인간 배아줄기세포는 자가증식(self-renewing) 및 전분화(pluripotency) 능력을 가지고 있는 세포로서 포배기 배아의 내세포괴(inner cell mass: ICM)에서 분리, 배양되고 있다. 이들은 적절한 분화 조건에서 인체를 구성하는 여러 종류의 특정 세포로 분화될 수 있고, 이를 이용한 세포치료 가능성에 대해 여러 연구자들이 보고하고 있다. 이러한 배아줄기세포를 이용한 세포치료에 대한 모델 시스템의 개발은 세포치료의 임상적용 이전에 선행되어야 하는 중요한 연구 분야로서 핵심적인 관련 기반 기술과 임상적인 평가 기준 개발에 필수적인 것으로 생각된다. 본 연구에서는 인간 배아줄기세포의 초기 분화과정에서의 형태학적 변화를 주기적인 관찰을 통해 모니터링하고 전자현미경을 이용하여 미세구조의 변화를 관찰하였으며, 세포치료제로서의 유효성을 평가하기 위해 인간 배아줄기세포를 인슐린 분비 세포로의 분화를 유도하였다. 효과적인 유도 분화 방법을 개발하기 위해 혈청이 존재하지 않는 배양액에 여러 가지 기능성 물질들을 첨가하여 단계적 배양을 실시하였다. 이러한 분화과정에서 인슐린 분비세포로의 분화 과정에 필요한 유전자들의 발현 양상을 real time RT-PCR과 immunocytochemistry 방법으로 살펴보았으며, 체외배양액으로 분비되는 인슐린의 양을 RIA 방법으로 측정하였다. 또한, 분화세포들의 생체내에서의 기능을 확인하기 위해 인슐린 분비세포로 분화 유도된 세포들을 streptozotocin을 주사하여 만든 당뇨 쥐의 꼬리 정맥을 통해 이식하였다. 인간 배아줄기세포는 분화 과정에서 여러 가지 형태의 세포들로 분화되었으며, 이들의 미세구조도 매우 다양하게 관찰되었다. 특히, 세포와 세포 사이를 연결하는 junctional complex와 세포내 대사과정에서 중요한 역할을 수행하는 mitochondria의 다양한 변화를 관찰할 수 있었다. 유도 분화를 위한 단계적 배양과정에서 GATA-4, Ngn-3, somatostatin, glucagon, alpha-fetoprotein 등과 같은 인슐린 분비세포 관련 유전자들의 발현이 증가됨을 확인할 수 있었다. 여러 가지의 단계적 배양조건에서 마지막 단계에 N2-insulin free 배양액에서 배양하는 방법이 가장 많은 양의 인슐린 분비를 유도함을 관찰하였다. 최종적으로 분화시킨 인간 배아줄기세포(Stage-III)의 인슐린 분비양은 N2+B27-insulin free 배양액에서 가장 높게 7.9±2.0 microU/ml로 측정되었다. 이렇게 유도 분화된 세포를 당뇨가 유발된 쥐에 이식하여 그 치료 효과를 확인하였으나, 혈당치의 감소 현상을 관찰하지는 못하였다. 이러한 결과의 원인으로는 인간의 세포를 쥐에 이식하는데 따른 면역학적 거부반응에 의한 이식 세포의 파괴 또는 정맥을 통해 이식된 세포의 환경 변화에 따른 기능 저하로 생각된다. 따라서, 차후의 연구에서는 면역거부반응이 나타나지 않는 SCID 생쥐를 사용하고, kidney capsule로의 이식과 같은 국소적인 이식 방법을 이용할 계획이다. 인간 배아줄기세포에서 분화된 특정 세포를 분리하기 위해 특이 항체(CD34, HEA, fibroblast) 를 이용한 magnetic cell sorting(MACS) system방법으로 세포를 분리, 배양하고 이들의 분화 특이성을 확인하였다. 각각의 항체에 대한 양성 또는 음성 세포들은 서로 상이한 유전자 발현 양상을 나타냈으며, 배양 기간에 따라서 그 양상이 변화됨을 확인하였다. 이러한 특이 항체를 이용한 MACS system은 유도 분화된 세포의 순수 분리뿐만 아니라 특정 계열의 세포를 분리하여 효율적으로 유도 분화시키는데 사용할 수 있을 것으로 생각된다.

Human embryonic stem cells (hESCs) have self-renewing and pluripotent competences which were isolated and cultured from the inner cell mass (ICM) of human blastocysts. Many researchers have been reported that they could be differentiated into the specific cell types and used for cell therapy of degenerative diseases. Prior to the clinical application, the model system of cell therapy using hESCs should be developed by defined criteria and for the evaluation of effectiveness of the cell therapy. In this study, morphological and ultrastructural changes of hESCs during differentiation process were observed to evaluate the differentiation potential, The hESCs were induced differentiation into insulin secreting cells and then the differentiated cells were transplanted into diabetic rats to develop the evaluation markers on the therapeutic efficacy of cell therapy. Undifferentiated humanES cells were formed embryoid bodies (EBs) by suspension culture. Differentiation procedure of EBs using serum-free culture media was consisted of three steps with several growth factors and reagents. The expression of differentiation related marker genes in each stage was analyzed by real-time quantitative RT-PCR, and concentration of secreted insulin in culture media of Stage-III cells was measured using human insulin specific RIA kit. The mRNA expression of endoderm marker genes (GATA-4, Ngn-3, Insulin, Glucagon, Somatostatin) was enhanced by the stepwise sequential culture media, with concomitant decreases in the expression of undifferentiation (Oct-4), ectoderm (Nestin, NF-68KD) and mesoderm (Enolase, Brachyury) marker genes. Concentration of secreted insulin in the stage-III cells with B27 supplement was significantly higher than those without it (7.9±2.0 U/ml vs 3.0±0.2 U/ml, p < 0.01). For the functional analysis of the differentiated cells, the cells were transplanted into the streptozotocin injected diabetic rats. However, we could not observed the decrease the blood glucose level after the cell transplantation. The reason of this phenomenon may related to the immune rejection of trnasplanted human cells or the dysfunction of cells by changes of environmental conditions. Therefore, we are planning to use immunodeficient SCID mice and to transplant at local site of kidney capsule. Magnetic cell sorting (MACS) systems with specific surface antibody was applied for isolation of specific cell types from differentiated hESCs. The isolated cells by MACS with antibodies to CD34, HEA and fibroblst were cultured and characterized their gene expressions. We found that the MACS system could be useful for the isolation and guided differentiation of hESCs into specific cell types. Several materials and systems are testing to develop novel and effective methods for the guided differentiation of hESCs into insulin secreting cells.
