1. 연구개발 최종 목표
암과 난치성질환의 치료를 위해 면역 불균형 기전의 이해가 매우 필수적임. 이들 환자의 면역 항상성을 담당하는 자연 살상 (NK) 세포와 조절 T (Treg) 세포의 역할 및 면역세포들 간의 상호작용을 연구하고, 면역불균형의 발생기전과 이들 세포의 활성/불활성화에 대한 분자적인 기초를 탐구하여 보다 효과적인 병용 면역치료법을 개발함.
2. 주요 연구개발 내용
1단계
● 마우스 멜라노마 종양모델을 확립하여 암 형성 및 진행과정에 따른 면역세포의 추이 및 NK 세포와 Treg세포의 활
1. 연구개발 최종 목표
암과 난치성질환의 치료를 위해 면역 불균형 기전의 이해가 매우 필수적임. 이들 환자의 면역 항상성을 담당하는 자연 살상 (NK) 세포와 조절 T (Treg) 세포의 역할 및 면역세포들 간의 상호작용을 연구하고, 면역불균형의 발생기전과 이들 세포의 활성/불활성화에 대한 분자적인 기초를 탐구하여 보다 효과적인 병용 면역치료법을 개발함.
2. 주요 연구개발 내용
1단계
● 마우스 멜라노마 종양모델을 확립하여 암 형성 및 진행과정에 따른 면역세포의 추이 및 NK 세포와 Treg세포의 활성화/불활성화 기작을 분자 및 세포레벨에서 연구하고 NK 세포 기능의 활성화를 조절 할 수 있는 리간드 및 세포 수용체들을 발굴하였음.
● 정상인과 암환자의 혈액에서 5-10% 존재하는 NK 세포를 고가의 장비나 특별한 사이토카인을 첨가하지 않고 단지 한 종류의 방사선 조사된 leukemia feeder cell (KL-1)을 사용하여 85-95% 순도의 강력한 항암 면역 NK 세포를 획득하는 방법을 개발하였음.
● NK 세포 활성화 및 불활성화에 요구되는 세포 분자적 메카니즘을 규명하였고 정상인의 NK 세포 활성화 조절자를 개발하였음.
● in vitro에서 배양한 antigen-specific Treg이 제 1형당뇨를 억제하는 결과를 발표하고 비임상 모델에서 Treg세포를 확장 배양하는데 성공하였음.
● 1단계에서 수행한 기초 및 비 임상연구를 기반으로 항암 및 이식면역 기초연구에 국한 하지 않고 실제 환자에 응용할 수 있는 translational researh platform을 개발하였음.
2단계
● Mouse/human NK 세포와 다른 면역세포들 간의 상호작용에 관한 연구를 기반으로 기 구축한 “NK 세포치료제”의 활성화 메카니즘을 분석하였고 나아가 암과 난치성질환에 적용 가능한 임상진입가능 세포치료제의 제반 플랫폼을 구축하였음.
● 건강한 공여자와 환자의 혈액을 사용한 NK 세포 확장법의 최적화, 표준배양공정 및 품질 분석과 동물효력시험법을 확립하고, 주입한 NK 세포의 생존률과 암부위로의 trafficking을 증대시키기 위한 유전자 도입법을 개발하였음.
● 마우스 동소 이식 모델을 구축하였고 이를 사용하여 NK 세포치료제의 효력 평가를 하였으며 NK 세포와 암 특이적 T 세포와 병용투여 효과를 검증하였음.
● Treg 세포 치료제 작용기전 연구를 통해 Autoimmune Diabetic NOD 당뇨모델에서 Treg 세포치료제의 효용성을 검증하고, 세포 치료제의 작용 기전에 관하여 연구함으로써 임상화를 위한 프로토콜을 개선하였음.
3단계
● NK 세포의 증식과 활성화를 조절하는 신규인자를 발굴하기 위하여 대조군과 함께 마이크로어레이를 수행하여 NK 세포 특이적 분화관련 인자와 증식조절 인자들의 ontology를 분석함.
● 마이크로 어레이 결과, 세포막, 세포질 발현단백질, 사이토카인 등 관련 특이분자 (LDLR, CD160, IL-21, CD30L, IL-8, CEACAM-1 etc.)를 발굴하였고, NK expansion과의 연관성에 대한 연구를 수행하였음.
● 체내 생존능이 증대된 NK 세포 치료제를 개발하기 위하여 다양한 활성유전자 co-stimulatory ligand (CD40, CD80, LIGHT)와 IL-15, IL-21, IL-7 등 사이토카인을 도입하여 배양 효율을 증대시킴.
● 자가 및 동종 NK 세포와 항암면역 T 세포, 조절 T 세포 상호작용 연구를 통한 병용치료 regimen를 확립하였고, 임상진입을 위한 NK 세포 기반 병용 면역 치료제 생산법 확립하였음.
본 과제를 수행하며 세포치료분야에서 새로운 기술 및 현상, 메카니즘들을 발굴하였고, 이들을 우수 논문으로 발표하였음. (상위 5% 국제 전문 학술지(SCI) 13편, 상위 10% 국제 전문 학술지 2편 포함 총 46편 게재). 또한 특허 출원 16건 (국내 12건, 해외 4건), 특허 등록 16건을 이루었고, ㈜엔케이맥스에 기술이전 1건 성공 (기술료: 12억) 및 그에 따른 사업화를 추진하였음.
3. 기대효과
● 암과 난치성 질환의 치료를 위한 새로운 세포치료제 개발로 지적재산권 및 특허 획득, 국제논문게재를 통하여 종양 및 장기 이식 면역분야에 탁월한 공헌을 하였고, 앞으로도 지속적인 성과가 예측됨.
