가. 연구결과 종합
□ 신약개발 분야 제약기업체 자체의 연구개발투자는 지속적으로 증가세를 보이고 있고 2013년에는 정부 R&D 대비 제약기업체 민간 R&D 투자가 5배 이상 높아진 시점에서 정부/민간 R&D의 역할 조정에 관한 논의가 필요함
○ 제약산업 선진국인 미국의 경우에도 과거에는 기업의 역량이 충분하지 못한 점을 고려하여 정부가 타겟발굴부터 임상시험까지 전단계를 지원하는 문제가 발생하였으나, 공공영역과 민간영역의 적절한 역할분담 필요성이 높아지고 민간의 신약개발투자가 2배 이상 확대됨에 따라 정부와 민간기업의 신약개발단계별 역할분담이 발전하고 성장하여 왔음
○ 우리나라의 경우 「범부처 신약개발 R&D 추진전략(’06.2.)」에서 부처간 역할분담(안) 및 신약개발 R&D에 대한 단계별* 정부지원 계획을 제시한 이래로 아직 정부의 전주기적지원(제1기)에 머물러 있으며, 민간의 연구개발 역량, 현장수요 등을 고려한 정부/민간 R&D의 역할 조정에 관한 구체적인 논의가 부족한 상황임
* 제1기(정부의 전주기 지원), 제2기(민간주도 신약개발로의 전환기), 제3기(민간주도 신약개발)
□ 국내 제약산업 부문의 정부/민간 R&D 영역에서 정부 연구개발지원이 민간(기업)의 연구개발투자를 촉진하는지 여부를 분석한 결과, 신약개발 분야 정부 R&D의 투자전략을 재조정할 필요성이 있으며, 특히 개발단계 이후 민간의 연구개발투자를 유인하기 위한 제도적 방안마련이 필요한 것으로 나타남
○ 정부연구비를 지원 받은 국내 제약기업(202개) 및 순수 신약개발사업(과제)를 수행하는 제약기업(113개)을 대상으로 정부 연구개발지원이 민간 연구개발투자를 촉진(대체)하는지 여부를 분석한 결과 기업유형별로 차이가 있는 것으로 나타남
- 대(중견)기업은 통계적으로 유의성이 없는 결과를 보인 반면 연구개발 비중이 높은 중소(벤처)기업은 민간 연구개발투자 유인 효과가 큰 것으로 나타남
□ 정부연구비를 지원 받는 국내 제약기업체의 역량을 파악하기 위해 민간 R&D의 연구개발 파이프라인, 연구개발 투자규모 등 실증데이터를 수집·분석한 결과, 경제적 성과창출을 위해서는 민간주도의 R&D 확대 방안 마련이 필요한 것으로 드러남
○ 정부지원을 많이 받고 있는 혁신형 제약기업(46개사)의 연구개발중인 파이프라인을 분석한 결과, 대(중견)기업의 신약 파이프라인은 합성신약 및 바이오신약에 중점을 두고 있고 개량신약(합성) 파이프라인의 비중이 높다는 특징을 보임
- 혁신형 제약기업 자체 연구개발비 대비 정부연구비 지원규모는 평균 8.6%(’14)로 낮다는 점과 미국, 일본 등의 파이프라인 분포 현황과 비교할 때 우리나라는 개량신약의 파이프라인 비중이 높다는 점 등을 종합적으로 고려할 때, 민간주도의 연구개발투자 확대 및 신약 파이프라인의 양적, 질적 향상이 필요함
○ 국가별 제약산업 부문의 연구개발투자(’11년)를 비교해 보면, 우리나라는 미국의 2%, 일본의 10% 정도로 낮은 수준을 보이며, 글로벌 상위 20대 제약기업(16.3%)에 비해 국내 상위 20대 제약기업(7.6%)의 매출액 대비 연구개발비 비중이 절반 이하로 낮은 수준임을 볼 때, 글로벌 신약개발을 위한 R&D 확대 방안 마련이 필요함
□ 제약산업의 기술적, 산업적 특성을 반영하여 신약개발단계, 의약품종류, 10대 질환 등의 분석기준에 따라 정부/민간 R&D의 투자포트폴리오를 비교분석한 결과, 공공영역은 질환규명, 타겟발굴 등 기초연구에서 주도적인 역할을 담당하고 개발단계(비임상~임상) 이후부터는 민간영역이 주도하는 등 정부/민간영역의 역할분담이 자발적으로 이루어지기 위해서는 장기적으로 민간주도의 역할 조정이 필요한 것으로 보임
○ 2013년 기준으로 신약개발 분야 민간 R&D의 경우 탐색(37.9%)과 임상(40.3%) 단계의 투자비중이 비슷한 수준으로 민간영역이 연구개발 비용이 적게 드는 신약개발단계의 앞부분에 많이 투자하고 있음을 보여주는데, 이는 향후 임상시험 단계에 대한 민간주도의 연구개발투자 확대가 필요함을 시사하고 있음
- 신약개발 분야 정부 R&D는 최근 7년간(’08~’14) 탐색(38.7%), 임상(31.9%), 비임상 (29.4%) 순으로 투자되었으며, 제약기업체를 주로 지원하는 임상 단계의 투자비중이 31.9%로 상당히 높은 편임
○ 의약품 전체에 대한 정부/민간 R&D의 투자포트폴리오를 비교해 보면, 제약기업체 민간 R&D와는 달리 정부 R&D는 신약 분야에 주로 투자
- 의약품 종류별 정부/민간 R&D 투자에서 합성신약의 비중이 각각 38.9%, 35.9%로 가장 높은 것으로 나타남
※ 정부 R&D : 합성신약(38.9%) > 바이오신약(33.6%) > 천연물신약(13.6%) 민간 R&D : 합성신약(35.9%) > 바이오신약(22.0%) > 개량신약(합성)(21.9%)
- 기업유형별로 대(중견)기업은 합성신약, 중소(벤처)기업은 바이오신약에 가장 많이 투자
○ 연구수행주체별로 대학/연구소는 타겟발굴및검증~비임상 단계까지, 산업체는 후보물질도출및최적화~임상3상 단계까지 수행하고 있어 산학연 주체간 역할 분담이 어느 정도 이루어지고 있으며, 특히 제약기업체에 대해서는 거의 전주기에 걸쳐 정부지원이 이루어지고 있음
