Ⅳ. 연구결과
○ 국내 표본감시체계는 일본의 표본감시체계와 유사하게 중앙부처를 중심으로 지방정부의 보건관련부서로 이어지는 체계로 구축이 잘되어 있으나, 표본감시에 참여하는 의사를 선정하는 과정이 명확하지 않아 자발적 참여도가 떨어지는 것으로 사료된다.

○ 보건의료 선진국의 표본감시체계는 국가별로 차이점이 있는 것으로 조사되었는바, 이는 각국의 문화적 배경과 감시체계가 발전해온 배경이 다르기 때문인 것으로 사료된다. 일본은 관료중심의 문화적 배경과 참여의사들의 직업에 대한 의무감에 바탕하여 좋은 신고율을 보이고 있다. 독일은 연방정부의 예산으로 운영되는 공공연구소가 주관하여 전염병감시체계를 운용하고 있으나, 자발형 표본감시에 참여하는 의사집단을 유지하는데 어려움을 겪고 있다. 영국의 경우 의사단체가 국가에 속하고 있는 보건의료체계에서 공공기관인 감염병센터를 주관기관으로 하고, 의사단체 또는 대학에 표본감시체계의 운용을 위탁하고 병원체검사를 중앙화하여 지원하는 체계로 민-관 협력체계가 효율적으로 운영되고 있다. 프랑스 역시 정부예산의 지원을 받는 공공기관이 전염병감시체계를 주관하고 있으나, 표본감시체계는 대학에 위탁운영하며, 병원체검사 역시 국가지정검사센터를 통하여 지원하고 있는 민-관 협력체계를 운영하고 있다.
○ 설문조사를 통하여 얻은 결과를 보면, 우리나라 의사들은 전염병 신고의 필요성은 잘 인식하고 있으나, 진단의 불명확성 때문에 신고하지 못하는 경우가 많고, 신고절차에 대한 인식이 부족하거나, 신고후 보건당국의 간섭을 불편하게 생각하는 경우도 있었다. 표본감시체계에 참여하는 의사들의 신고체계에 대한 인식도는 비교적 높으며, 관리주체를 비정부기구로 할 경우 적극 참여하겠다는 경우가 현체제의 유지에 호감을 가지는 경우보다 높게 나타나, 이런 결과를 정책에 반영할 필요가 있겠다.
○ 대한의사협회 소속 단위 의사회의 협력을 통하여 시범사업실시 가능성을 확인하였다.

To protect public health from catastrophic outbreak of communicable disease, most countries set the control system of communicable disease. "The communicable disease control law" define the notifiable disease categories; 6 entities of Group I, 9 entities of Group II, 18 entities of Group III and 15 entities of Group IV and 9 entities of Group V. Also socially issuing communicable disease; type A, type B, and type C hepatitis, influenza, sexually transmitted disease (syphilis, gonorrhea, chlamydia, non-gonorrheal urethritis, chancroid, genital herpes, condyloma accuminata), Vancomycin resistant Staphylococcus aureus, foreign input communicable disease(Chagas, angiostrongyliasis, gnathostomiasis, filariasis, hydatidosis) and Creutzfeldt -Jakob disease are classified as group for sentinel surveillance. However, sentinel surveillance system in Korea does not effectively work, because it has been operated non-voluntarily, compulsorily and uncooperatively. The aim of this study is to deduce a scheme improving notification rate in sentinel surveillance system. To reach this goal, our team has been tried to understand the sentinel surveillance system in Korea, to survey the perception of the medical doctors for report their case with communicable disease, and to figure out the sentinel surveillance system with advanced countries for communicable disease surveillance. The hardware of the sentinel surveillance system of Korea had been organized well. However, the process of choosing the medical doctor to join the sentinel system is not clear, so that they have not understand the reason why they have to report the communicable disease cases. After looking around the foreign countries - Japan, Germany, England and France - we felt there are some difference in operating the sentinel surveillance system of each contry, according to the cultural background. The sentinel surveillance system of Japan and Germany is governmental oriented, i.e. the institute to control the sentinel system is belong to the government . However, the system of England and France is a cooperated system between government and nongovernmental institutes, universities or organization with financial support of the government. The result of the survey for perception of medical personnel regarding report of surveillance system, the main reason of lower report rate is that undefined diagnosis seems to make a hesitation to report. Sometimes they are afraid of an uncomfortableness in reporting or they do not recognized the significancy of reporting communicable disease. The medical personnel joining sentinel surveillance system have a good perception for reporting communicable disease than those not joining the sentinel system. They say, if the main body would be changed from the governmental side to nongovernmental organization, they would be active in voluntary sentinel surveillance system. We also have a chance to discuss with some branches of Korean Medical Association to have a cooperative relationship with the government. We find the possibility, if we have a chance to set an cooperative pilot project regarding sentinel surveillance system, they will serve actively to improve reporting communicable disease.
