위암에 대한 제 2상 전기 임상시험 결과 SKI 2053R은 360 - 400 mg/m2 단독투여시 그 항암 유효율이 17.1%로 cisplatin의 12-20%와 동등또는 그이상의 약효를 나타냈으며, 부작용은 그 강도와 빈도면에서 cisplatin에 비하여 매우 적게 나타났다. 소세포폐암에 대한 제 2상 전기임상시험에서도 SKI 2053R은 400-440 mg/m2 단독투여시 그 항암 유효율은 17.1%였고 부작용은 적게 나타났으며, 비소세포폐암, 자궁경부암 및 두경부암등에 대한 전기 제 2상 임상시험은 계속 수행중이다. 또한 최적의 용법, 용량을 찾아내기 위한 방안의 하나로 24시간 지속정주 제 1 상 임상시험을 현재 진행중이다. 위암에 대한 제 2상 후기 임상시험을 SKI 2053R과 5-FU복합요법에 의하여 실시중인데, 기존의 cisplatin + 5-FU요법과 동등 또는 그 이상의 항암효과를 나타내면서 부작용의 빈도와 강도변에서 비교적 낮은 수준을 나타내고 있다.
Rat와 rabbit에서 실시한 최기형성시험 결과 SKI 2053R은 최기형성이 있는 것으로 나타났으며, SKI 2053R과 타항암제와의 병용시 상가, 상승 또는 길항효과를 관찰한 결과 SKI 2053R과 5-FU는 상승작용을 나타내었다. 정맥내 주입시간에 따른 항암효과 및 독성발현의 추이를 생체외 및 생체내에서 관찰한 결과 SKI 2053R은 농도보다는 시간의존적인 항암효과를 나타내었고, 장시간 저농도로 노출시 단시간 고농도 노출시 보다 독성이 줄어듬을 관찰할 수 있었다.
제제개선을 위한 연구로 용해도 개선을 위한 용매 및 부형제들을 탐색한 연구 결과 물과 에탄올올 1:1로 혼합한 용매에서 가장 용해도가 높았으며, 부형제로서는 ß-cyclodextrin을 사용할 때 그 용해도가 약 20배 이상 증가하고 그 안정성도 우수함이 확인되었다. 또한 액상 주사제로의 검토와 1 vial당 용량을 50 mg 에서 100 mg으로 높이는 연구도 진행하여 그 안정성을
검토한 결과 안정성이 우수함이 확인되었다.

Early phase II clinical trials of SKI 2053R, a new platinum anticancer drug, have been conducted. SKI 2053R has been evaluated in the treatment of 123 patients with stomach cancer (37 patients), non-small cell lung cancer (31 patents), small cell lung cancer (39 patients), head and neck cancer (9 patients), and cervical cancer (7 patients). SKI 2053R was given at a dose of 200-440 mg/m2 1-h iv infusion every three or four weeks with 240 courses. One hundred seventeen patients were evaluated for side effects, toxicity, and response. In all treatment courses, nausea and vomiting and hematological toxicity, particularly anemia, were the main toxic effects but were rarely severe: the observed maximum level of toxicity was grade III according to standard World Health Organization (WHO) criteria.
Nausea and vomiting of grade III was seen in 10 patients, and anemia of grade III occurred in 1 patient. Other toxicities were mild nephrotoxicity, hepatotoxicity, and diarrhea. Of 35 eligible stomach cancer patients, 6 (17.1%) achieved responses. The response rates (percentage) achieved in non-small cell lung cancer (NSCLC), small cell lung cancer (SCLC), and cervical cancer were 5.9, 17.1, and 14.3%, respectively. There is no patient showing a response in head and neck cancers yet. Later phase II clinical trial has being conduct at 9 hospitals with SKI 2053R 400 mg/m2 plus 5-FU 1,000 mg/m2/day for 5 days. SKI 2053R plus 5-FU has been evaluated in the treatment of 40 patients with advanced stomach cancer.
Twenty one patients were evaluated for side effects and response. Of 21 eligible stomach cancer patients, 7 (33.3%) achieved responses. Leukopenia of grade IV was seen in 1 patient, and of grade III was seen in 4 patients. Grade III side effects of proteinuria, elevation of
GOT/GPT, thrombocytopenia, elevation of ALP, and pain were seen in 3, 2, 1, 1, and 1 patients, respectively. The effect of exposure time on the in vitro cytotoxicity of SKI 2053R and cisplatin toward human lung-and stomach-adenocarcinoma cell lines was investigated by various exposure time (1, 4, 12, and 24 h). The cytotoxic effect of SKI 2053R on the ceIl lines was significantly increased with increasing exposure time, however, the cytotoxic effect of cisplatin against cell lines was inversely correlated with increasing exposure time. This results strongly suggest that the therapeutic efficacy of SKI 2053R given by continuous long-term infusion clinical study should be investigated. Phase I clinical trial of SKI 2053R has being conduct with malignant patients by 24-h continuous infusion. SKI 2053R was given to 3 patients at each dose level; 350, 400, 450, and 500 mg/m2 24-h i.v. infusion. Of 12 patients, 4 stable disease were achieved. There was no above grade III or severe side effects The further clinical evaluation of SKI 2053R would be performed at a higher dose.
Conclusively, SKI 2053R has potent anticancer activity to various cancer especially for stomach cancer, and has very low incidence and mild side effects. Therefore, development of SKI 2053R should be continued as a new 3rd generation platinum anticancer agent.
