본 연구의 궁극적인 목표는 최근 류마티스 관절염의 병인에 중요한 역할을 담당하는 것으로 알려진 신생혈관을 억제하는 기전을 규명함으로써 류마티스 관절염 치료에 적용할 수 있는 새로운 기술을 개발하는 데에 있다. 류마티스 관절염은 활막세포 비후, T 림프구 침윤, 신생혈관 증식이 조직학적인 특징이므로 류마티스 활막염을 이루는 3 가지 구성세포인 T 림프구-활막섬유아세포, 그리고 T 림프구-혈관내피세포와의 cross-talk를 중심으로 혈관신생에 미치는 영향을 조사하고자 한다. 특히 CD40L-CD40, Fas-FasLL system 과
본 연구의 궁극적인 목표는 최근 류마티스 관절염의 병인에 중요한 역할을 담당하는 것으로 알려진 신생혈관을 억제하는 기전을 규명함으로써 류마티스 관절염 치료에 적용할 수 있는 새로운 기술을 개발하는 데에 있다. 류마티스 관절염은 활막세포 비후, T 림프구 침윤, 신생혈관 증식이 조직학적인 특징이므로 류마티스 활막염을 이루는 3 가지 구성세포인 T 림프구-활막섬유아세포, 그리고 T 림프구-혈관내피세포와의 cross-talk를 중심으로 혈관신생에 미치는 영향을 조사하고자 한다. 특히 CD40L-CD40, Fas-FasLL system 과 관절염의 악화와 억제에 영향을 미치는 사이토카인을 위주로 활막세포에서 VEGF의 생성과 조절에 미치는 영향을 조사하며, 또한 내피세포의 증식 및 억제에 미치는 영향을 조사하고자 한다. 나아가서 VEGF가 혈관생성 및 염증에 기여하는 기전을 규명함으로서항 류마티스 효과를 갖는 생물학적 치료제를 개발하여 난치성 류마티스 질환에 응용하고자 한다.
[제 1 차년도] 1) 류마티스 관절염 환자에서 VEGF의 임상적 의미 규명, 2) T 림프구의 활성화에 따른 CD40L 와 FasL의 시기별 표현 분포 조사, 3) 림프구에서 FasL 및 CD40L의 표현에 영향을 미치는 인자 조사, 4) FasL가 활막 섬유아세포의 apoptosis에 미치는 영향 조사, 5) CD40L 와 FasL가 혈관내피세포의 증식과 apoptosis 에 미치는 영향을 조사, 6) 활막 섬유아 세포에서 VEGF 생성에 영향을 미치는 인자 조사
[제 2 차년도] 1) 세포표면 혹은 soluble FasL (sFasL)가 활막 섬유아세포에서의 VEGF 생산에 미치는 영향을 조사, 2) 사이토카인과 sFasL에 의한 VEGF 억제에 미치는 synergism 혹은 antagonism을 조사, 3) IL-4 에 의한 혈관 내피세포의 증식, 이동(chemotaxis), tube formation 에 미치는 영향 조사, 4) 혈관내피세포의 증식, 이동, tube 형성에 미치는 Fas ligand(CD178)와 CD40 ligand(CD154)의 영향, 5) VEGF mRNA의 전사에 영향을 주는 transcription factor를 EMSA를 이용하여 분석, 6) sFasL의 cDNA를 발현 벡터에 클로닝하여 E. Coli에서 sFasL를 발현 유도, 7) CD40L/FasL와 사이토카인에 의한 혈관내피세포에서 VEGF 수용체(VEGF receptor-1,2; FLT-1 and FLK-1)의 표현 조절을 조사
[제 3 차년도] 1) IL-4가 혈관내피세포에서의 신생혈관 억제기전 연구: 세포주기에 대한 분석, 2) IL-4의 내피세포에서의 VEGF 수용체 발현에 대한 영향 및 부착분자 표현에 미치는 영향 조사, 3) VEGF가 단핵구 및 활막섬유아세포에서 염증유발 물질의 유도에 대한 조사, 4) 관절염 동물 모델 구축; Collagen induced arthritis model 및 관절염 중증도 평가, 5) 항 VEGF hexapeptide 및 sFasL를 관절염 동물모델에 투여함으로서 치료효과 및 그 기전 조사
1) 류마티스 관절염 환자에서 VEGF의 임상적 의미(염증지표와 순상관관계를 보임)을 규명하였으며, 2) 류마티스 활막염은 T-세포, 활막섬유아세포, 혈관내피세포의 상호 관계(cross-talk)에 의해 상호 증폭되므로 이들 세포들에서 활막염(synovitis)에 영향을 주는 인자로서 T세포에서 CD40L, FasL의 표현에 영햐을 미치는 사이토카인 및 약제들의 영향을 규명하였다. 3) 특히 활막섬유아세포에서 VEGF가 주로 생성되므로 VEGF의 생성에 대한 positive, negative regulator를 분석하였으며, 여기서 CD40L를 비롯한 호염성 사이토카인은 VEGF의 유도를 담당하며, 항염성 사이토카인인 IL-4 및 항류마티스 약제인 COX-2 억제제 및 calcineurin inhibitor인 cyclosporine이 VEGF의 생성을 억제하였으며, 여기에 관련된 기전으로 c-AMP와 AP-1 pathway를 통하여 일어남을 규명하였다. 4)sFas ligan은 활액내에서 의미있게 증가되어있으나, 이를 통하여서는 활막섬유아세포의 증식이 억제되지 않았으며, 이것은 FLIP의 과발현과 연관되어 있음을 확인하였고, sFas를 활막섬유아세포에 ligation 시켯을때, VEGF의 생성이 특이적으로 억제됨을 확인하였다. 5) 내페세포에 대한 IL-4 는 chemotaxis와 tube formation을 억제시켰으나 sFasL는 tube formation에는 영향이 없었으나 chemotaxis 억제효과를 보였다.
