우리나라 보건산업은 성장잠재력이 큰 것으로 평가되나, 소비자 기호의 급변, 고기술 분야로의 발전 여부, 전반적인 산업 개편의 필요성(영세성 탈피) 등 다양한 기와와 도전 요인에 직면해 있다.
2000~2006년간 보건산업의 총생산액은 36.6% 증가하였다. 보건산업이 우리나라 전체 GDP에서 차지하는 비중은 2006년 5.9%이며, 제조업에서 차지하는 비중은 2006년 23.9%였다. 최근 의료기기산업이 연평균 생산액 증가율이 14.3%로서 가장 빠른 속도로 성장하고 있다.

우리나라 보건산업은 성장잠재력이 큰 것으로 평가되나, 소비자 기호의 급변, 고기술 분야로의 발전 여부, 전반적인 산업 개편의 필요성(영세성 탈피) 등 다양한 기와와 도전 요인에 직면해 있다.

식품산업에서는 과자류, 라면, 음료 등의 생산이 감소하는 반면, 기구용기포장지 제조업, 편의식품을 중심으로 한 기타식품류 등의 성장이 두드러지고 있으며, 향후 이와 같은 추세가 이어질 것으로 예상된다. 가공식품류와 더불어, 현재 빠른 성장 속도를 보이고 있는 기구용기포장지 제조업과 향후 발전가능성이 높은 건강보조식품 등의 해외시장 진출 확대가 필요한 시점이다. 의약품산업은 현재 우리나라 GDP에서 차지하는 비중이 1.45%로서 시장규모의 확대가 필요하며, 이를 위해 신약개발 촉진, 해외시장 확대가 시급하다. 또한 강점이 있는 제네릭 시장과 신약 시장을 동시에 성장시키기 위한 노력이 필요하다. 의료기기산업의 경우 대형업체로 생산이 집중되는 현상이 나타나고 있는바, 이는 기술집약적인 산업의 특성상 바람직한 현상으로 풀이된다. 또한 의료기기산업은 생산액 대비 수출 비중(40%, 2006년)이 상대적으로 높다는 긍정적인 측면이 있다. 의료기기산업의 고기술화를 촉진하기 위해 연구개발지원, 연구개발 인력공급 등이 확충되어야 한다. 화장품산업에서는 소비자 기호의 변화로 기능성화장품의 생산증가가 두드러지는 것이 특징이다. 개방 환경에 따라 수입이 확대될 것으로 예상되므로, 국내산업의 해외진출에 박차를 가해야 할 것으로 전망된다.
본 연구에서 한국 및 주요국의 식의약품 경쟁력을 비교한 결과, 우리나라는 일부 품목을 제외하면 미국 및 EU 5개국에 비해 국제경쟁력을 보유하고 있지 못한 것으로 나타났다. 특히 우리나라가 경쟁력을 보유하고 있는 품목((030341, 030342, 030343, 030710, 030791, 080240, 110812, 121120, 180610, 190230, 210112, 210310, 220890, 390610, 391239, 340211, 901812))이라고 하더라도 선진국과의 경합관계에 놓여 있어 향후 경쟁 심화가 불가피한 것으로 분석되었다.
한국의 FTA 정책에 따른 수출 증대 예상 품목을 분석한 결과, 중국 및 ASEAN과의 FTA에서 식의약품 수출 증대 예상 품목이 가장 많으며, 선진국 및 여타국과의 FTA에서는 수출 증대 예상 품목 수가 미미한 것으로 분석되었다. 다만, FTA 체결 상대국 시장에 수출특화하고 있는 품목을 고려할 때 이들시장에서 수출 증대의 가능성이 있는 것으로 나타났다.
