만성신장질환은 전세계적으로 급증하는 질환이며, 국내 유병율은 약 13%에 달한다. 만성신장질환은 궁극적으로는 부석 또는 이식에 이르게 할 뿐만 아니라 심혈관계 사망을 증가시키는 위험인자로 중요한 질환이다. 따라서, 한국인의 유전적, 사회문화적, 한국적 원인 질환의 특성에 맞는 만성신장질환 코호트 확립을 통해 한국인 만성신질환의 임상 결과를 파악하고, 신기능 약화인자와 심혈관계 등 각종 합병증 발병 관련 기초 자료를 확립하고, 한국적 치료 가이드라인의 확립이 절실히 필요하다. 본 연구의 궁극적 목표는 대한민국 만성신장질환을 대표하는
만성신장질환은 전세계적으로 급증하는 질환이며, 국내 유병율은 약 13%에 달한다. 만성신장질환은 궁극적으로는 부석 또는 이식에 이르게 할 뿐만 아니라 심혈관계 사망을 증가시키는 위험인자로 중요한 질환이다. 따라서, 한국인의 유전적, 사회문화적, 한국적 원인 질환의 특성에 맞는 만성신장질환 코호트 확립을 통해 한국인 만성신질환의 임상 결과를 파악하고, 신기능 약화인자와 심혈관계 등 각종 합병증 발병 관련 기초 자료를 확립하고, 한국적 치료 가이드라인의 확립이 절실히 필요하다. 본 연구의 궁극적 목표는 대한민국 만성신장질환을 대표하는 코호트를 확립하고 만성신장질환의 신기능 변화와 사망률, 합병증 발생 등 임상 경과를 규명하고, 악화 및 합병증 발생의 위험인자, 생화학적 지표, 유전적 요인의 영향 등을 밝히는 것이다. 이를 위해 10년간 장기간 추적 관찰을 위한 질환별 코호트를 확립하고 추적관찰에 필요한 공통수행지침을 마련하고자 하였다. 본 연구의 기본 설계는 성인 4대 질환 및 소아에 대한 만성신장질환 코호트 연구이며, 이를 위해 1차 연도에는 전국 11개 병원, 13개 등록 단위(신장내과와 소아과)를 아우르는 연구협력 네트워크를 구축하였다. 만성신장질환 연구자 네트워크를 구성하였으며, 서울의대 예방의학교실에서 코호트 운영 및 연구방법론을 지원하고, 역학 및 통계적 관리를 담당하였다. 또한, 대상자에 대한 시료 뱅킹과 시료의 안전성 확보를 위한 체계를 구축하였으며, 건강 설문지, 동의서, 전체 CKD 중례 기록지와 세부 질환별 중례기록지, 추적관찰지를 개발하였다. 또한, Web-based CRF를 개발하여 환자 관리 DB를 구축하였다. 향후 10년간 추적 관찰할 수 있는 기반을 구축한 후 하반기에는 성인 및 소아 만성신장질환 환자 등록을 시작하였으며, 2012.2.29일 현재 939명의 소아 및 성인 만성신장질환 환자(성인은 753명, 소아는 186명)를 등록하였다. 이중 기초 자료 분석이 가능한 환자를 분석하였으며, 성인 만성신장질환자의 남녀비는 6:4이고, 평균연령은 52.2세였다. 등록된 성인 만성신장질환 중 신장질환의 원인별로 보면 사구체질환 32.2%, 당요병성 신중 17.2%, 고혈압 및 비단백ㄴ성 신질환 16.2%, 상염색체우성 다낭신 28%였다. 만성신질환을 분기별로 보면 1-2기가 27.9%, 3기 40.1%, 4기 22.9%, 투석전 5기 9%였다. 20세 미만 소아 만성신장질환은 평균연령 10.4세였고, 남녀비는 6:4이었다. 만성신짛화 분기별로 보면 1기 11.5%, 2기 15.6%, 3기 36%, 4기 18.8%, 5기 18.3%였다. 현재 월 150명 이상의 환자가 등록되는 추세로 본 연구의 종료 시점까지는 목표 수 이상의 환자를 등록하여 연구분석에 적합한 코호트가 구성되리라 예상된다. 따라서, 국내 최초로 전국적 규모의 성인과 소아를 아우르는 본 만성신질환 코호트 사업은 향후 한국을 대표하는 만성신질환 코호트로서 만성신질환 관련 연구와, 국가 보건학적 자료로서 매우 유용하게 활용될 것으로 기대한다.

Chronic kidney disease (CKD) is a rapidly growing disease and the prevelence of CKD in Korea is estimated to be around 13% of adult population. CKD not only leads to dialysis or kidney treansplantation but is also a major threat to cardiovascular mortaloty. Therefore, it is imperative to establish a Korean CKD cohort reflecting the genetic and socio-cultural properties of Korean population and, thereby, exploring the natural course and the aggravating factors of CKD and various complications of CKD. The goals of the present cohort study are 1) to establish a CKD cohort representing Korean KCD pupulation, and 2) to investigate the renal progression, mortality, complications, risk factors, role of biochemical parameters and the genetic influence. For these purpose, large-scale CKD cohort, comprising both adult, and children, was established. The basic design of the present study is CKD cohort study comprising the four major etiologic diseases of adult CKD and children. During the first resarch year, a research network encompassing eleven institutions (thirteen emollment units) was established. Experts in the Department of Preventive Medicin, Seoul National University participated as a collaborator and provided a major consultation on the methodology in maintaining the cohort and epidemiological expertise. A biobank was also established for the sampling, safe transport and stocking of the DNA, serum and urine. After finalizing the research protocol, various forms such as health questionnaires, consent forms, case-report forms, follow-up forms were developed. Moreover, web-based case-report forms (CRF) were developed for the systemic management of the patient data. During the second half of the first research year, the investigators atarted to enroll CKD patients. As of Feb 29, 2012, 939 CKD patients (753 adult, 186 children) were enrolled in the cohort study. As for the adult CKD patients, male : female ratio was 6:4 and patients were 52.2 years old on the average. The etiologic diseases of adult patients were GN (32.2%), diabetic nephropathy (17.2%), hypertensive nephropathy (16.2%), ADPKD (28%) and others. The CKD stages were stages 1 to 2 (27.9%), stage 3 (40.1%), stage 4 (22.9%), and stage 5 (9%). As for children, the average age was 10.4 and male to frmale ratio was 6:4. The CKD stages were stage 1 (11.5%), stage 2 (15.6%), stage 3 (36%), stage 4 (18.8%), and stage 5 (18.3%). It is expected that target enrollment number will be met and the adequate cohort will be established. The present CKD cohort wil be the first CKD cohort representing Korean population and will provide a valuable information regarding CKD reserach and also useful data for national health policy.
