monoclonal antibody was performed. p53 status was determined by immunohistochemistry and direct sequencing of exons 5 through 8 of the p53 gene. Effectiveness of Research One-way analysis of the variance showed that the microvessel density(MVD), the Ki-67 labeling index, and the apoptotic index increased significantly with the progression of cervical neoplaisa from the normal epithelium, through CIN, to carcinoma. The all indices were significantly and positively associated with each other. The MVD in patients with recurrence or death was significantly higher than that in the disease-free patients. VEGF expression progressively increased along the continuum from normal epithelium to squamous cell carcinomas. A strong correlation was observed between VEGF expression and MVD. VEGF expression correlated statistically with p53 protein expression. Our results suggest that tumor progression in the cervical epithelium is accompanied by angiogenesis and an increase in both cell proliferation and apoptosis. VEGF expression is involved in the promotion of angiogenesis in cervical neoplasia and p53 is likely to be involved in regulation of VEGF expression. Angiogenesis may be a prognostic indicator in patients with squamous cell carcinoma of the cervix. This study may give some information about the anti-angiogenesis therapy and gene therapy of cervical neoplasia.
