○ 2단계 연구 : 난임상담 실태조사 및 요구도 조사
- 난임여성 1,252명을 대상으로 조사한 결과 난임상담 이용률은 3.9%로 미미한 수준임.
- 난임여성들은 주로 난임시술과 관련된 스트레스 대처(45.7%)와 부정적 감정해소(21.4%)를 위해 상담을 받았으며, 난임전문 산부인과의사에게 44.7%, 난임전문상담사에게 31.9%가 상담을 받음. 대부분 시술병원(71.7%)에서 상담을 받았으며, 난임상담 시범 기관인 인구보건복지협회에서는 15.2%가 상담받음.

○ 3단계 연구 : 난임상담 전달체계 개발
- 난임상담 전달체계 구축을 위한 기본 방향으로 난임치유과정 촉진도모, 수요자 중심의 전달체계구축, 난임여성에 대한 의료적 지원과 심리사회적 지원의 통합 모델지향, 단계적 지원체계의 개발, 효과성 지표 제안, 난임상담 사업의 중장기적 플랜을 제시함.

- Through FGI, a total of four dimensions of psychological difficulties of subfertile women were derived (emotion, marital relationship, family relationship, social relationship). In the emotional dimension, a total of five subcategories -'negative emotion', 'self-guilt/guilt/feeling inferior','excessive expectation and dissatisfaction following it,' 'loss of control’ and 'difficulties of everyday life’ - were derived. In the aspect of marital relationship, two subcategories of 'marital conflict due to difficulty' and 'distance from husband' were derived. In the aspect of family relations, there were three subcategories of 'failing to disclose subfertility/not being understood', 'burden and pressure from family' and 'getting hurt from family reactions.’ Three subcategories of 'suffering experienced in social relations', 'suffering from excessive interest of people or lack of social awareness', and ‘distressful exposure to babies in social relations' were derived. In total, 13 subcategories were derived.
- On the survey of 1,609 women, it was demonstrated that 29.3% were high risk depressed patients who needed infertility counseling, and 26.5% were high risk anxiety patients and 28.8% were high risk stress group (DASS≥ moderate).

○ Investigation on the current status and demands of infertility counseling
- According to survey conducted on 1,252 women, the utilization rate of counseling was 3.9%. The counseling focused mostly on stress coping for subfertility treatments (45.7%) and reducing negative moods (21.4%). Most of them received counseling from subfertility specialist obstetricians (44.7%), and subfertility specialist counselors (31.9%). Most received counseling at the medical hospitals (71.7%), and 15.2% received counseling at the Population Health and Welfare Association, which is an organization for the demo infertility counseling. The cost of counseling was free for 59% of the cases, and 82.5% who received counseling reported that they were satisfied with infertility counseling.
- 88.5% of subfertile individuals reported a high demand for infertility counseling, and the respondents reported that they needed counseling especially when they experienced difficulty after failures of several medical procedures (21.8%), when the outcome was poor (19.5%), and when they experienced difficulty in pregnancy before treatment (18.6%). 44.9% requested that the counseling fee should be free because of financial burden, and 51.1% of respondents said that the fee of less than 30,000 won would be appropriate.
- The implementation rate of infertility counseling was 36% in the top 50 subfertility medical treatment institutes. The counseling was comprised of subfertility education (31%), and individual counseling (27%). Counseling programs were conducted mostly by doctors (40%) and nurses (40%) and most of them were free of charges. The responses for government subsidy needed to activate the infertility counseling were for a counseling agency (26%), training and provisioning professional staffs such as subfertility specialty counselors (26%), educating hospital staffs (26%), financial support (16%), and quick guide and promotion (6%), respectively.
○ The development of infertility counseling delivery system
- The proposed infertility counseling delivery system is to promote the infertility treatment process, to build a user-friendly delivery system, to incorporate the integrated model of medical support and psychological support for the subfertile women and to develop a stepwise care system.
- The main recipient of infertility counseling will be female(couple) who is/are diagnosed as subfertile.
- Based on the findings of this study, 60,000 out of 200,000 subfertile women are estimated to be in demand of infertility counseling. According to the needs of the subfertile women, the timing of the intervention is likely to be from the time of diagnosis of subfertility until the end of the medical procedure.
- According to the proposed delivery system, all recipients will be provided with infertility counseling regardless of their income level, up to 4 times a month, for a period of 12 months of service (extended for one more year through re-examination). Therefore, the maximum number of counseling that an individual can receive is 96 times.
- The infertility counseling facilities will include the central infertility counseling support center and regional (SeoulGyeonggi, Busan, Gwangju) infertility counseling centers, and provides counseling services in cooperation with treatment specialty hospitals and nearby private centers, university counseling center, and local health centers.
- The counseling service providers would be majored in counseling and will be divided into the first and the second class depending on their counseling experience. They will provide support on a step by step basis depending on the needs of their clients. As infertility counseling is in a field of specialty counseling, it requires the counselors to acquire medical information through a standardized curriculum with a basis on the professional competency as counselors.
- Depending on the client's needs, information provision and decision making counseling, coping skills enhancing counseling, crisis intervention counseling, professional counseling and marriage/sex counseling may be provided.
- In order to promote infertility counseling, it is necessary to establish a central infertility counseling center along with three regional centers under the Ministry of Health and Welfare. Each center would be responsible for comprehensive planning, implementation, evaluation, research, and guidance/ supervision of each consulting organization. This requires staffing and budget.
- As for the financial support of infertility counseling, we suggest using the form of electronic voucher currently used in the community service investment project carried out by the ministry of health and welfare.
- The recommended fee is 80,000 won per session, which is expected to be an appropriate cost for ensuring the stable supply of professional counselors.
- We propose that “the Korean Society for infertility counseling” (tentative name) should be founded for continuous training and management of infertility counseling specialists.
