본 연구과제에서는 결핵의 예방 백신으로 현재 널리 이용되고 있는 BCG백신 보다 더 방어효율이 높은 예방 백신을 개발을 위한 기초연구를 수행하고자 하였으며, 개발방향은 (1) subunit 백신 (2)유전자재조합 BCG 백신과 DNA 백신 (3) 결핵균약독백신으로 각 과제별로 연구를 진행하고자 하였으며 아울러 (4) 예방 백신으로 개발한 것의 유효성을 실험동물 모델을 통하여 검정하고자 하였으며 그 연구결과를 요약하면 다음과 같다.
1) 결핵균 배양액에서 여러 단계의 정제과정을 통하여 30 kDa, 32 kDa, 32.5 kDa
본 연구과제에서는 결핵의 예방 백신으로 현재 널리 이용되고 있는 BCG백신 보다 더 방어효율이 높은 예방 백신을 개발을 위한 기초연구를 수행하고자 하였으며, 개발방향은 (1) subunit 백신 (2)유전자재조합 BCG 백신과 DNA 백신 (3) 결핵균약독백신으로 각 과제별로 연구를 진행하고자 하였으며 아울러 (4) 예방 백신으로 개발한 것의 유효성을 실험동물 모델을 통하여 검정하고자 하였으며 그 연구결과를 요약하면 다음과 같다.
1) 결핵균 배양액에서 여러 단계의 정제과정을 통하여 30 kDa, 32 kDa, 32.5 kDa, 36 kDa 항원 등을 분리 정제하였으며, 이들 항원은 모두 결핵환자에서 항체의 검출 및 세포매개 면역반웅을 유도할 수 있어 향후 결핵의 진단시약이나 예방백신 성분으로 이용할 수 있음을 제시하였다.
2) 결핵균배양액에서 상기 항원을 제거한 상태에서 면역학적인 유용성이 있는 native 항원을 탐색하고자 복잡하고 다양한 분획과정을 거쳐 PPD 양성건강인의 말초혈액 림프구에서 IFN-γ 생산을 유도하는 항원을 정제하고 N -terminal sequencing을 결과 세 개의 항원이 지금까지 알려지지 않은 새로운 단백질임을 확인하였다. 이 가운데 두 개의 항원을 유전자재조합기법으로 대량 생산하여 정제하였으며, 면역원성을 조사한 결과 antigen 85complex (30-32 kDa) 항원보다 강력함을 확인하여 향후 매우 예방백선으로 유용할 수 있음을 제시하였다.
3) 결핵균 단백질 항원 가운데 주요 연구 대상인 6 kDa, 16 kDa, 23 kDa, 30-32 kDa 38 kDa 등을 유전자재조합기법으로 생산하고 정제하였으며, 이 가운데 32 kDa인 antigen 85A을 결핵균감염동물실험을 통하여 예방효과를 검정한 결과 그 예방효과를 확인하였다.
4) Mycobacterium-E. coli shuttle expression vector를 이용하여 BCG예방백신균주에 외부항원의 발현체계를 확립하고, 결핵균의 19 kDa 항원, 38 kDa 항원, M. bovis의 MPB70항원을 각각 BCG에서 발현하여 그 유효성을 평가한 결과 예방백신인 BCG보다는 미약하지만 어느 정도 예방효과가 있음을 확인하였다.
5) 결핵균항원 38 kDa 항원을 발현하는 DNA 백신 제조기술을 확보하였으며, 38 kDa DNA 예방백신의 유효성을 조사한 결과 일부 장기에서 예방효과가 있음을 확인하였다.
6) 마우스를 이용한 결핵균 감염 모델을 확립하고, 마우스에 결핵균 감염 후 방어면역 지표인 IFN-r 의 측정, 각 장기에서 결핵균수의 측정, 병리조직소견의 관찰 등 예방백신 후보물칠의 예방효과검정에 관련된 제반 검사체계를 확립하였다.
7) 타세부과제의 협력연구차원에서 제 2세부과제에서 준비한 TSP항원의 유효성 검정실험을 수행하였으며, 그 예방효과가 탁월함을 확인하여 향후에 이에 대한 체계적인 연구가 필요함을 제시하였다.
8) 결핵균의 잠보감염 모델인 Comell Model를 확립하였으며, 결핵균 재활성화 (reactivation)를 예방하기 위한 기초연구로 IL-12싸이토카인 예방치료제의 효과를 조사한 결과 폐장에서의 결핵균 reactivation을 일부 예방할 수 있음을 확인하였다.
9) 결핵균의 병독유전자를 knock-out하여 약독백신의 개발에 필요한 결핵균유전자 knock-out system를 개발하기 위하여 먼저 여러 단계의 클로닝과정을 거쳐 대장균 rfbC 유전자의 knock-out system을 확립하였다. 이는 결핵균의 rfbC 유전자 기능을 분석하는데 도움을 줄 뿐 만 아니라 이러한 유전자 knock-out system을 미리 유전학적인 도구들이 충분한 대장균 시스템에서 검증하는 실험에 활용하였다.