○ 2014년 제약기업체에 대한 정부연구비 중에서 64.1%(545억원)가 연구개발 역량을 보유한 혁신형 제약기업을 지원하였으며, 특히 정부연구비를 지원 받은 중견기업의 95.6%(378억원)가 혁신형 제약기업에 해당됨
- 기업유형별로 대기업은 비임상~임상, 중견기업과 중소(벤처)기업은 후보물질도출 및 최적화~임상 단계에 정부 R&D 지원이 이루어졌으며, 특히 중소(벤처)기업에 대한 비임상~임상 단계의 연구개발비 중 50% 정도를 정부가 지원하였음
나. 시사점
□ 본 연구에서는 신약개발 분야 민간 R&D의 실증데이터, 정부/민간 R&D의 투자포트폴리오 비교 및 정부 연구개발지원의 민간 연구개발투자에 대한 효과 등 데이터 기반의 분석결과로 부터 정부/민간 R&D의 역할조정 및 투자포트폴리오 조정방향을 제시하였다는 점에서 의의를 찾을 수 있음
○ 연구개발 역량을 보유한 대(중견)기업에 대한 정부 연구개발지원이 민간 연구개발투자를 촉진하는 효과가 없는 것으로 볼 때, 향후 정부 연구개발지원에 따른 민간(기업)이 임상 시험 이후의 개발 단계에 적극적으로 투자하도록 유도할 필요가 있음
- 대(중견)기업에 대해서는 매출액 성과를 재투자하는 등 민간 자체의 연구개발비 비중을 점진적으로 늘려 민간주도의 임상시험이 이루어질 수 있도록 유도하면서 정부 R&D는 기초연구를 지원하는 방향으로 전환할 필요가 있음
○ 연구개발 비중이 높고 신약개발 관련 기반기술을 보유한 중소(벤처)기업에 대해서는 유효한 후보물질의 임상시험에 대한 정부지원은 필요할 것이며, 이러한 기업들에서 정부 R&D 지원이 기업의 R&D 투자 증가로 이어지므로 민간 R&D 투자 확대 및 역량 강화의 효과를 기대할 수 있음
○ 민간주도의 신약개발로 전환 시 정부의 역할은 기초․응용연구 및 국가적 대응이 필요한 질환(희귀질환, 감염병 백신 등)의 신약개발에 집중해야 할 것임
□ 정부예산을 신약개발 전 단계에 충분히 지원하기 힘든 현실적인 상황에서 민간 기업이 유기적으로 산·학·연 등과 연계하여 신약개발에 참여하고 장기적으로 신약개발 주체간 자발적인 역할분담 및 역량공유가 민간주도로 이루어질 수 있도록 시스템 구축이 필요함

In accordance with the continuous increase in government support for the bio-field, level of science and technology with quantitative outcome are steadily increasing. However, economical performance like technology transfer and product commercialization are relatively weak so there is growing emphasis on the efficient investment of government R&D. New drug development, one of the largest R&D investments on the bio-field, was a target of efficient rightsizing and as a result, various improvements have been made in coordination among branches of the government and initiation of new drug R&D project through full cycle in trans-government in 2011. Even though private R&D investment on new drug development were recently increased over 5 times compared to the governmental R&D investment, the government has still supported throughout the entire process of new drug development, considering that domestic pharmaceutical companies lack the capability of R&D. Therefore, necessity of role sharing between government and private sectors is brought up for the field of new drug development. In this study, the field of new drug development, one of the largest R&D investments on the bio-field and in demand of establishing industrial ecosystem, was selected as a pilot application. We collected empirical data of private R&D and proposed the portfolio of investment and a role adjustment between government and private R&D sectors for improving government’s R&D investment efficiency and creating economical performance.
