◆ 연구내용 및 방법
제 1 세부과제
1) 직장 신경내분비종양의 생물학적 특성 분석
직장 신경내분비종양 284례를 대상으로 병리학적 소견을 조사하여 림프절 전이와의 관련성을 분석하였으며, 또한 L 세포 표지자인 GLP1, PPY, PYY 염색 및 EC 세포 표지자인 somatostatin 염색을 시행하여 L세포형과 non-L세포 형간의 임상적 양상을 비교분석하였음. 분석 결과 0.5cm 이상 1cm미만의 종양에서도 4 %의 전이율을 보일 뿐 아니라 림프관 침범이 없는 경우도 림프절 전이가 있었음. 직장 신경내분비종양의 80%가 L세포로 되어 있지만, L세포형에서도 림프절 전이를 보이므로 L세포 종양을 경계성으로 분류하는 것은 모순이라는 결론을 얻음.
2) 대장암 종양 표지자로서 Dipeptidyl peptidase (DPP) 발현 의의 규명
대장암 관련 물질로 대장암 환자의 분변 및 조직에서 동정된 Dipeptidyl peptidase (DPP)의 조직 발현 및 그 의의를 규명하고자하였음. DPP10은 대장암 조직 2.9%에서 단백 발현의 완전 소실을 보였으며 DPP10의 발현의 소실은 불량한 예후와 상관이 있었음. 또한 DPP10 발현을 정상 대장-대장암-정상 간-간전이 quadruplet으로 구성된 68 set의 조직에서 DPP10의 발현을 조사한 결과, 정상 조직에 비해 암 조직에서는 발현이 증가되었으나, 간전이에서는 원발암에 비해 발현이 감소되었음
3) 대장의 측방발육형 종양의 육안 형태에 따른 임상병리학적 특성 분석
제 2 세부 과제
1) 혈액에서 저질량 대사체 이온 (low mass ion)을 이용한 대장암 진단저질량 대사체의 분석용 기기로 MALDI-TOF/TOF를 이용하여 대장암을 진단할 수 있는 방법을 연구하였음. Training set으로 정상 203명, 대장암 277명, 유방암 136명, 림프종 51명, 위암 86명의 serum을 이용하여 대장암을 진단할 수 있는 식을 생성함. Validation set으로 정상, 대장암, 유방암, 림프종, 위암, 난소암, 대장 선종환자들의 serum을 이용. MALDI-TOF/TOF에서 얻어진 mass spectrum을 이용하여 대장암과 정상인을 구별할 수 있는 식을 개발하여 low mass ion peak 중에서 중요한 peak를 선별하고 가중치를 부여함. 알고리듬을 2개를 생성함. 알고리듬 1은 주로 대장암을 포함한 암 환자와 정상을 구별하고 알고리듬 2는 대장암과 다른 암종을 구별하는 식임. 10000개의 peak를 사용하는 것보다 선별된 104개, 23개의 peak를 사용할 때 민감도, 특이도가 높았음.
2) CEA 관련 임상병리학적 분석 및 in vito assay
대장암 조직 388례에 대한 CEA 면역조직화학 발현을 조사한 결과 재발, 분화도, 신경 침습, 침윤성 성장과 연관이 있었으며 단변량분석에서 무병생존율과도 연관이 있었음. CEA 음성 및 양성 대장암 세포주를 대상으로 세포의 증식, 침습, 방사선 감수성 등에 대한 in vitro assay 시행 결과, CEA 발현 저하와 세포 침습 및 방사선 감수성은 차이가 없었으나 CEA 음성 세포 주인 LOVO세포주를 대상으로 CEA 발현을 억제시켰을 때는 대조군에 비해 침습이 감소
3) Coagulation과 관련된 물질(D-dimer, Fibrinogen degradation product)의 대장암과 정상인의 농도 비교
제 3 세부과제
1) Calgranulin B의 조직 특이 발현 및 대장암에서의 발현 의의 규명
2) 대장암 분변 표지자로서 DMBT1
3) 분변에서 FOBT 기반 calgranulin B 와 DMBT1 층화 분석에 의한 대장 질환 감별.
