□ 결론
○ 본 연구의 목적은 국비진료대상자의 의료이용에 불필요한 의료이용이 있는지
그리고 있다면 불필요한 의료이용을 억제할 수 있는 의료이용적정화 방안을
모색하는 것이었음.

□ 결론
○ 본 연구의 목적은 국비진료대상자의 의료이용에 불필요한 의료이용이 있는지

○ 질병별 단순비교분석에서 질병별 외래의료이용량과 입원의료이용량은 국비진료
대상자가 다른 비교집단에 비해 1위나 2위를 차지하는 경우가 많았지만, 연령,
상병수, 이용 의료기관, 상이여부 등의 변수를 통제한 후 국비진료대상자의 질병별 외래의료이용량과 입원의료이용량은 4개 집단 중에서 3위나 4위에 위치하는 경우가 많은 것으로 분석됨. 따라서 국비진료대상자가 다른 집단과 비교해서 상대적으로 불필요한 의료이용이 많지 않다는 것임.
○ 이러한 결과는 국비진료대상자들의 경우 다른 비교집단에 비해 상대적으로 장애가 있고, 앓고 있는 질병이 많은 등 상대적으로 건강상태가 좋지 않기 때문에 의료이용량이 많은 것으로 판단됨. 따라서 다른 집단(감면대상, 건강보험,
의료급여)대상자와 비교해서 상대적으로 불필요한 의료이용이 더 적다고 할 수
있음.
□ 정책방향
○ 이러한 연구결과를 고려할 때 본 연구에서 검토한 ‘의료비 선지급제도 및 본인부담금제도’는 투입(예산)대비 효과(의료비 절감)측면에서 효과적이지 않은
것으로 판단되며, 그 외 현재 건강보험과 의료급여에서 실시하고 있는 중복투약 제한 제도나 질병별 또는 만성질환자 중심 주치의 제도도입, 고액진료비 등
의료이용량이 많은 환자를 대상으로 한 사례관리 확대방안, 의료이용 및 건강
교육강화, 예방사업 및 퇴원후 관리강화 등 다양한 방안의 검토가 필요할 것으로 판단됨. 본 연구는 국비진료대상자의 의료이용에 초점을 두었지만 감면진료
대상자의 의료이용량과 위탁의료기관에서 보훈대상자들의 의료이용량이 크게
증가하고 있어 이에 대한 추가적인 연구가 필요할 것으로 판단됨.

This study, based on literature review and a telephone survey, was intended
to look into how much, if any, unnecessary care was being provided to
“government-subsidized medical care recipients” and seek ways to rationalize
their use of medical services. In terms of both per capita inpatient
This study, based on literature review and a telephone survey, was intended
to look into how much, if any, unnecessary care was being provided to
“government-subsidized medical care recipients” and seek ways to rationalize
their use of medical services. In terms of both per capita inpatient and
outpatient days and per capita health expenditure, government-subsidized
medical care recipients are found to have spent more than their comparison
group—those entitled to reduced or waived service fees, National Health
Insurance enrollees, and medical aid recipients. This is attributable to the
relatively poorer health status of government-subsidized medical care
recipients, among whom disease and disability are more prevalent than among
their comparison group, and who therefore are assumed to use more health
care resources. When such variables as “age”, “number of disease”, “type of
medical provider”, and “presence of in-the-line-of-duty disability” are
controlled and the conditions equalized, however, both the adjusted number of
inpatient and outpatient days and the adjusted expenditure per person were
found to be lower for government-subsidized medical care recipients than for
their comparison group. This shows to some extent that unnecessary use of
health resources is less likely among government-subsidized medical care
recipients. The outcomes imply that “advance payment” and “out-of-pocket
payment”, both of which this study examines in depth, are not cost-efficient
and that other methods—such as “designated doctor system” (for persons with
particular disease or particular chronic illnesses), improved case management
for those who tend to use a lot of health care (in particular, high-cost care
services), strengthening of education on health and health care use, disease
prevention programs, and after-discharge health management—should be
further considered.
