● 기능성 소화불량증은 식이습관이나 헬리코박터 감염 등의 환경적인 요소 및 유전적인 요소가 복합적으로 관여하여 증상을 나타내는 질환으로서, 상부소화관 지각수용체인 capsaicin/transient receptor potential vanilloid (TRPV1)수용체 발현의 차이가 기능성 소화불량증 환자에서 증상의 차이를 유발하는 것으로 알려짐.

● 삶의 질 척도인 PAGI-SYM (Patient Assessment of Upper Gastrointestinal Symptoms)와 PAGI-QOL (Patient Assessment of Upper Gastrointestinal -QOL)를 조사한 결과, 증상의 종류 및 내시경 소견, 헬리코박터 감염 여부 등과 무관한 것으로 나옴.
● 불안 및 우울증 척도 HADS (Hospital Anxiety and Depression Scale)를 조사하여 우울증 환자를 배제하였으며, 식습관, 음주력, 흡연력도 함께 조사함.
● 각각 한국어와 일본어로 된 식습관 척도를 조사하여 비교한 결과, 한국인들이 일본인들보다 유의하게 매운 음식을 많이 섭취한다는 것을 확인할 수 있었으나, 이는 내시경적 위염 및 혈청 헬리코박터 항체 검사결과와 무관한 것으로나옴.
● 한국인 기능성 소화불량증 환자에서는 postprandial type이 우세한 반면, 일본인기능성 소화불량증 환자에서는 epigastric pain type이 우세한 것으로 보아 매운 음식을 많이 섭취하는 한국인의 식습관인 환경적 요소가 실제로 한국인에서 비감각화를 통해서 기능성 소화불량증의 증상 차이에 영향을 미치기 때문으로 추정됨.
● 한국인과 일본인 사이에 상부소화관 감각수용체 발현의 차이를 보이는지, 나아가서 상부소화관 지각수용체의 서로 다른 발현에 따른 증상의 발현 차이가 있는지 알아보기 위해서 Capsaisin/transient receptor potential vanilloid (TRPV) 수용체의 발현 여부 조사 중이며, TRPV1 polymorphism (1103C>G,M3151, rs222747) 분석을 통해서 strength of association을 제시할 예정임.

● Functional dyspepsia is a complex disease based on various etiology such as genetic polymorphism, Helicobacter pylori infection, diet, and other environmental factors. Recently, it has been reported that a receptor of the upper gastrointestinal tract called capsaicin/transient receptor potential vanilloid (TRPV1) plays a role in different symptomatic manifestation.
● In this study, we evaluated the variables including diet, H. pylori infection, and endoscopic findings of upper gastrointestinal endoscopy to evaluate the differences between Korean and Japanese patients with functional dyspepsia.
● Korean and Japanese adults between 20 and 70 years old who visited the outpatient clinic due to idiopathic upper gastrointestinal symptom were included in the study.
● The findings of PAGI-SYM (Patient Assessment of Upper Gastrointestinal Symptoms)와 PAGI-QOL (Patient Assessment of Upper Gastrointestinal -QOL) showed no correlation beteen the quality of life and type of functional dyspepsia, endoscopic finding, and the presence of H. pylori infection.
● To exclude depression, HADS (Hospital Anxiety and Depression Scale) was evaluated with diet, smoking, and alcohol drinking.
● Korean and Japanese diet survey revealed that there was a significant difference in the intake of spicy foods between two countries.
However, higher amount of spicy food intake was not related to the type of symptoms, endoscopic finding, and H. pylori infection rate.
● Postprandial type was more common in Korean functional dyspepsia patients, while epiastric pain type was more common in Japanese unctional dyspepsia patients.
● Different expression of capsaisin/transient receptor potential vanilloid (TRPV) beteen the Korean and Japanese functional dyspepsia patients are under the evaluation by analyzing TRPV1 polymorphism (1103C>G, M3151, rs222747).
● Through our finding, we could confirm that there is a great difference in the symptomatic manifestation between the Korean and Japanese functional dyspepsia patients, but there was no link between the symptomatic manifestation and endoscopic findings, H. pylori infection, and quality of life.
