1. 연구의 목적

1. 연구의 목적
본 연구의 목적은 한국인에게 익숙한 원예활동을 주로 하여 고령의 노인을 위한 한국형 원예치료 프로그램을 개발하고, 노인들을 대상으로 실제 이 프로그램을 실시하여 신체적 및 정신적 효과를 정량화하여 궁극적으로 실제 고령의 노인에게 적용이 가능한 한국형 원예치료 프로그램의 모델을 제시하는 것이다. 두 가지의 다른 프로그램 즉, 주로 섬세한 손동작과 인지능력, 주의 집중력, 기억력 등 정신작용을 필요로 하는 실내형 원예치료 (실내군) 프로그램과 주로 근력, 균형 및 전신적인 협동운동을 필요로 하는 실외형 원예치료(실외군) 프로그램을 개발하여, 양로시설에 수용되어 있는 75세 이상 고령의 비활동적인 노인에서 첫째, 일상생활동작, 섬세한 손동작, 손의 근력, 전신적인 균형감각 , 척추 관절의 유연성 및 민첩성 등의 신체에 미치는 영향을 조사하고, 둘째, 인지기능, 의사소통, 사회인식 등의 정신적 효과를 조사하며, 셋째, 실내군 및 실외군과 프로그램에 참여하지 않은 대조군 간의 차이를 확인하여 치료 효과를 정량화하여, 효과가 과학적으로 입증된 한국형 원예a 치료 프로그램의 하나의 모델로서 제시하고자 한다.
2. 연구의 필요성
경제수준의 향상으로 인하여 평균 수명이 증가한 결과 2001년 현재 우리나라 65세 이상 노인인구가 전체의 7.4%를 차지하고 있으며 점차적으로 고령화 사회가 되어 2030년에는 19.3%에 이를 것으로 보고 되고 있으며 18), 1960년 이후 사회가 급격히 산업화, 도시화됨에 따라 핵가족화가 급격히 진행되어 요양기관에서 생활하는 노인의 숫자가 급격히 증가하고 있다. 이들 요양기관에서 생활하는 노인들은 주위 관심의 결여로 인하여 지적, 신체적 활동량이 매우 적어 가정에서 생활하는 경우에는 발생빈도가 낮은 조기치매, 폐렴, 심장기능 장애, 우울증 등 여러 가지 노인병에 많이 이환되고 있다34,36,38). 기계와 교통수단이 발달함에 따라 신체적 활동량이 더욱 감소할 것으로 예상되는 21세기에는 이런 여러 가지 노인질환 들의 이환율이 더욱 증가하여 매우 심각한 사회적 및 경제적인 문제로 대두될 것으로 예상된다. 21세기의 복지사회로 발전하려는 대한민국에서 노인병 문제는 시급히 해결되어야 할 과제이나 노인 인구의 급격한 증가로 인하여 이들 문제의 해결에는 대단히 많은 비용이 소요될 것으로 예상된다.

The purposes of study were to develope horticultural program for elderly Korean over 75 years of age and to evaluate the physical and psychological effects of this horticultural therapy program using the Functional Independence Measure(FIM), range of motion(ROM) of thoraco-lumbar spine, balance(Borg's balance and Sensory Organization Test(SOT)), fast walk, hand grasp power and finger pinch power, thumb muscle contractibility(RMS), hand dexterity, and Korean Minimental Status Examination(MMSE-K) and also to investigate the difference between indoor and outdoor horticultural therapy. Relatively healthy one hundred and fifty women over 75 participated in the study.
Fifty women, served as controls, the other fifty participated in the indoor horticultural therapy which mainly using both hands in a sitting position, and another fifty women participated in outdoor horticultural therapy which using entire body. Horticultural therapy was done two hours per session, twice a week and continued for 20 weeks. FIM, ROM of thoraco-lumbar spine, balance(Borg's balance and SOT), fast walk, hand and finger grasp power, RMS, hand dexterity, and MMSE-K were checked prior to the therapy, at 10 weeks, and at 20 weeks from the beginning of the therapy. The data were analyzed with one way and repeated measured ANOVA.
The results are as follows:
1. As the therapy continued, the FIM score, ROM of thoraco-lumbar spine, balance(Borg's balance and SOT), fast walk, hand and finger grasp power, RMS, hand dexterity, and MMSE-K were not significantly increased in the control group(p>0.05).
2. In indoor horticultural group, both hand function such as the hand dexterity, the muscle contractibility of both thumb abductor, grasp power of left hand, both pinch power are improved, and the FIM score, especially self-care, sphincter control, communication and social cognition subscales are significantly increased which showed improved hand and cognitive function.
3. In outdoor horticultural group, not only gross motor and balance related test results such as fast walk, Borg's balance, SOT, but hand function including both thumb abductor contractibility, hand dexterity, grasp power of right hand, pinch power of left finger also improved statistically.
4. As compared to indoor group, outdoor group showed improvement in gross motor activity such as balance, walking speed, spinal range of motion.
5. As compared to outdoor group, indoor group showed almost same level of improvement in fine motor strength and coordination such as both hand grasp power, both finger pinch power, contractibility of both thumb abductors, and cognitive function.
We developed indoor and outdoor horticultural therapy program for the elderly Korean over 75 year and proved their effects physically and psychologically. We also proposed and concluded that horticultural therapy ,whether it is indoor or outdoor, is a useful method to promote physical and cognitive ability of elderly Korean without any expensive investment. Further studies are needed to compare horticultural therapy with other therapies especially in energy expenditure or cardiac loading.