식의약품 분야에서 국내 기업들의 R&D투자 금액이 증가하고 있기는 하나 분야별 선두 기업을 제외하고는 매출액 대비 R&D 투자 수준이 세계적인 수준에 미치지 못하고 있다. 특히 의약품과 의료기기는 R&D 투자가 높은 산업으로 주요 글로벌 회사의 R&D 집약도가 의약품 산업 14%~20%, 의료기기 산업 4%~13% 반면 국내 기업은 의약품 산업 4%~6%, 의료기기 산업은 메디슨을 제외한 대부분의 기업이 1% 미만이다. 영세한 국내 시장과 기업 규규모를 가지고 거대 글로벌 기업과 경쟁하기 위해서 한국형 모델 개발이 필요하다. 예를 들면 의약품 산업의 경우 국내 기업(신약후보 개발)- 글로벌 제약회사(임상실험, 승인, 마케팅), 국내기업간의 공동연구, 개발 비용이 적게 드는 국산 천연물 신약 개발등이 있을 수 있다. 한국의 특허 등록은 국내 시장과 해외 시장에서 점차 증가추세에 있다. 1990년~2005년 동안 미국 시장에서 한국의 특허 등록 건수와 특허 등록 점유율은 식품 산업 129건, 0.8%, 의약품 산업 354건, 0.6%, 의료기기 산업(의료/레져) 569건, 0.4%, 화장품 산업 32건, 0.4%이다. 제품과 시장 확대, R&D 생산성 강화로 인해 세계적으로 대형 M&A가 추진되고 있으나 아직까지 한국기업의 M&A 활동은 미미한 실정으로 한미FTA이후 영세한 국내 기업이 거대 글로벌 제약회사와 경쟁하기 위해서 효과적인 한국형 M&A 전략에 대한 연구가 필요하다.
보건산업의 성장으로 고용 역시 꾸준한 증가추세에 있다. 2002~2006년간 보건산업 가운데 의료기기 산업이 가장 높은 고용증가율(27.3%)를 기록하였고, 의약품산업 23.4%, 식품산업 12.8%를 기록하였다. 화장품산업은 2003년을 기점으로 이전에 비해 고용이 감소하는 추세를 보이고 있다. 보건산업의 대형화, 기술 집약화가 진행되면서 연구인력을 중심으로 한 양질의 전문인력 수급이 중요한 과제이다.
그리고 틈새시장을 노리는 중소기업의 연구인력 고용에 대한 지원이 필요하다.
식품산업 전반의 고용(및 매출액)은 최근 큰 변동 없이 완만한 증가추세를 유지하고 있는바, 국내시장이 이미 성숙기 도달한 상태이며 향후 해외사장 진출 확대가 시급하다. 식품산업 가운데서 수요가 증가하고 있는 낙농제품, 아이스크림 제조업, 기타 식품제조업 등의 연구개발인력은 증가하고 있으나, 곡물가공품 분야 등의 연구인력은 급감하였다. 의약품산업의 고용은 2002~2006년간 23.1% 증가하여 보건산업 가운데 의료기기 다음으로 높은 고용증가율을 보였다. 특히 핵심인력인 연구직이 전체 고용에서 차지하는 비중이 동기간 중 7.7%에서 9.02%로 증가하였다. 전체 의약품산업의 전체 고용 가운데 가장 큰 비중을 차지하는 직종은 영업직으로서 약 34.7%에 달하는바, 업체들의 국내시장 위주의 시장전략을 간접적으로 보여준다. 2006년 현재 의료기기산업의 고용자는 26,339명으로서 최근 빠른 속도로 증가하고 있는바, 2002~2006년간 의료기기산업 고용은 27.3% 증가하였다. 연구인력의 경우 방사선장치, 전기식 진단 및 요법기기 제조업의 연구인력이 급증하였다. 의학지식과 공학을 접목시키는 분야의 특성상 연구인력의 공급이 중요한 과제이며, 특히 전문대학원 등을 통한 고급인력 공급이 중요하다. 과점적 시장구조를 가지고 있는 화장품산업의 고용추이는 일부 대기업에 크게 좌우되며, 틈새시장을 노리는 중소기업의증가가 전체 고용에 미치는 영향은 크지 않은 것으로 보인다. 연구인력의 수요 역시 일부 대기업에 크게 의존하고 있는데, 300명 이상 업체가 고용한 연구인력이 전체 연구인력의 약61%에 달한다. 중소기업들이 특화된 기술을 개발하여 틈새시장을 공략할 수 있도록 연구개발 등 지원이 필요하다.

In general, Korean health industries has been steadily growing. Among them, the medical device and the pharmaceutical industries recently show the highest growth rate. However, health industries face both challenges and chances such as changes of consumers' tastes, successful movement toward high-technology, needs to re-build industrial structure, etc.
During 2000-2006, the Korean health industries have grown by 36.6%. The portion of the health industries in the national GDP is 5.9% in 2006, and that in the manufacture GDP is 23.9% in the same year. Average growth of production is 14.3%.
