소염진통제 중 비스테로이드성 소염진통제(NSAIDs)는 염증성 질환의 치료와 각종 급만성 질환에 동반되는 발열 및 통증의 완화를 위해 널리 쓰이고 있으나 효과와 더불어 많은 부작용을 유발한다. 대표적 부작용으로는 위장관 기능 장애, 소화성 궤양, 심장 또는 신기능 장애등 이 있고, 그 중에서 위장관 병증이 임상적으로 가장 흔하다. NSAIDs 처방은 점차 증가하고 있으며 더 늘어날 것으로 예상 되나, NSAIDs의 부작용에 대한 약물반응연구는 부족한 실정이다. 대사체학을 이용한 생체시료의 분석 방법으로 전 세계에서 다양한 연구가 수행되고 있으며 대사체학적 기법을 이용하여 암 및 고혈압 등의 질병, 약물에 의한 치료 효과 및 독성에 관한 연구를 수행하고 있다. 본 연구는 이러한 추세에 맞추어 약물에 의한 부작용으로 발생되는 병증을 예측하기 위한 대사체학적 분석방법을 모색하고, 이러한 결과를 토대로 약물 안전성 평가 및 부작용을 예측하기 위한 생체지표물질을 탐색하고자 한다. 본 연구에서는 NSAIDs 가운데 사용빈도가 높은 Naproxen sodium, Etodolac, Ketoprofen을 대상 약물로 선정하여 소염진통제의 안전성 평가를 위한 대사체 연구 실험을 진행하였다.
각 약물의 농도 선정 실험을 통해 대조군과 저농도, 중농도, 고농도로 농도를 선정하고, 실험동물(랫드)에 투여하여 위 손상을 유도하였다. 약물 투여 7시간 후 적출한 위의 손상정도를 OLYSIA BioAutoCell ver.3.2 프로그램을 이용하여 위 손상도 값을 산출하였다. 이들 약물 모두는 one-way ANOVA와 Tukey's multiple comparison test에서 통계적 유의성을 보여주었으며, Naproxen sodium, ketoprofen의 경우, 10~100mg/kg에서 농도 의존적으로 위 손상이 증가하였는데 비하여, etodolac의 경우, 약물 농도 선정 실험에서와 달리 150mg/kg에서 500mg/kg 과 비슷한 정도의 위 손상을 보였다. 위 손상이 진행되는 동안 실험동물(랫드)의 뇨를 일정하게 채취하고 NMR을 분석하여 1-ethylnicotinamide, 2-hydroxyisobutyrate 등 46개의 내인성대사체 물질을 도출하였다. 이 내인성 대사체의 그룹간 변화를 SIMCA-P+ 소프트웨어를 사용하여 다변량 통계분석 한결과 glabal 및 targeted profiling에서 모든 약물에서 저, 중 및 고농도 투여시 내인성 대사체들이 명확히 clustering 되었다. PCA 다변량 통계분석(주성분 분석)의 loading plot과 VIP(variable mportant points) 결과에 따라 그룹간 변화에 영향을 주는 내인성 대사체 성분들을 도출하고 one-way ANOVA test를 통해 통계적으로 유의성 있는 성분을 확인하여 NSAIDs에 의한 위손상 예측생체지표로 citrate, 2-oxoglutarate, 1-methylnicotinamide, 3-hydroxybutyrate, acetoacetate, succinate, hippurate를 도출하었다.
본 실험을 통하여 실험동물(랫드)에서 대사체학을 이용하여 뇨의 NMR 데이터로부터 global profiling 및 targeted profiling을 통하여 내인성 대사체 변화 패턴을 분석함으로써 소염진통제에 의한 위 손상을 예측할 수 있음을 확인할 수 있었고, 소염진통제 약물의 안전성 유효성스크리닝 및 의약품 평가를 위한 대사체 생체지표 후보물질을 제시하였다. 본 연구를 통하여 도출된 위 손상과 관련된 내인성 대사체는 위 손상 여부를 예측하고 진단하는데 이용 가능할 것이다. 특히 극소량의 대사체 변화를 신뢰성 있고 감도가 우수한 방법으로 검출함으로써 질병의 증상이 발현되지 않는 초기 상태에서의 조기진단이 가능할 것으로 사료되며, 대사체군 프로파일링(profiling)을 통하여 약물의 예측가능한 부작용과 독성 screening 및 약제 효능을 예측하여 신약후보물질의 표적화(targeting)에 기여할 수 있을 것으로 기대된다. 또한 확보된 대사 profile의 데이터베이스화를 통한 정보제공은 향후 이를 바탕으로 한 대사체학 이용 의약품 개발지원 및 평가에도 활용 될 것이다.

