Ischemic cell injury may arise from complex interactions including disturbances in energy metabolism and modifications in synaptic transmission. Several studies have shown that the depolarization of membrane is a trigger signal for the induction of neuronal hyper-excitability, irreversible cellular dysfunction, and cell death, which is mainly mediated by activation of glutamate receptor. Most likely brain ischemia, myocardial ischemia initates various cellular changes progressively, ultimately leading to irreversible myocardial injury, cell death, and tissue necrosis. Since complex mechanisms are involved in cellular changes caused by ischemia, several strategies for the protection against ischemic injury have been proposed, including oxygen free radical scavangers, adenosine agonists, calcium channel blocks, and ATP-sensitive potassium channel (&K_{ATP}$) openers, etc. &K_{ATP}$ openers have shown protective properties on the ischemia-reperfusion injury both in heart and brain, presumably through "ischemic preconditioning" an endogenous protective mechanism. Certain similarties for preconditioning between heart and brain mediated by &K_{ATP}$ channel have been reported, raising possibility of &K_{ATP}$ openers as cardio and neuroprotective agents. Since the cardioprotective and vasorelaxant potencies of &K_{ATP}$ openers follow distince structure-activity relationships and can be separable, substantial amount of efforts have been focused to find more cardioselective and safer agents.