In food industry, production of ramen(Korean noodle), sweeties, and drinks has reduced while production of medical devices, containers and packers, convenient food has increased. It is expected that this trend would be continued in the future. As domestic food market is saturated, it is necessary that it should widen its market to the overseas. Pharmaceutical industry's portion in the domestic GDP is just 1.45%, which is quite low as we consider its importance in the entire economy. To boost Korean health industries, the development of new drugs and create new foreign markets are essential. Especially, both new drugs and generics are keys to develop pharmaceutical industry. Medical device industry is now growing in the fastest speed among other health industries. The recent growth in terms of manufacturers' size is affirmative due to the industry' tendency toward high technology. among the entire health industries. To move toward high-end products, more government support for R&D activity and more supply of specialized engineers are essential. In cosmetic industry, production of functional cosmetics has increased very fast. Under the condition of market opening, it is expected that more cosmetics would be imported. Therefore, Korean manufacturers have to make an effort to widen foreign markets.
Korea’s food & drug products except for some particular goods such as kimchi, soy sauce, and Korean noodles don’t have international competitiveness as compared with U.S. and EU 5 countries. Some Korean food & drug products that have international competitiveness such as HS 030341, 030342, 030343, 030710, 030791, 080240, 110812, 121120, 180610, 190230, 210112, 210310, 220890, 390610, 391239, 340211, and 901812 items also conflict with other advanced countries’ products in the world market, thereof there are the inevitable deepening of competition in the export market of those items.
According to the economic effects analysis of Korea’s FTA in food & drug industry, the Korea-China FTA and the Korea-ASEAN FTA will contribute the increase of food & drug products export, whereas the effects of the Korea’s FTA with the other countries are relatively insignificant in the food & drug industry. Nevertheless, there is an opportunity of export increase because a good many food & drug products of Korea are export specialized items in the partner’s markets.
As overall size of health industries grows, employment in the industries also increases. During 2002-2006, employment in medical device industry has grown by 27.3%, the highest growth rate among the whole heath industries. Pharmaceutical industry's employment has grown by 23.4%, and food industry' has grown by 12.8%. However, employment of cosmetic industry has decreased since 2003. As the size of producers in health industries keeps bigger and move toward high-end products, enough supply of specialized R&D workers is required.
Moreover, support for small and medium-sized firms who experience shortage of R&D workers.
Since Korean food market seems to be saturated, dramatic increase of employment in the industry have not been witnessed recently. Growing sectors such as dairy, ice cream, and other foods have shown fast increase of R&D workers. Employment in pharmaceutical industry has increased by 23.1% during 2002-2006, which is the second fastest growth rated in health industries. The portion of R&D workers among the total employees has grown from 7.7% to 9.02% in the same period. Sales persons' portion in the entire employees is the biggest, about 34.7%, which reflects the tendency of Korean pharmaceutical firms to focus on domestic market by intensified sales strategies. As of 2006, medical device industry employs 26,339 workers, whose number grows has grown most rapidly, by 27.3% during 2002-2006. The number of R&D staffs has been increased very fast recently for high-tech production, such as radiation devices. The supply of specialized workers id of great importance because the industry needs workers who have knowledges in both medical science and engineering.
Education of higher qualified workers is crucial, for example, through specialized graduate schools. Due to oligopolistic market structure of cosmetic industry, employment of cosmetic industry highly depends on a few big players. R&D workers are also concentrated in those big ones: 61% of total R&A workers in the industry is hired by firms who employ more 300 employers. The support for small and medium-sized(who have potential to capture niche markets) to find R&D workers.
In health and medical industry, R&D intensity of domestic companies except for only a few leading companies in each area has yet to come up to the global level in spite of recent increased R&D investment. Comparing pharmaceutical and medical device industries which require high R&D investment, things are worse. R&D intensity of major domestic companies in pharmaceutical industry ranges from 4% to 6% while that of major global companies exists from 14% to 20%. In medical industry, R&D intensity of most Korean companies excluding Medison is less than 1% while that of global companies is between 4% and 13%. Therefore it needs to develop an effective strategy for Korean companies suffering from shortage of R&D expense and small domestic market. On the other hand, the number of patent granted to Korean companies has been increasing steadily both in global and domestic market. The patent market shares of Korean companies are 0.8% in food, 0.6% in pharmaceutical, 0.4% in medical device and 0.4% in cosmetics for 1990~2005 .
The Although global major companies, especially in pharmaceutical industry, have been pushing ahead with M&As due to expansion of markets and products and R&D productivity, Korean companies still do not actively participate in this trend. Studies on developing Korean M&A strategies have to be conducted in order to effectively cope with global competition and prepare for effectuation of KORUS FTA.