NSAIDs can cause side effects which included stomach-bowel functional disorder, gastirc ulcer, cardio or kidney-related disorder, among which the stomach-bowel disorder is most common. The prescription of NSAIDs has increased and is expected to rise. There hasn't been much research going on the clinical testing of NSAIDs' side effects. Analysis methods for biological samples using the metabolomic approach can be used a tool for monitoring the progression of a disease from the onset of the disease, the development to the treatment. A large number of research institutes all around the world are performing studies on diseases such as cancer and hypertension, therapeutic effects of drugs and toxicity using the metabolomic approach. Under the circumstances, this research project was conducted to find analysis methods using the metabolomic approach to predict the onset of the disease resulting from drug side effects and to look for biomarkers for evaluating drug safety and predicting drug adverse events. In the project, tests were conducted on the frequently used Naproxen sodium, Etodolac and Ketoprofen among NSAIDs. In comparison with the control group, the concentration was set as low, medium and high dose through the conduct of tests designed to set dose degrees of the drugs. They were injected into laboratory animals (rat) to cause stomach damage. The stomach was removed from the laboratory animals after 7 hours of injection, respectively. The values of stomach damage were obtained using the OLYSIA BioAutoCell ver.3.2 program. As a result, a statistically significant differences were shown among all of the three drugs in one-way ANOVA and Tukey's multiple comparison test. In case of Naproxen sodium and ketoprofen, stomach damage was found to be increasing depending on the degrees of concentration ranging from 10 to 100mg/kg. On the other hand, as for etodolac, similar levels of stomach damage showed from 150mg/kg to 500mg/kg in contrast with results of tests for setting concentration degrees of the drugs. Multivariate statistical analysis was conducted on endogenic metabolites to identify changes in different groups by using the SIMCA-P+ software. A total of 46 endogenic metabolites including 1-ethylnicotinamide and 2-hydroxyisobutyrate were obtained after NMR analysis of rat urine collected at uniform levels. The results showed that metabolites of urine were clustered according to their drug response in global and targeted profiling. Each group were identified by PCA loading plot, VIP(variable important points) and one-way ANOVA test. We were able to search the endogenous metabolites citrate, 2-oxoglutarate, 1-methylnicotinamide, 3-hydroxybutyrate, acetoacetate, succinate, hippurate for estimating the gastric damage of NSAIDs. The research outcomes showed that stomach damage caused by anti-inflammatory pain killers could be predicted by analyzing change patterns in endogenic metabolites through the global profiling and targeted profiling of NMR data and the analysis of endogenic metabolites in urine using the metabolomic approach. In addition, it was found that changes occurred in endogenic metabolites depending on degrees of damage in stomach and their correlation was proven. Furthermore, candidate metabolite biomarkerswere identified for safety and effectiveness
screening of anti-inflammatory pain killers and drug evaluation. Substances obtained through the research project can be used as endogenic metabolites associated with stomach damage to predict and diagnose stomach damage caused by anti-inflammatory pain killers. Since even the minimum amount of changes in metabolites can be detected in a reliable and sensible manner, a disease can be diagnosed in its early stages where symptoms of the disease are not developed. The outcomes are expected to help strengthen the screening of predictable side effects and toxicity of drugs through the metabolite profiling of the animals used in the research and to contribute to the drug candidate targetingby predicting the drug efficacy. The database will be established based on the obtained metabolite profiles to provide useful information and is expected to be used to provide assistance for drug development using metabolomics and the drug evaluation.