As for research contents, we performed comparative analysis between government and private R&D portfolios and drew the improvement plan based on the effect analysis of government R&D investment on private R&D investment and analysis of capacity of private R&D on new drug development. We studied whether government R&D investment lead to promote or substitute private R&D investment by comparing domestic pharmaceutical companies(202) with government grants and domestic pharmaceutical companies(113) with the drug development project. The statistical analyses showed that the difference between enterprise types exists. We haven’t seen any statistical significance in stimulating private R&D investment of large- and medium-sized companies, but on the contrary, the effect appeared to be greater in small companies and ventures, which are mostly focused on R&D. Based on these results, it is necessary to re-adjust the investment strategies of government R&D on new drug development to promote private R&D investment, in particular for large and medium-sized companies with R&D capability.
From analyzing the pipelines of innovative pharmaceutical companies(46) with government grants, it was shown that the pipeline of new drug is exclusively focused on synthetic drug and biopharmaceutical. Moreover, the pipeline of IMD(synthesized) was in the highest proportion among entire pharmaceuticals products. In our country, the number of new drug pipelines appeared to be very small compared to that of the United States so it requires that R&D investment should be increased in private sector and the pipeline of new drug should be improved in quantitative and qualitative way. The comparative analysis of the investment portfolios between government and private R&D was conducted in terms of development stage, drug type, disease type. As a result, private R&D showed similar ratios of investment between discovery and clinical stages. It is likely to show a lot of investment in the early stage of drug development due to low R&D cost. In the case of government R&D, the investment ratio of discovery stage is 38.7 %, which is the highest through all stages. But the proportion of investment to support the pharmaceutical companies, primarily given to clinical stage, found in significantly higher levels as 31.9%. From these results, it seems to require a role adjustment in accordance with the R&D investment, considering that government leads to be responsible for basic research including target excavation and private sector should be led after the development phase. By institution, universities and research institutes carried out target excavations until non-clinical stage and industries accomplished from candidates generation to clinical stage. It seems that some extent of role sharing exists between academia and industries. For pharmaceutical companies, government R&D supported over the entire period of drug development. Among the government funding for pharmaceutical companies in 2014, 64.1% of the funding were given to innovative pharmaceutical companies with R&D capabilities. Moreover, 95.6% of medium-sized companies with government grants appeared to be innovative pharmaceutical company. By enterprise types, the government support was made from non-clinical to clinical stage for large companies and from candidates generation to clinical stage for medium/small companies and ventures. Particularly for SME and ventures, 50% of the investments were made from non-clinical to clinical stage.
Overall, government grant has no effect on stimulating private R&D investment for large- and medium-sized companies with R&D capabilities. This result shows that there is a need for incentives to invest actively by pharmaceutical companies after clinical stage. The government R&D needs to be switched in the direction of supporting basic research by gradually, according to increasing the investment share of the private sector by reinvesting revenues of large- and medium-sized companies. For SME and ventures with higher proportion of R&D and related basic technologies, government should support for clinical trial of a valid candidate, which leads to the increase in R&D investment of pharmaceutical companies in private sector and reinforcement of their competencies. Since the government budget is limited to support all stages of new drug development, industry should be connected with academia and institutes for participating in the development of new drug efficiently and in the long term, it is necessary to build the system of voluntary coordination and sharing competencies among civilians in new drug development.