● 장기간 생존 가능한 NK 세포 확장법의 개발로 원천기술을 확보하고 NK 세포와 항암제 및 면역조절제, 그리고 바이러스 제제 등의 병용치료법의 개발로 국제 특허 획득과 세포 치료제 시장의 우위 선점을 기대함.
● 개발된 “항암 NK 세포 확장법”은 효능, 비용 및 배양방법 면에서 기존에 알려진 면역치료요법에 비해 탁월한 우수성을 지니고 있으며 본 연구팀의 독자 기술로써 불필요한 외국 로얄티 지불을 삭감하고 대상 환자군의 범위를 대폭확대 할 수 있어서 많은 사람들에게 치료 기회를 제공할 수 있음.
● 본 연구는 기초연구를 기반으로 하는 translational research로서 항암 및 이식 면역학분야에 학문적인 기여를 하는 동시에 인력배출 및 인력교류를 통한 국제경쟁력을 확보하고 특허획득을 통한 기술 이전 등으로 고 부가가치를 생성할 수 있을 것으로 사료됨.

Understanding the mechanisms of immune imbalance extremely essential to the treatment of cancer and refractory diseases. Development of more effective combination immunotherapies by studying the roles of NK and regulatory T cells (Treg) responsible for immune homeostasis of these patients and the interactions among these immune cells, and by exploring generation mechanisms of immune imbalance and molecular basis for activation/inactivation of these cells.
Research Contents
1st Stage
● Studies of molecular and cellular mechanisms underlying activation/inactivation of NK cells and Treg cells as wll as transition of immune cells following tumorigenesis and tumor progression, and discovery of ligands and cell receptors able to regulate activation of NK cell functions by establishing mouse melanoma tumor model.
● Development of method to acquire potent anti-tumor NK cells by expanding NK cells, which comprise 5-10% of cells in blood of normal persons and cancer patients, up to 85-95% purity using only one kind of irradiated leukemia feeder cell (KL-1) without usage of expensive instruments or addition of special cytokines.
● Identification of molecular and cellular mechanisms required for activation/inactivation of NK cells and development of reculators to activate NK cells in normal persons.
● Annonucement of results of in vitro cultivated antigen-specific Treg to suppress type 1 diabetes and success in expanding Treg cells in non-clinical model.
● Development of translational research platform not limited only to basic research for anti-tumor and transplantation immunity but also applicable to actual patients through the basic and non-clinical studies accomplished at the 1st stage
2nd Stage
● Analysis of activation mechanisms of the established NK cell therapy based on studies for interactions between mouse/human NK cells and other immune cells and furthermore establishment of overall platform of cell therapy able to enter into clinical stage applicable to cancer and other refractory diseases,
● Optimization of NK cell expansion protocols using blood of healthy donors and patients, establishment of standard cultivation process, quality analysis and efficacy test protocols in animals, and development of transfection protocols of genes to increase survival rate of transferred NK cells and their trafficking around tumors
● Establishment of mouse orthotopic transplantation model and by employing it, evaluation of efficacy of NK cell therapy and verification of efficacy of combinatory administration of NK cells and tumor specific T cells.
● Verification of efficacy of Treg cell therapy in diabetes model of autoimmune diabetic NOD though studying mechanisms of action of Treg cell therapy, and improvement of protocols for clinical application by studying mechanisms of action of cell therapy.
3rd Stage
● Analysis of ontology of NK cell specific differentiation and proliferation control related genes through performing microarray analysis along with control group to identify novel factors to regulate growth and activation of NK cells.
● Discovery of relevant specific molecules (LDLR, CD160, IL-21, CD30L, IL-8, CEACAM-1 etc.) such as cell membrane and cytosolic proteins, and cytokines though analysis of microarray data, and conducting of studies on their involvements in NK cell expansion.
● Increases of expansion efficiency by introducing various activation genes, co-stimulatory ligands (CD40, CD80, LIGHT), and cytokines such as IL-15, IL-21 and IL7 to manufacture NK cell therapeutic agents with enhanced in vivo persistence.
● Identification of correlations among cell senescence, apoptosis and autophagy through protein expression analysis and suggestion of methods to overcome in vivo persistence though introducing senescence suppressor.
● Establishment of regimen for combination therapy through studying interactions among autologous and allogeneic NK cells, anti-tumor immune T cells and regulatory T cells and establishment of production methods for NK cell based combination immune therapeutic agents to enter into clinical trials.
Discovery of novel technology, phenomenon and mechanisms in the cell therapy field by performing this grant proposal and their publications in excellent journals. (Total 46 publications including 13 publications in top 5% international journals (SCI) and 2 publications in top 10% international journals). In addition, 16 patent applications (12 domestic and 4 international patents), 16 patent registrations, 1 technology transfer to NK max company (royalties: 1200 million won) and the following advances in relevant businesses.
Expected Research Outputs
● Expectation of excellent contribution to the fields of tumor and organ transplantation and continued achievements in the future through acquisition of interlectual property and patent, and publications in international journals by developing novel cell therapies to treat cancer and refractory diseases.
● Securement of soruce technology by developing expansion methods of long-lived NK cells, and expectation of international patent acquisition and dominant preempt in cell therapy market by developing combination therapies such as NK cells, anti-cancer drugs, immunomodulatory agents and viral agents.
● Verification of excellent superiority of the developed "anti-tumor NK cell expansion protocol" compared to the previously known immune therapeutics in terms of efficacy, cost and expansion method, and provision of treatment opportunities to many more patients by reducing unnecessary foreign royalty payments with our group's proprietary technology and by significantly expanding the ranges of patient groups for treatment.
● This study is expected to scholastically contribute to the fields of anti-tumor and transplantation immunity as translational research based on basic science, to acquisition of international competitiveness through education and exchange of relevant people and to production of high values such as technology transfer through acquiring patents.