10) 이어서 본 연구에서 최종 목표로 하는 새로운 결핵백신에 근접하기 위하여 결핵균이 속해있는 세균집단인 항산균 (Mycobacterium) 중 Mycobacterium smegmatis에서 본 유전자 knock-out system을 검증하였다. 본 시스템을 통한 결핵백신 제작에서 표적 유전자의 하나인 rmlC 유전자의 활성 측정법을 확립하고 활성부위를 결정하는 연구를 수행하여, 총 3 개의 truncation rmlC 유전자들을 제조하고 단백발현 벡터에 클로닝하여 각 변이 RmlC 단백들을 발현시켜 그 활성을 측정하였다. 아울러 wild type RmlC 단백을 과다발현 시키고 순수 정제하는 데 성공하였으며 HPLC로 정제 단백의 활성을 확인하였다.
이상과 같은 본 연구과제의 결과는 향후에 결핵균체나 배양액에서 면역학적으로 유용한 항원을 분리 정제하고, 유전자재조합항원을 이용한 subunit 백신, 유전자재조합 BCG 백신, DNA 백신, 결핵균약독백신 등의 개발에 널리 활용될 수 있을 것으로 생각하며, 특히 결핵균감염실험동물을 이용한 예방백신후보물질의 유효성평가 기술을 여러 연구자들에게 제공할 수 있는 계기가 되어 예방백신개발 연구는 물론 결핵약제의 개발연구의 활성을 제고할 수 있을 것으로 생각한다.

Tuberculosis (TB) is one of the major public health problems in the world. More than a third of world populations are infected with Mycobacterium tuberculosis, and there are about 8 million new TB cases and 3 million deaths each year in the world. Current control strategies against TB consist mainly of early diagnosis, effective chemotherapy, and BCG vaccination. With lack of satisfactory diagnostic tests and with dIfficulty in access to adequate supply of drugs in the countries with high prevalence of TB, an effective and affordable vaccine against the disease would be of great value for worldwide TB control programs. Although BCG vaccine has been widely used in most of the countries in the world, its protective efficacy against TB among adults is in doubt and controversy. Recent widespread of HIV infection and a marked increase in multidrug resistant (MDR)-TB cases in certain countries during the last decade brought unprecedented attention for new vaccine development. As continuation of our previous efforts, This study was initiated to develop and evaluate new vaccines, and the specific aims were: (a) to isolate and characterize the native M. tuberculosis antigens which are immunologically active, (b) to produce and characterize recombinant protein antigens of M. tuberculosis, (c) to prepare recombinant BCG and DNA vaccines expressing M. tuberculosis antigens, (d) to develop gene knock-out system(s) in M. tuberculosis for developing attenuated vaccine, and (e) to evaluate the new vaccine candidates in animal model of tuberculosis.
a) For preparation of native subunit vaccines, M. tuberculosis were grown in Sauton medium, and culture filtrate proteins (CFP) were prepared from the culture supernatant. A series of chromatographic techniques were then employed to isolate and purify the immunogenic proteins from CFP, and the major antigens obtained were 30 kDa, 32 kDa, 32.5 kDa, and 36 kDa.
b) The rest of the protein mixture was then used for the second round of chromatographic purification steps, and each fraction was monitored for stimulation of IFN-r production by peripheral blood monocytes of tuberculosis patients. Three major components with small molecular weights were identified, and their N-terminals were sequenced. Based on the sequence data, two proteins were identified, and recombinant proteins were then prepared based on the sequences of TB genome sequences and showed stronger cell-mediated immune responses compared to the native antigens like 30-33 kDa proteins above.
3) Genes of the major M. tuberculosis antigens were cloned and expressed in Escherichia coli, and the expressed proteins were purified and characterized. The recombinant proteins include 6 kDa, 16 kDa, 23 kDa, 30-32 kDa, 38 kDa, etc. Of these recombinant proteins, the 32 kDa protein (antigen 85A) was used in protection experiments in mice and showed a limited efficacy against M. tuberculosis infection in mice.
4) M. tuberculosis antigens such as 19 kDa and 38 kDa and M. bovis MPB70 antigen were expressed in BCG using a Mycobacterium-E. coli shuttle expression vector, and the recombinant BCG vaccines were used in protection experiments in mouse model of TB. However, all of the recombinant BCG expressing M. tuberculosis antigens did not show enhanced protection against M. tuberculosis infection in mice compared their parent BCG.
5) A DNA vaccine expressing the 38 kDa protein of M. tuberculosis using a vector containing CMV promoter, but its protective efficacy was less than BCG.
6) CFP and Triton X -100 soluble proteins (TSP) antigens were evaluated for their protective efficacy in mouse model and showed slightly greater protection against M. tuberculosis infection in mice than BCG.
7) A latent infection model of M. tuberculosis called the "Cornell Model" was established to evaluate preventive therapeutic vaccines, and a DNA vaccine expressing IL-12 cytokine was partially effective in preventing relapse of M. tuberculosis in the Cornell model.
8) A homologous recombination system was developed to use in knock-out of targeted genes of M. tuberculosis as an effort to develop attenuated M. tuberculosis vaccines, and its effectiveness of targeted gene knock-out was demonstrated in M. smegmatis.
9) A series of mutants in rmlC gene of M. tuberculosis were prepared in order to knock-out the gene of M. tuberculosis, and its overexpressed proteins were purified by HPLC and characterized.
In summary, several basic technology were developed and established to prepare subunit vaccines based on both native and recombinant proteins, recombinant BCG overexpressing M. tuberculosis proteins, DNA vaccines expressing M. tuberculosis antigens or cytokines, and attenuated M. tuberculosis vaccine by knocking-out virulence factors. In addition, animal model of TB including the latent infection model was established and used in protective efficacy tests of new vaccine candidates. The results of the study would be, therefore, very useful in developing better TB vaccine(s) in the future.