6) IL-4는 혈관내피세포에서 VEGF 수용체 2의 표현을 억제시켰으며 또한 cyclin D, E의 발현을 억제하였다. 7) 관절염 동물 모델인 Collagen induced arthritis를 구축하여 항VEGF hexapeptide를 처치하였을 경우에 치료효과 및 그 기전을 조사하였으며, VEGF는 혈관신생 유도 기전이외에도 직접적으로 염증유발 물질의 유도기능을 가지고 있음을 확인하였다.

In viewing of characteristics of synovitis, it could be hypothesized that persistence and perpetuation of synovitis occurs the result of intimate cross-talk between the T lymphocyte and synoviocyte and T lymphocyte-endothelial cells.

The following results was obtained; 1) Ciculating levels of VEGF levels in rheumatod arthtiris was positively correlated with the clinical and laboratory measures of RA 2)Given that the synovitis would be augmented by the interaction between the T lympohocyte- synoviocyte as well as the T lymphocyte-endothelial cells, the factors and drugs influencing the expression of CD40L, FasL was defined 3) IL-4 and COX-2 inhibitor as well as calcineurin inhibitor cyclosporine was revealed to suppress the expression of VEGF in synoviocyte, while the CD40 ligand increase the expression of VEGF in synoviocyte. The pathway inhibiting the VEGF expression in synoviocyte was mediated by the c-AMP and AP-1 pathwasy. 4) The levels of soluble FasL was significantly increased in synovial fluid in patients with rheumatoid arthritis and was inversely correalted with the levels of VEGF. However, increase soluble fasL did not induce the apoptosis of synoviocyte. The resistance of apoptosis by sFasL was associted with high expression of FLIP, which is the anti-apotototic molecle. Furthermore, treatment of sFasL on synoviocyte inhibited the expression of VEGF by dose-dependent manner. 5) IL-4 inhibited the chemotaxis and tube formation of HUVEC, while sFasL only inhibited the chemotaxis. 6) IL-4 inhibited the expression of VEGF receptor II, and expression of cyclin D and E. 7) VEGF exhibted the proinflammatory effect by the induction of TNF-alpha and IL-6 in monocyte and synoviocyte 8) Anti-VEGF hexapeptide significantly decreased the induction and severity of CIA. Collectively, these study provide the basic knowledge in the modulation of VEGF expression and in vitro effect on angiogenesis and inflammation in vitro and in vivo.