Candidates 로 선정된 DMBT1, calgranulin B를 cancer 환자와 정상 대조군, 염증성 장 질환, 대장선종 환자에서 Western blot을 통해서 각각의 optical density를 측정하여 cancer 와 정상 대조군사이 cut-off valuse를 구하여 감별진단에 이용하였음. Test set (group A; training set) 과 validation set (group B)로 나누어서 각각의 군에서 대장암 진단률 (민감도, 특이도)를 구함. Test set (group A)에서 구해진 각각의 cut-off value를 validation set 및 combine set에 적용하여 cancer 및 선종 및 염증성 장질환을 포함한 대장질환의 진단 민감도 및 특이도를 구함. FOBT를 기반으로 하는 층화 분석으로 대장암, 대장암+진행성 용종, 대장암+진행성 용종+염증성 장질환의 진단률이 대장암은 83%에서 91%까지 향상, 대장암+진행성 용종은 81%에서 89%까지 향상되었고, 대장암+진행성 용종+염증성 장질환은 79%에서 89%까지 향상되었고, 층화 분석 방법을 진단 KIT에 이용하여 Proto-type의 진단 kit을 고려해 볼 수 있겠음.
4) 대장암 환자 분변에서 western blot후 LC-MS/MS 후 list-up한 proteins 후보군 중에서 candidates로 선정한 marker의 preliminary 하게 cancer 와 control 군을 대상으로 western blot 실험 진행하여 진단 marker로서 가능성 탐색

1) Identification of Tumor-specific Biomarker in Adenomas and Carcinomas of Colorectum

1) Identification of Tumor-specific Biomarker in Adenomas and Carcinomas of Colorectum
- Biologic Characterization of Rectal Neuroendocrine Tumors
We characterized of biologic features of 284 rectal neuroendocrine tumors (NET), and immunohistochemical staining for GLP1, PPY, PYY, somatostatin was performed to subclassify L cell or non-L cell type. Rectal NET shows lymph node metastasis even in 4% of tumors with size of 0.5cm-0.9cm, and lymph node metastatis is related to tumor size, pT stage, lymphatic invasion and mitotic counts. Seventy-nine percent of rectal NET is L cell type and L cell tumors can show lymph node metastasis.
- Dipeptidyl peptidase 10, a noble prognostic marker in colorectal cancer By using proteomic analysis, we aimed to identify proteins related to metastasis of colorectal cancer to the liver. Proteomic and Western analysis showed that dipeptidyl peptidase (DPP) 10 was downregulated in primary tumor compared with normal colonic mucosa. Loss of DPP10 expression was found in 2.9% of primary colorectal cancers, and loss of DPP10 expression was an independent factor for poor patient prognosis (p = 0.008).
- Clinicopathological differences of laterally spreading tumors of the colorectum according to gross appearance We investigated the clinicopathological features of specific four subtypes of 158 laterally spreading tumors (LSTs). The mean size and the location of the LST varied according to the subtype. High-grade adenoma and malignant transformation were more frequent in LST-G-NM and LST-NG-PD than in the other type. In particular, submucosal invasive carcinoma was associated more frequently with larger LST-G and LST-NG tumors, and two-thirds of LST-NGs 30 mm were associated with invasive carcinomas.
- Absence of hMLH1 or hMSH2 expression as a stage-dependent prognostic factor in sporadic colorectal cancers
This study was designed to test the prognostic value of MMR status in sporadic colorectal cancers.MMR status of 318 colorecal cancers was determined by the immunohistochemical analysis of hMLH1 and hMSH2 expression. MMR status was not a statistically significant prognostic factor in stage I and II patients, while the MMR defect predicted a significantly better overall survival in stage III and IV patients. Thus, MMR status may be a stage-dependent prognostic factor in patients with sporadic colorectal cancer.
