Background and Purpose: Recently, the prevalence of heart failure has been increased dramatically because of rapid rise of life expectancy and the prevalence of ischemic heart disease. The Korean Acute Heart Failure registry(KorAHF) aims to evaluate the clinical characteristics, management, hospital course, and long-term outcomes of patients hospitalized for acute heart failure syndrome (AHFS) in Korea. Methods and Results: Patients hospitalized for AHFS in 10 tertiary university hospitals across the country have been consecutively enrolled since March 2011. This enterprise is supposed to be carried out for eight years and this report is for 2013 to 2015. 1. We enrolled total 5625 subjects and finally closed the enrollment. 2. The cause of in-hospital death was adjudicated by independent event committee. 3. We had independent data management and audit team who has checked the quality of data and improved it. 4. We gathered follow-up data of at least 1 year. 5. We published our results of interim analysis of 2,066 patients in European Journal of Heart Failure, which has the highest impact factor in heart failure field. 6. Our analysis reveals the characteristics of AHFS in Korea as below. - Ischemic heart disease was the leading cause of heart failure (37%), followed by cardiomyopathy (21%), valvular heart disease (14%), tachycardia induced heart failure (11%), and hypertensive heart failure (3.9%). - The most frequent aggravating factor of heart failure was ischemia (26%), tachyarrhythmia including atrial fibrillation (21%), infection (20%), poor compliance such as high salt diet (8%), and renal failure (5%). - The median length of hospital stay was 9 days and mean cost for an admission was about 9,700 U.S. dollars. In hospital mortality was 4.8% and 1.2% of patients received heart transplantation. Post-discharge 90-day mortality was 8.7% and readmission rate was 14%. - The most important prognostic factors of in-hospital mortality of AHFS were renal failure and low blood pressure at admission. Conclusions: We established a registry for AHFS in Korea which has high quality data about the characteristics, management, hospital course, and outcomes of patients hospitalized for AHFS. Our analysis reveals that the prognosis of AHFS in Korea is poor and that there are specific features, including lower blood pressures at admission and lower rates of heart failure related to hypertension, compared with other registries. Adherence to current guidelines should be improved.
