The present study aims at suggesting ways to improve the quality of community care services. Toward that end, this study attempted to: (i) calculate the number of the frail elderly in provinces, cities and other areas in accordance with four different definitions of frailness; (ii) calculate the number and examine regional distribution of service facilities that can be used to provide the frail elderly; (iii) conduct surveys that estimate level of elderly care service utilization and service users' satisfaction level; (iv) conduct focus group interviews with elderly care service facility workers in four areas (Daegu metropolitan city, Gumi city, Jangsung County, and Kocksung County) to have a better understanding of grassroots efforts underway to develop networks among community care service facilities; and (v) trace experiences of U.S.A., Canada, and Japan in integrating community care services and extract relevant lessons from them.
The study revealed the following results. First, the number of the frail elderly significantly varied depending on the definitional scope of frailness, ranging from 327,415 (12% of the total elderly population) to 2,640,205 (43%). This suggests that the government should give more consideration on choosing a clear-cut definition along with taking regional differences into account before implementing policy instruments for the frail elderly. Second, there were large regional differences in the number of service facilities that can be used to deliver community care services for the frail elderly, except in the case of health service centers. This result calls for more studies on policy measures for inducing health service centers to deliver community elderly care services. Third, many elderly users of community care services responded that they are highly satisfied with the services they received, evaluating them as effective for enhancing and/or maintaining their health status. Fourth, those working for community care service facilities recognized the need for integration of community services and formal networking among facilities. Currently, they are trying to communicate and establish informal networks among themselves. They were found, however, to think informal networks as limited to a large extent. Finally, several OECD countries standardized assessment tools and placed much emphasis on the integration of elderly care facilities and the development of single entry point for an integrated case management system.
Based on these findings, the study draws some implications for community care policies for the frail elderly. First, community care service facilities should be integrated to provide adequate services to the frail elderly and enhance the efficiency of services. To facilitate such efforts would require formulation of an efficient administrative structure. In addition, a full-scale database must be established to share information necessary for facilitating service integration. Second, regional mid- to long-term plans for the frail elderly should be formulated with emphasis on regional characteristics. Finally, standardized assessment tools and manuals for services should be developed at the national level.