2) Identification of Blood Tumor Marker in Colorectal Cancer Patients without Elevation of Serum CEA Level
- Diagnostic marker for colorectal neoplasm
To identify the blood diagnostic marker for colorectal neoplasm, we collected the blood from normal people, colorectal patients and other cancer patients. After getting the peaks of spectrum from MALDI-TOF Mass spectrometry using blood sample, the peaks which discriminate patients with colorectal cancer from non-colorectal cancer were selected. In the field of low mass ion, we developed the equation selected from peaks in training set. We validated this low mass ion equation in validation set. The sensitivity and specificity of low mass ion equation were 94.41% and 88.36%, respectively. The diagnostic discrimination of this low mass ion equation was better than fecal occult blood test. This low mass ion equation can be a useful diagnostic marker for colorectal cancer. Among peaks, the most discriminative peak was “fibirinogen alpha chain”. The fibrinogen levels of patients with colorectal cancer were higher than those of normal people.
-Prognostic marker for colorectal cancer
a) Fibirinogen
We analyzed the correlations between preoperative plasma fibrinogen levels and clinicopathologic parameters, disease-free survival and overall survival in 640 patients with curative resection for colon cancer. Mean preoperative plasma fibrinogen levels were 327.37±90.38 mg/dl. Higher preoperative plasma fibrinogen levels correlated with advanced tumor stage, deeper invasion, elevated CEA and elevated mGPS, but not with node stage, venous invasion, angiolymphatic invasion and perineural invasion. Multivariate analysis showed elevated preoperative plasma fibrinogen level, venous invasion, and perineural invasion were independent poor prognostic factors for 5 year disease-free survival. Furthermore hyperfibrinogenemia, advanced tumor stage, angiolymphatic invasion, and perineural invasion were associated with significantly poor 5 year overall survival.
b) CEA expression
To identify prognostic and surveillance roles of carcinoembryogenic antigen (CEA) expression in curatively resected colorectal cancer, 294 patients who underwent curative resection for colorectal cancer were included in this study. CEA expression was positive in 73.1%. CEA expression was an independent prognostic factor for overall and disease-free survival. In patients with positive tissue CEA, follow-up CEA levels were elevated at the time of recurrence in 57.6%. In patients with tumor without CEA expression, follow-up CEA levels were elevated at recurrence in 14.3%. CEA expression is a prognostic factor in colorectal cancer. Tissue CEA evaluation would be helpful to determine the role of serial CEA testing for surveillance in curatively resected colorectal cancer
3) Evaluation of Diagnostic Utility of Calgranulin B and Development of New
Diagnostic Markers for Colorectal Diseases by Using Stool In order to investigate the diagnostic biomarkers for colorectal cancer or adenoma and evaluate the diagnostic methods, we collected the stool samples from patients of colorectal cancer, adenoma and controls. The stool preparated for proteomics analysis (2D-electrophoresis, MALDITOF) The immunohistochemical stain for calgranulin B or DMBT1 was undertaken in tissue expression of colorectal cancer and normal tissues.
-A Stratified Combination Test of Fecal DMBT1 and Calgranulin B with Fecal Occult Blood Test for the Improvement of Colorectal Cancer Diagnosis
Each single marker (FOBT, Calgranulin B, DMBT1) had no appropriate results for diagnostic rates of CRC, in terms of high sensitivity and specificity. However, the stratified diagnostic fecal test showed high sensitivity and elevated specificity compared with single calgranulin B or single FOBT test in CRC. The sensitivity of stratified combined fecal test presented to increase up to 92.6% in CRC. We had a similar data for CRC including advanced adenoma by proceeding stratified fecal combined test. Compared to that of FOBT itself, stratified FOBT employing calgranulin B and DMBT1 showed higher sensitivity for screening CRC.
-DMBT1 expression in colorectal caner
DMBT1 expression in normal colon tissues were weak or faint pattern, but high expression or loss status in colorectal cancer tissues were observed. Loss of DMBT expression associated with distan metastasis and poor prognosis.

Calgranulin B expression was associated with low grade dysplasia of colorectal cancer.
